Abstract

The invention relates to methods and devices for characterizing tissue in vivo, e.g., in walls of blood vessels, to determine whether the tissue is healthy or diseased.

Claims

1. An in vivo method for characterizing plaque composition in a blood vessel wall, the method comprising

(a) through whole blood, illuminating a tissue in the blood vessel wall with any two or more single wavelengths or one or more narrow wavelength bands of near-infrared radiation within a wavelength range of about 1100 to 1415 nm;

(b) through whole blood, detecting radiation reflected from the tissue, the radiation having a wavelength of from about 1100 to 1415 mn;

(c) processing the detected radiation to characterize the plaque composition; and

(d) providing an output indicative of the characterization of the plaque composition.

2. The method of claim 1, wherein the one or more narrow wavelength bands each span about 1.0 nm to about 100 nm within the wavelength range of 1100 to 1415 nm.

3. The method of claim 1, wherein two single wavelengths are used.

4. The method of claim 1, wherein two narrow wavelength bands, each spanning 1.0 nm to 30 nm within the wavelength range of 1100 to 1415 nm are used.

5. The method of claim 1, wherein at least one narrow wavelength band and at least one single wavelength are used.

6. The method of claim 1, wherein the wavelength range is about 1100 to 1350 nm.

7. The method of claim 1, wherein the wavelength range is about 1150 to 1250 nm.

8. The method of claim 1, wherein the wavelength range is about 1175 to 1280 nm.

9. The method of claim 1, wherein the wavelength range is about 1190 to 1250 nm.

10. The method of claim 1, further comprising illuminating the tissue in the blood vessel wall with any two or more single wavelengths or one or more narrow wavelength bands of near-infrared radiation within a second wavelength range of about 1600 nm to 1780 nm;

and further detecting radiation reflected from the tissue having a second wavelength of from about 1600 nm to 1780 nm.

11. The method of claim 10, wherein the second wavelength range is about 1650 to 1730 nm.

12. The method of claim 1, wherein the processing is done using chemometric discrimination algorithms.

13. The method of claim 1, further comprising preprocessing the detected radiation to remove spectral information not related to a characterization of the tissue.

14. The method of claim 1, wherein the blood vessel is an artery.

15. The method of claim 1, wherein the blood vessel is a coronary artery.

16. The method of claim 1, wherein the tissue comprises a lipid pool.

17. The method of claim 1, wherein the tissue comprises a lipid pool and a thin fibrous cap.

18. The method of claim 1, wherein the tissue comprises a lipid pool and a thick fibrous cap.

19. The method of claim 1, wherein the tissue comprises fibrotic or calcific tissue.

28. The method of claim 1, wherein illuminating a plaque comprises scanning the tissue with having a wavelength that varies between two wavelengths within the range of about 1100 to 1415 nm.

20. An apparatus for characterizing plaque composition within a blood vessel containing whole blood, said apparatus comprising:

a near-infrared radiation source that generates radiation for illumination of tissue through blood, the radiation comprising any two or more single wavelengths or one or more narrow wavelength bands of near-infrared radiation within a wavelength range of about 1100 to 1415 nm, the radition being sufficient to penetrate whole blood;

one or more radiation conduits for transmitting radiation from the radiation source, through whole blood, to the tissue and for receiving radiation not absorbed by the tissue;

a radiation detector that collects radiation not absorbed by the tissue across a wavelength range of substantially 1100 to 1415 nm, the radiation detector having a sensitivity sufficient to detect the radiation after the radiation has been attenuaded by whole blood;

a processor that processes the collected radiation to determine the composition of plaque in the tissue; and

an output device for providing output characterizing the plaque composition.

21. The apparatus of claim 20, wherein the near-infrared radiation source generates a wavelength range of about 1100 to 1350 nm.

22. The apparatus of claim 20, wherein the near-infrared radiation source generates two narrow wavelength bands, each spanning 1.0 nm to 30 nm within the wavelength range of 1100 to 1415 nm.

23. The apparatus of claim 20, wherein the near-infrared radiation source generates a wavelength range of about 1150 to 1250 nm.

24. The apparatus of claim 20, further comprising a near-infrared radiation source that generates radiation comprising any two or more single wavelengths or one or more narrow wavelength bands of near-infrared radiation within a second wavelength range of about 1600 to 1780 nm.

25. An instrument for in vivo characterization of plaques, the instrument comprising

a) means for illuminating plaques through whole blood, the means for illuminating using near-infrared radiation comprising any two or more single wavelengths or one or more narrow wavelength bands of near-infrared radiation within a wavelength range of about 1100 to 1415 nm;

b) means for collecting radiation through whole blood, the radiation being within the wavelength range that is not absorbed by the plaques;

c) means for determining, from the collected radiation the amounts of absorbance of radiation by the illuminated plaques; and

d) means for discriminating one illuminated plaque from another illuminated plaque within the wavelength range, wherein the discriminating means comprises

i) means for preprocessing the absorbance amounts using a chemometric preprocessing technique, and

ii) means for performing a chemometric discrimination algorithm on the preprocessed absorbance amounts to characterize the plaques; and

e) means for providing an output indicating the characterization of plaque composition.

26. A method for in vivo characterization of plaque in blood vessel walls utilizing a fiber optic probe, the method comprising:

introducing the probe into a blood vessel;

directing, throught whole blood between the probe and the wall and onto a plaque in the blood vessel wall, near-infrared radiation comprising any two or more single wavelengths or one or more narrow wavelength bands within a wavelength range of about 1100 to 1415 nm;

throught whole blood, detecting radiation not absorbed by the blood vessel wall, the detected radiation having a wavelength within the range of substantially 1100 to 1415 nm; and

analyzing the detected radiation to categorize the plaque in the blood vessel wall.

27. An in vivo method for characterizing a plaque in a blood vessel wall, the method comprising:

(a) through whole blood, illuminating a plaque in the blood vessel wall with any two or more single wavelengths of near-infrared radiation within a wavelength range consisting of wavelengths from about 1100 nm to about 1415 nm;

(b) detecting, through whole blood, radiation reflected from the plaque, the reflected radiation having a wavelength of from about 1100 to 1415 nm;

(c) processing the detected radiation to characterize the plaque in the blood vessel wall; and

(d) providing an output indicating the plaque characterization.

29. An in vivo method for characterizing plaque composition of a plaque in a blood vessel wall, the method comprising:

(a) through whole blood, illuminating a tissue in the blood vessel wall with any two or more single wavelengths of near-infrared radiation from within a particular wavelength range;

(b) through whole blood, detecting radiation reflected from the tissue, the reflected radiation having a wavelength within the particular wavelength range;

(c) processing the detected radiation to characterize the plaque in the blood vessel wall; and

(d) providing an output indicating the characterization;

wherein the particular wavelength range consists of wavelengths between about 1100 nm and about 1415 nm.

30. An in vivo method for characterizing plaque in a blood vessel wall, the method comprising:

selecting a plurality of single wavelengths from a first band that consists of wavelengths between 1100 nm and 1415 nm;

through whole blood, illuminating a plaque on the blood vessel wall with NIR having the wavelengths selected from the first band;

through whole blood, detecting radiation reflected from the plaque, the detected radiation having a wavelength from within the first band;

processing the detected radiation to characterize the plaque; and

providing an output indicative of the plaque characterization.
