Abstract

A tissue surface treatment apparatus includes a housing having a proximal end, a distal end including a tissue contacting surface, an interior defined by the housing and a handpiece coupled to the housing. The contactinc surface has a plurality of apertures. An energy delivery device including an electrode is positionable in the housing interior. The electrode includes a tissue penetrating distal end in substantial alignment with an aperture. The electrode is configured to be advanced from the housing interior through the aperture and into a target tissue site to define an ablation volume at least partly bounded by a tissue surface. An advancement device is coupled to the energy delivery device and is at least partly positionable within at least one of the housing or the handpiece. The advancement device is configured to advance the electrode from the housing interior into the target tissue site and withdrawal the electrode into the housing interior. A cable is coupled to the housing or the energy delivery device and is configured to be coupled to an energy source.

Claims

1. A method of performing tissue resection or biopsy on a target tissue, comprising:

(a) inserting into the target tissue an array of electrodes to form an ablation region in the target tissue, the array of electrodes extending from a tissue contact surface of a treatment device wherein at least a portion of the tissue contact surface is made of a conformable porous material adapted to conform to a tissue surface,

(b) delivering a vacuum between the tissue contact surface and the tissue surface though the porous material, and

(c) applying RF energy to the electrodes under conditions effective to produce the ablation region.

2. The method according to claim 1, further comprising cutting and removing tissue in the ablation region after the RF energy has been applied to the electrodes.

3. The method according to claim 1, wherein the step of applying RF energy to the electrodes includes applying the RF energy while the vacuum between the tissue contact surface and the tissue surface is being delivered.

4. The method of claim 1, wherein the array of electrodes inserted into the target tissue includes a linear array of electrodes.

5. The method of claim 4, wherein multiple ones of the electrodes are positive electrodes, and multiple ones are negative electrodes.

6. The method of claim 1, further comprising:

configuring said array of electrodes to provide substantially uniform energy distribution throughout the ablation region.

7. The method of claim 1, wherein the ablation region forms at least one plane of coagulated tissue.

8. The method of claim 1, further comprising:

infusing a solution into the target tissue volume via at least one of the electrodes.

9. The method of claim 1, further comprising:

measuring at least one of a temperature or impedance at one or more sensors positioned on at least one of the array of electrodes.

10. The method of claim 9, further comprising:

changing the RF energy to the electrodes in response to the temperature or impedance measured at the one or more sensors.

11. A method of performing tissue resection or biopsy on a target tissue, comprising:

(a) inserting into the target tissue an array of electrodes effective to form a substantially rectangular ablation region in the target tissue, when RF energy is applied to the electrodes, the array of electrodes extending from a tissue contact surface of a treatment device wherein at least a portion of the tissue contact surface is made of a conformable porous material adapted to conform to a tissue surface,

(b) delivering a vacuum between the tissue contact surface and the tissue surface though the porous material,

(c) applying RF energy to the electrodes under conditions effective to produce such rectangular ablation region, and

(d) cutting and removing tissue in the ablation region.

12. The method of claim 11, wherein the array of electrodes inserted into the target tissue includes a linear array of electrodes.

13. The method of claim 12, wherein multiple ones of the electrodes are positive electrodes, and multiple ones are negative electrodes.

14. The method of claim 11, further comprising:

configuring said array of electrodes to provide substantially uniform energy distribution throughout the ablation region.

15. The method of claim 11, wherein said rectangular ablation region forms at least one plane of coagulated tissue.

16. The method of claim 11, further comprising:

infusing a solution into the target tissue volume via at least one of the electrodes.

17. The method of claim 11, further comprising:

measuring at least one of a temperature or impedance at one or more sensors positioned on at least one of the array of electrodes.

18. The method of claim 17, further comprising:

modulating the application of RF energy to the electrodes in response to the temperature or impedance measured at the one or more sensors.
