Abstract

Adventitious lung sounds indicative of lung congestion are detected using an implantable sensor. The sensor is adapted to be positioned adjacent to a pulmonary system and to output signals indicative of lung sounds in response to pulmonary system activity. A controller receives the signals and processes the signals to detect the presence of adventitious lung sounds. A respiratory cycle sensor operating in conjunction with the lung-sound sensor enables classification of an adventitious lung sound according to its time occurrence within the respiratory cycle. Posture sensing in conjunction with lung-sound sensing provides valuable additional information as to the severity of the lung congestion.

Claims

1. An implantable system comprising:

an implantable housing;

a plurality of implantable sound-detecting sensors adapted to be positioned in a spaced-apart configuration with each implantable sensor at a different location adjacent to a pulmonary system and further adapted to output signals indicative of lung-sounds, at least one of the plurality of implantable sound-detecting sensors located external of the implantable housing;

at least one wireless transmitter in communication with at least one of the plurality of implantable sound-detecting sensors and configured to wirelessly transmit the lung sound-signals output by the at least one implantable sound-detecting sensor; and

a controller adapted to monitor the lung-sound signals.

2. The implantable system of claim 1, wherein the controller is located within the implantable housing.

3. The implantable system of claim 1, wherein the implantable system is configured to individually transmit the lung-sound signals respectively output from the implantable sound-detecting sensors of the plurality to an external receiving device.

The implantable system of claim 1, wherein the implantable system is configured to calculate a composite signal by mathematically combining the lung-sound signals output from two or more of the plurality of implantable sound-detecting sensors and transmit the composite signal to an external receiving device.

The implantable system of claim 1, further comprising an external receiver, the external receiver configured to receive each of the lung-sound signals transmitted by the at least one wireless transmitter, wherein the controller is coupled to the external receiver and located external of a patient.

6. The implantable system of claim 1, wherein the plurality of implantable sound-detecting sensors comprises a plurality of accelerometers adapted to output signals indicative of lung-sounds.

The implantable system of claim 1, further comprising an implantable pulse generator within the implantable housing and configured to output a cardiac pacing therapy, wherein the controller is configured to at least one of initiate cardiac therapy and increase frequency of cardiac stimulation based on identification of adventitious lung-sounds and the controller is further configured to decrease frequency of cardiac stimulation when adventitious lung sounds are no longer detected.

8. The implantable system of claim 1, further comprising:

at least one stimulating lead adapted to be positioned within the heart; and

a pulse generator adapted to output electrical pulses to the lead, wherein the controller is further adapted to control the output of the pulse generator in response to the controller detecting characteristics of the lung-sound signals.

9. The implantable system of claim 1, wherein the controller is adapted to identify adventitious lung sounds from the lung-sound signals being monitored.

10. The implantable system of claim 9, further comprising an implantable pulse generator adapted to output a cardiac pacing therapy, wherein the controller is adapted to at least one of initiate cardiac therapy and modify cardiac pacing therapy based on identification of adventitious lung-sounds.

11. The implantable system of claim 9, wherein the controller is located within the implantable housing and is further adapted to associate a degree parameter with each adventitious lung-sound identification, the degree parameter indicative of severity of pulmonary congestion.

12. A method of detecting lung sounds, comprising:

obtaining lung-sound signals indicative of lung-sounds from a plurality of implantable sound-detecting sensors respectively positioned at different spaced-apart locations adjacent to a pulmonary system, at least one of the implantable sound-detecting sensors located external of an implantable housing;

transmitting the lung-sound signals from each of the plurality of implantable sensors using circuitry within the implantable housing;

receiving the wirelessly transmitted lung-sound signals; and

analyzing the received lung-sound signals to detect respiratory events.

13. The method of claim 12, wherein receiving the transmitted lung-sound signals comprises receiving the transmitted lung-sound signals at a location external of a patient.

14. The method of claim 12, further comprising:

generating respiratory event information based on the detected respiratory events; and

transmitting the respiratory event information to an external location.

15. The method of claim 12, further comprising:

generating a combined signal by combining the lung-sound signals from at least some of the plurality of implantable sound-detecting sensors; and

transmitting the combined signal to an external location.

16. The method of claim 12, wherein the detected respiratory events comprise adventitious lung-sounds, and the method further comprises at least one of initiating and modifying a pacing therapy delivered to a heart from an implantable pacemaker based on detection of adventitious lungs-sounds.

17. The method of claim 12, further comprising:

detecting a posture of the patient; and

transmitting the lung-sound signals from a selected one of the plurality of implantable sound-detecting sensors based on the detected posture.

18. The method of claim 12, further comprising at least one of initiating and modifying a pacing therapy delivered to a heart from an implantable pacemaker based on the detected respiratory events.

19. A lung-sound analyzing system, comprising:

means for obtaining a plurality of lung-sound signals indicative of lung-sounds sensed from a plurality of implantable sound-detecting sensors positioned at spaced-apart locations adjacent to a pulmonary system using an implantable housing and at least one lung-sound sensor external of the implantable housing;

means for transmitting the lung-sound signals obtained at each of the plurality of spaced-apart locations using circuitry within the implantable housing; and

means for analyzing the plurality of lung-sound signals to detect respiratory events.

The system of claim 19, wherein the means for analyzing the plurality of lung-sound signals to detect respiratory events comprises means for analyzing the plurality of lung-sound signals to detect adventitious lung sounds, the system further comprises means for implantably delivering a cardiac therapy to a heart, and the therapy is at least one of initiated and modified based on the detection of adventitious lung sounds.

21. The system of claim 19, wherein means for analyzing comprises means for analyzing the plurality of lung-sound signals to detect respiratory events internal to the patient.

22. The system of claim 19, wherein means for analyzing comprises means for analyzing the plurality of lung-sound signals to detect respiratory events external of the patient.
