1. A method of collaborative failure analysis, the method comprising:
- receiving, into a tool, data from at least one first external source that comprises at least one airplane system malfunction case, the at least one airplane system malfunction case being predefined by the at least one first external source;
receiving, into the tool, from a plurality of second external sources, a plurality of effects resulting from the received at least one airplane system malfunction case, the plurality of second external sources comprising a plurality of users; and
generating a recommendation for a design change to an airplane design based on the received at least one airplane system malfunction case and the received plurality of effects resulting from the received at least one airplane system malfunction case.

Analyzing system malfunction data and associated direct and indirect effects in a collaborative tool to determine relationships between the effects and the system malfunction information. System malfunction cases and other data are received and stored. A user may view and/or modify the system malfunction data. The data may be presented in a relational structure. Each system malfunction case is linked to one or more direct effects, and each direct effect is linked to one or more indirect effects.
