Abstract

A medical probe assembly, system, and methods for the use thereof to treat tissue are described. The system optionally comprises an energy source, two internally-cooled probe assemblies, and one or more cooling devices to provide cooling to at least one of the probe assemblies. The probe assemblies may be configured in a bipolar mode, whereby current flows preferentially between the probe assemblies. The probe assemblies and system described herein are particularly useful to deliver radio frequency energy to a patient'"'"'s body. RF energy delivery may be used for various applications, including the treatment of pain, tumor ablation and cardiac ablation.

Claims

1. A method of treating spinal tissue of a patient'"'"'s body, the method comprising:

providing a system comprising an energy source and first and second internally-cooled probe assemblies, wherein each of the probe assemblies comprises an electrically conductive energy delivery device electrically coupled to said energy source;

inserting the energy delivery devices of said first and second internally-cooled probe assemblies into spaced-apart treatment sites for said spinal tissue;

delivering stimulating energy through said energy delivery devices for determining proximity of at least one of said energy delivery devices to a neural structure; and

delivering energy from said energy source to said spinal tissue through said energy delivery devices to treat said spinal tissue;

wherein said stimulating energy is capable of eliciting a response from said neural structure without damaging said neural structure and wherein delivering stimulating energy comprises delivering said stimulating energy preferentially between said energy delivery devices in a bipolar manner.

2. The method of claim 1, wherein the step of inserting said energy delivery devices further comprises a step of inserting at least one

introducer tube into a patients body and wherein said energy delivery devices are inserted into the treatment sites through a bore of said at least one introducer tube.

3. The method of claim 1, wherein the step of inserting said energy delivery devices comprises utilizing an aid to assist in positioning at least one of said energy delivery devices.

4. The method of claim 3, wherein said aid is selected from the group consisting of fluoroscopic imaging, impedance monitoring and tactile feedback.

5. The method of claim 1, further comprising the steps of:

providing an apparatus operable to deliver a cooling means to one or more probe assemblies when said one or more probe assemblies is located at a treatment site in a patients body; and

delivering said cooling means to at least one of said first and second probe assemblies while said energy delivery devices are located at said spaced-apart treatment sites.

6. The method of claim 1, further comprising a step of at least one of inserting material to and removing material from said spinal tissue, wherein said step of at least one of inserting and removing material is performed at least one of before and after the delivery of energy.

7. The method of claim 6, wherein the step of inserting material comprises injecting a treatment composition into said spinal tissue and wherein said treatment composition is selected from the group consisting of an anesthetic, a dye and an antibiotic.

8. The method of claim 1, further comprising a step of providing at least one additional electrically conductive component coupled to said energy source and operable to transmit energy between said energy source and said patient'"'"'s body.

9. The method of claim 8, further comprising a step of controlling a flow of energy between at least two electrically conductive components selected from the group consisting of said first probe assembly, said second probe assembly and said at least one additional electrically conductive component.

10. The method of claim 9, wherein said step of controlling a flow of energy comprises altering an electrical impedance between a current sink and at least one electrically conductive component selected from the group consisting of said first probe assembly, said second probe assembly and said at least one additional electrically conductive component.

11. The method of claim 1, wherein said spinal tissue is selected from the group consisting of an intervertebral disc, spinal neural tissue and a vertebra or portions thereof.

12. The method of claim 1, wherein said energy source is an electrical generator and wherein the step of delivering energy comprises delivering electrical current in a radio frequency range.

13. The method of claim 1, wherein said energy delivery devices are operated in a bipolar mode, whereby energy delivered from said energy source to treat said spinal tissue is preferentially concentrated between said energy delivery devices.

14. The method of claim 1, wherein said system further comprises a controller operable to control at least one aspect of a treatment protocol selected from the group consisting of energy delivery and cooling supply and wherein said controller is operable to control said at least one aspect based on at least one of a temperature measurement, an impedance measurement and an error signal.

15. The method of claim 14, further comprising a step of measuring a treatment parameter selected from the group consisting of temperature and impedance and altering said at least one aspect of a treatment protocol based on a measured treatment parameter.

16. The method of claim 1, wherein said internally-cooled probe assemblies each comprise:

an elongate member having a distal region and a proximal region and defining a lumen therebetween;

an energy delivery device associated with said distal region of said elongate member, said energy delivery device comprising a protrusion; and

a temperature sensor associated with said protrusion.

17. The method of claim 1, wherein said internally-cooled probe assemblies each comprise at least two tubular members disposed within said lumen for delivering a cooling fluid to and removing a cooling fluid from said energy delivery devices.

18. A method of treating spinal tissue of a patient'"'"'s body by delivering energy from an energy source to said patient'"'"'s body, the method comprising the steps of:

providing first and second internally-cooled probe assemblies and at least one additional electrically conductive component, wherein each of the probe assemblies comprises an electrically conductive energy delivery device electrically coupled to said energy source;

delivering energy from said energy source to spaced-apart treatment sites for said spinal tissue through said energy delivery devices; and

altering an electrical impedance between a current sink and at least one electrically conductive component selected from the group consisting of said first probe assembly, said second probe assembly and said at least one additional electrically conductive component;

whereby altering an electrical impedance serves to adjust a flow of energy between at least two electrically conductive components selected from the group consisting of said first probe assembly, said second probe assembly and said at least one additional electrically conductive component.

19. A method of treating spinal tissue of a patient'"'"'s body, the method comprising:

providing a system comprising an energy source and first and second internally-cooled probe assemblies, wherein each of the probe assemblies comprises an electrically conductive energy delivery device electrically coupled to said energy source;

providing at least one additional electrically conductive component coupled to said energy source and operable to transmit energy between said energy source and said patient'"'"'s body;

inserting the energy delivery devices of said first and second internally-cooled probe assemblies into spaced-apart treatment sites for said spinal tissue;

delivering energy from said energy source to said spinal tissue through said energy delivery devices; and

controlling a flow of energy between at least two electrically conductive components selected from the group consisting of said first probe assembly, said second probe assembly and said at least one additional electrically conductive component.

20. The method of claim 19, wherein said step of controlling a flow of energy comprises altering an electrical impedance between a current sink and at least one electrically conductive component selected from the group consisting of said first probe assembly, said second probe assembly and said at least one additional electrically conductive component.
