Abstract

A device and method for cardiac rhythm management in which a cardiac site is paced in accordance with a pacing mode that employs sense signals from a different cardiac site. A protective period triggered by the sensing of intrinsic activity at the paced site is used to delay pacing by a protective delay interval without otherwise disturbing the pacing algorithm.

Claims

1. A method for operating a cardiac rhythm management device, comprising:

pacing a first ventricle in accordance with a pacing mode in which the first ventricle is paced at a first specified pacing instant defined with respect to expiration of an escape interval which is reset by an event in the contralateral second ventricle;

inhibiting a pace to the first ventricle during a ventricular protective period that is initiated by a first ventricular sense; and

,

delaying a pace to the first ventricle by a protective delay interval when the pace would otherwise occur within the ventricular protective period.

2. The method of claim 1 wherein the first ventricle is the left ventricle and the second ventricle is the right ventricle.

3. The method of claim 2 wherein the pacing mode is a biventricular pacing mode where the paces to the right and left ventricles are separated by a specified offset interval.

4. The method of claim 2 wherein the pacing mode is a left ventricle-only pacing mode with the left ventricle being paced upon expiration of an escape interval which is reset by a right ventricular sense.

5. The method of claim 1 wherein the first ventricle is also paced when triggered by a second ventricular sense unless the pace to the first ventricle would be delivered within the ventricular protective period, in which case the pace to the first ventricle is delayed by the protective delay interval.

6. The method of claim 1 wherein the protective delay interval is a fixed value.

7. The method of claim 1 wherein the protective delay interval is a variable interval that delays the pace to the first ventricle only enough to fall outside of the ventricular protective period.

8. The method of claim 1 wherein the variable protective delay interval is limited to a specified maximum value.

9. The method of claim 1 wherein the delayed pace to the first ventricle is inhibited if it would still fall within the ventricular protective period.

10. The method of claim 9 wherein a pace is delivered to the second ventricle if the pace to the first ventricle is inhibited.

11. A cardiac rhythm management device, comprising:

sensing channels for sensing a first ventricle and second ventricles and generating sense signals upon detection of depolarization occurring in a ventricle;

a pacing electrode adapted for delivering paces to the first ventricle;

a controller for controlling the delivery of paces in accordance with a programmed pacing mode; and

,

wherein the controller is programmed to;

pace the first ventricle such that the first ventricle is paced at a first specified pacing instant defined with respect to expiration of an escape interval which is reset by an event in the contralateral second ventricle;

inhibit a pace to the first ventricle during a ventricular protective period that is initiated by a first ventricular sense; and

,

delay a pace to the first ventricle by a protective delay interval when the pace would otherwise occur within the ventricular protective period.

12. The device of claim 11 wherein the first ventricle is the left ventricle and the second ventricle is the right ventricle.

13. The device of claim 12 wherein the pacing mode is a biventricular pacing mode where the paces to the right and left ventricles are separated by a specified offset interval.

14. The device of claim 12 wherein the pacing mode is a left ventricle-only pacing mode with the left ventricle being paced upon expiration of an escape interval which is reset by a right ventricular sense.

15. The device of claim 11 wherein the controller is programmed such that the first ventricle is also paced when triggered by a second ventricular sense unless the pace to the first ventricle would be delivered within the ventricular protective period, in which case the pace to the first ventricle is delayed by the protective delay interval.

16. The device of claim 11 wherein the protective delay interval is a fixed value.

17. The device of claim 11 wherein the protective delay interval is a variable interval that delays the pace to the first ventricle only enough to fall outside of the ventricular protective period.

18. The device of claim 11 wherein the variable protective delay interval is limited to a specified maximum value.

19. The device of claim 11 wherein the delayed pace to the first ventricle is inhibited if it would still fall within the ventricular protective period.

20. The device of claim 19 wherein the controller is programmed such that a pace is delivered to the second ventricle if the pace to the first ventricle is inhibited.
