1. An implantable medical device (IMD) comprising:
- a processing unit for controlling the IMD and including a ventricular pacing protocol (VPP) module for selectively operating the IMD in an atrial based pacing mode or a dual chamber based pacing mode, wherein VPP is a pacing protocol that utilizes an atrial based pacing mode while sensing for intrinsic AV conduction, wherein the pacing mode permits a full cardiac cycle between two successive atrial pacing pulses to elapse without ventricular activity and delivers a ventricular pacing pulse in a cardiac cycle immediately following the cardiac cycle that is permitted to elapse without ventricular activity;
a pulse generator for selectively providing cardiac pacing under the control of the processing unit;
an intrinsic conduction checking module communicatively coupled with the processing unit and causing the processing unit to perform conduction checks when operating in the dual chamber based pacing mode, the conduction checks occurring according to a predetermined pattern; and
an AV interval module for varying a duration of an AV interval when operating in the dual chamber based pacing mode and based upon the predetermined pattern.

Abstract
A pacing protocol is provided that reduces or minimizes ventricular pacing in favor of intrinsic conduction. When operating in a mode that provides ventricular pacing, a series of conduction checks are performed to determine if intrinsic conduction has returned. These conduction checks occur according to a predetermined pattern that generally includes longer intervals between subsequent attempts. The AV interval provided for dual chamber based pacing is modulated and generally moves from a larger value to a nominal value as the interval between unsuccessful conduction checks increases.

operating an implantable medical device (IMD) having cardiac pacing capabilities in an atrial based pacing mode if intrinsic conduction is present and in a dual chamber pacing mode according to a ventricular pacing protocol (VPP) if intrinsic conduction is not present, wherein VPP is a pacing protocol that utilizes an atrial based pacing mode while sensing for intrinsic AV conduction, wherein the pacing mode allows a full cardiac cycle between two successive atrial pacing pulses to elapse without ventricular activity and delivers a ventricular pacing pulse in a cardiac cycle immediately following the cardiac cycle that is allowed to elapse without ventricular activity; performing a conduction check at predetermined intervals while operating in the dual chamber pacing mode, wherein the predetermined intervals vary according to a predetermined pattern provided for sequential unsuccessful conduction checks and the predetermined pattern includes an initial minimum interval and a maximum interval; selecting a subsequent interval from the predetermined pattern with each unsuccessful conduction check, until the maximum interval is reached; determining an AV interval for the dual chamber pacing mode, wherein the AV interval varies from a first duration to second duration as the predetermined pattern progresses with the first duration being greater than the second duration. - View Dependent Claims (13, 14, 15, 16, 17)

operating an implantable medical device (IMD) having cardiac pacing capabilities in an atrial based pacing mode if intrinsic conduction is present and in a dual chamber pacing mode according to a ventricular pacing protocol (VPP) if intrinsic conduction is not present, wherein VPP is a pacing protocol that utilizes an atrial based pacing mode while sensing for intrinsic AV conduction, wherein the pacing mode allows a full cardiac cycle to elapse without ventricular activity and delivers a ventricular pacing pulse in a cardiac cycle immediately following the cardiac cycle that is allowed to elapse without ventricular activity; performing a conduction check at predetermined intervals while operating in the dual chamber pacing mode, wherein the predetermined intervals vary according to a predetermined pattern provided for sequential unsuccessful conduction checks and the predetermined pattern includes an initial minimum interval and a maximum interval; selecting a subsequent interval from the predetermined pattern with each unsuccessful conduction check, until the maximum interval is reached; determining an AV interval for the dual chamber pacing mode, wherein the AV interval varies from a first duration to second duration as the predetermined pattern progresses with the first duration being greater than the second duration, further comprising; determining a percentage time that the IMD operates in the dual chamber based pacing mode and the AV interval is determined at least in part based upon the percentage.
