1. A medical device, comprising:
- a catheter with a distal end and a proximal end and at least one catheter lumen extending therebetween;
a sensor located at the catheter distal end; and
at least two sensor leads, a distal end of each of the sensor leads in electrical communication with the sensor, and a proximal end of each of the sensor leads extending to the catheter proximal end;
wherein the sensor includes at least two electrodes, where each electrode is in electrical communication with at least one of the at least two sensor leads, and the electrodes are located at the catheter distal end such that the electrodes contact a target site when the catheter distal end is placed at the target site;
wherein each electrode has an end plate with a major axis that is substantially perpendicular to a longitudinal axis of the catheter, and wherein at least a portion of the end plate overhangs the distal end of the catheter.

Abstract
An apparatus and method for catheter-based sensing and injection at a target site within a patient'"'"'s body. An injection catheter is equipped with electrodes at its distal end which contact the surface of a target site, such as an AV node of the heart. Electrical signals detected at the target site by the electrodes are fed via leads to the proximal end of the catheter, where they are received by a monitor, such as an EKG monitor. If the electrical signals satisfy predetermined criteria, a needle within the catheter is extended into the target site, and a therapeutic agent is injected into the target site.

19. A sensor-equipped catheter injection system, comprising:

a catheter with a distal end and a proximal end and at least one lumen extending therebetween; a sensor located at the catheter distal end, the sensor including at least two electrodes, the electrodes being located at the catheter distal end such that the electrodes contact a target site when the catheter distal end is placed at the target site, and the sensor further includes an insulator to electrically insulate the at least two electrodes from one another; at least two sensor leads, a distal end of each of the sensor leads in electrical communication with one of said at least two electrodes, and a proximal end of each of the sensor leads extending to the catheter proximal end; at least one needle having a distal end in the form of a needle tip and a proximal end to receive a therapeutic agent, slidably disposed in one of the at least one catheter lumens between a retracted position in which the needle tip is within the catheter distal end and an extended position in which the needle tip extends beyond the catheter distal end, the at least one needle being electrically insulated from the at least two electrodes; and a sensor monitor in electrical communication with the proximal ends of the at least two sensor leads to monitor electrical signals from the at least two electrodes; wherein each electrode has an end plate with an outer circular edge and a major axis that is substantially perpendicular to a longitudinal axis of the catheter, and wherein at least a portion of the end plate overhangs the distal end of the catheter. - View Dependent Claims (20, 21, 22, 23, 24, 25, 26, 27, 28, 29, 30, 31, 32, 33)

34. A method for sensor-guided catheter-based injection, comprising:

providing a catheter with a distal end and a proximal end and at least one lumen extending therebetween; providing a sensor located at the catheter distal end, the sensor including at least two electrodes, the electrodes being located at the catheter distal end such that the electrodes contact a target site when the catheter distal end is placed at the target site, and the sensor further including an insulator to electrically insulate the at least two electrodes from one another, wherein the sensor comprises a rough surface and an end plate, at least a portion of the end plate overhanging the distal end of the catheter; providing at least two sensor leads, a distal end of each of the sensor leads being in electrical communication with the sensor, and a proximal end of each of the sensor leads extending to the catheter proximal end; providing at least one needle having a distal end in the form of a needle tip and a proximal end adapted to receive a therapeutic agent, the at least one needle being slidably disposed in one of the at least one catheter lumens between a retracted position in which the needle tip is within the catheter distal end and an extended position in which the needle tip extends beyond the catheter distal end, the at least one needle being electrically insulated from the at least two electrodes; providing a sensor monitor in electrical communication with the proximal ends of the at least two sensor leads to monitor electrical signals from the at least two electrodes; placing a distal end of the catheter in contact with a target site within a patient'"'"'s body; securing the sensor to a tissue surface at the target site by the rough surface of the sensor; monitoring electrical signals detected by the electrodes; determining whether the electrical signals detected by the electrodes meet a predetermined injection criteria; and injecting the therapeutic agent into the target site, if the electrical signals detected by the electrodes meet the predetermined injection criteria. - View Dependent Claims (35, 36, 37, 38, 39, 40)
