Abstract

A method and apparatus for delivering cardiac resynchronization therapy (CRT) in which an evoked response electrogram is recorded during one or more cardiac cycles and used to aid in the selection of resynchronization pacing parameters and/or to monitor the effectiveness of resynchronization therapy. The morphology of an evoked response electrogram may be recorded and analyzed to determine if and when intrinsic activation of one ventricle is occurring in order to optimally adjust the programmed atrio-ventricular (AV) delay interval for ventricular resynchronization pacing of a patient with intact AV node conduction.

Claims

1. A method for operating a cardiac rhythm management (CRM) device in a patient, comprising:

delivering paces to both right and left ventricles in accordance with a programmed biventricular pacing mode, such that the biventricular paces are delivered at a programmed biventricular offset interval and after a programmed AV delay interval with respect to an atrial event;

recording evoked response test electrograms during the biventricular pacing;

comparing the test electrograms to evoked response template electrograms representing biventricular capture, left ventricle-only capture, and right ventricle-only capture to determine the type of pacing event that has occurred during a paced cardiac cycle;

wherein,

the CRM device maintains a count of a number of instances of each type of pacing event.

2. The method of claim 1 further comprising:

comparing the test electrograms to evoked response template electrograms representing intrinsic excitation of the right or left ventricle with a pace being delivered during a refractory period to determine if that type of pacing event has occurred;

wherein the CRM device maintains a count of pacing events in which intrinsic excitation of the right or left ventricle has occurred with a pace being delivered during the refractory period.

3. The method of claim 1 further comprising:

comparing the test electrograms to evoked response template electrograms representing fusion beats where capture and intrinsic activity occur together to determine if that type of pacing event has occurred;

wherein the CRM device maintains a count of pacing events in which a fusion beat has occurred.

4. The method of claim 1 further comprising:

comparing the test electrograms to evoked response template electrograms representing no capture or excitation of either ventricle to determine if that type of pacing event has occurred;

wherein the CRM device maintains a count of pacing events in which no capture or excitation has occurred.

5. The method of claim 1 further comprising sorting the counts of each type of pacing event into separate bins according to the pacing rate that was in operation when a particular pacing event occurred.

6. The method of claim 1 further comprising triggering an alarm if a count for a particular type of pacing event or events exceeds a specified threshold.

7. The method of claim 6 further comprising triggering an alarm if a specified percentage of total counted events are not biventricular captures.

8. The method of claim 4 further comprising triggering an alarm if a specified percentage of total counted events are fusion beats.

9. The method of claim 6 further comprising initiating automatic data recording if the alarm is triggered.

10. The method of claim 6 further comprising adjusting one or more pacing parameters if the alarm is triggered.

11. A cardiac rhythm management (CRM) device, comprising:

pacing channels through which paces may be delivered to both right and left ventricles;

one or more sensing channels;

a controller programmed to deliver the paces in accordance with a programmed biventricular pacing mode at a programmed biventricular offset interval and after a programmed AV delay interval with respect to an atrial event;

wherein the controller is programmed to;

record evoked response test electrograms during the biventricular pacing;

compare the test electrograms to evoked response template electrograms representing biventricular capture, left ventricle-only capture, and right ventricle-only capture to determine a type of pacing event that has occurred during a paced cardiac cycle; and

,

maintain a count of a number of instances of each type of pacing event.

12. The device of claim 11 wherein the controller is further programmed to:

compare the test electrograms to evoked response template electrograms representing intrinsic excitation of the right or left ventricle with a pace being delivered during a refractory period to determine if that type of pacing event has occurred; and

,

maintain a count of pacing events in which intrinsic excitation of the right or left ventricle has occurred with a pace being delivered during the refractory period.

13. The device of claim 11 wherein the controller is further programmed to:

compare the test electrograms to evoked response template electrograms representing fusion beats where capture and intrinsic activity occur together to determine if that type of pacing event has occurred; and

,

maintain a count of pacing events in which a fusion beat has occurred.

14. The device of claim 11 wherein the controller is further programmed to:

compare the test electrograms to evoked response template electrograms representing no capture or excitation of either ventricle to determine if that type of pacing event has occurred; and

,

maintain a count of pacing events in which no capture or excitation has occurred.

15. The device of claim 11 wherein the controller is further programmed to sort the counts of each type of pacing event into separate bins according to the pacing rate that was in operation when a particular pacing event occurred.

16. The device of claim 11 wherein the controller is further programmed to trigger an alarm if a count for a particular type of pacing event or events exceeds a specified threshold.

17. The device of claim 16 wherein the controller is further programmed to trigger an alarm if a specified percentage of total counted events are not biventricular captures.

18. The device of claim 14 wherein the controller is further programmed to trigger an alarm if a specified percentage of total counted events are fusion beats.

19. The device of claim 16 wherein the controller is further programmed to initiate automatic data recording if the alarm is triggered.

20. The device of claim 16 wherein the controller is further programmed to adjust one or more pacing parameters if the alarm is triggered.
