Abstract

An apparatus for and method of measuring pressure through a septum in a patient'"'"'s heart is disclosed. A lead inserted into the right side of a heart is routed through the septum to gain access to the left side of the heart. The lead includes an attachment structure that secures the lead to one or both of the septal walls. The attachment structure may include at least one protruding tine, membrane, inflatable balloon, involuted spiral or J-lead that engage one or more sides of the septum. The lead also includes one or more sensors for measuring cardiac pressure on the left side of the heart and, as necessary, the right side of the heart.

Claims

1. A lead configured for measuring pressure in a heart comprising:

a lead body having a proximal portion and a distal portion;

a connector assembly at the proximal portion of the lead body, the connector assembly to couple with an implantable cardiac device having circuitry that process signals to determine cardiac pressure;

an adjustable attachment structure attached to the distal portion of the lead body and adapted to attach the lead body to a heart wall through which the lead body passes by adjusting to a thickness of the heart wall;

a tensile member coupled to the adjustable attachment structure, wherein longitudinal movement of the tensile member slidingly moves the adjustable attachment structure relative to the lead body in a longitudinal direction; and

at least one pressure sensor attached to the lead body to measure pressure at a distal side of the heart wall.

2. The lead of claim 1 wherein the adjustable attachment structure comprises tines spaced longitudinally along the lead body, membranes spaced longitudinally along the lead body, balloons spaced longitudinally along the lead body, or involuted spirals spaced longitudinally along the lead body.

The lead of claim 1 wherein the adjustable attachment structure comprises a first attachment structure positioned at a fixed location at a distal end of the lead body and a second attachment structure adapted to be positioned at a plurality of locations proximal to the first attachment structure, wherein the tensile member is coupled to the second attachment structure, and wherein longitudinal movement of the tensile member slidingly moves the second attachment structure relative to the lead body.

4. The lead of claim 3 wherein the first attachment structure comprises at least one tine, membrane, balloon, involuted spiral or J-lead.

5. The lead of claim 3 wherein the second attachment structure comprises at least one tine, membrane, balloon or involuted spiral.

6. The lead of claim 1 comprising at least one electrode at the distal portion of the lead body.

7. The lead of claim 6 wherein the at least one electrode is configured to pace the heart or sense signals in the heart.

8. The lead of claim 1 wherein the adjustable attachment structure comprises a plurality of structures spaced longitudinally along the lead body and having different orientations relative to a cross section of the lead body.

9. The lead of claim 1 comprising at least one branch lead body wherein each branch lead body includes at least one pressure sensor.

10. The lead of claim 1 wherein the adjustable attachment structure is biased to expand outward from the lead body.

11. The lead of claim 1 comprising a sharp tip at the distal end of the lead body.

12. The lead of claim 1 comprising a lumen within the lead body, wherein the tensile member is disposed within the lumen.

13. The lead of claim 1 comprising a handle at a proximal portion of the tensile member.

14. The lead of claim 1 wherein a distal end of the lead lies relatively flat against a septal wall to reduce the likelihood of blood clots forming in a left side of the heart.

15. The lead of claim 1 comprising a lumen within the lead body, wherein the tensile member is disposed within the lumen.

16. A lead configured for measuring pressure in a heart comprising:

a lead body having a proximal portion and a distal portion;

a connector assembly at the proximal portion of the lead body, the connector assembly to couple with an implantable cardiac device having circuitry that process signals to determine cardiac pressure;

at least one adjustable attachment structure attached to the distal portion of the lead body and adapted to attach the lead body to a heart wall through which the lead body passes;

a tensile member coupled to the adjustable attachment structure, wherein longitudinal movement of the tensile member slidingly moves the adjustable attachment structure relative to the lead body in a longitudinal direction;

at least one pressure sensor attached to the lead body to measure pressure at a distal side of the heart wall; and

at least one branch lead body extending from the lead body, each branch lead body comprising at least one pressure sensor.

17. The lead of claim 16 wherein the at least one adjustable attachment structure comprises at least one tine, membrane, balloon, involuted spiral or J-lead.

18. The lead of claim 16 wherein the at least one adjustable attachment structure comprises tines spaced longitudinally along the lead body, membranes spaced longitudinally along the lead body, balloons spaced longitudinally along the lead body or involuted spirals spaced longitudinally along the lead body.

19. The lead of claim 16 wherein the at least one adjustable attachment structure comprises a plurality of attachment structures having different orientations relative to a cross section of the lead body.

20. The lead of claim 16 wherein the at least one adjustable attachment structure is biased to expand outward from the lead body.

21. The lead of claim 16 comprising a sharp tip at the distal end of the lead body.

22. The lead of claim 16 comprising a lumen within the lead body, wherein the tensile member is disposed within the lumen.

23. The lead of claim 16 wherein a distal end of the lead lies relatively flat against a septal wall to reduce the likelihood of blood clots forming in a left side of the heart.

24. The lead of claim 16 comprising a handle at a proximal portion of the tensile member.

25. The lead of claim 16 wherein the adjustable attachment structure comprises a first attachment structure positioned at a fixed location at a distal end of the lead body and a second attachment structure adapted to be positioned at a plurality of locations proximal to the first attachment structure, wherein the tensile member is coupled to the second attachment structure, and wherein longitudinal movement of the tensile member slidingly moves the second attachment structure relative to the lead body.
