Abstract

The present disclosure relates to systems and methods for:

• 1) displaying all vital central station (CS) information and controls on a single screen;
• 2) linking peripheral central stations (pCSs) to a master central station (mCS);
• 3) operating the system disclosed in U.S. Ser. No. 10/460,458, without medical professionals (MPs) in the mCS or without any mCS;
• 4) linking a remote controlled defibrillator (RCD™) unit to an arrest sensor;
• 5) operating an RCD unit in a motor vehicle and linking an RCD unit to a vehicle communications system;
• 6) linking an RCD unit to a CS through a network of: a) non-vehicle-based stationary units (SUs), b) vehicle-based SUs/vehicle communication systems, or c) non-vehicle-based SUs and vehicle-based SUs/vehicle communication systems;
• 7) using an RCD unit with a chest compression device;
• 8) using the network of RCD units and MPs for disaster monitoring; and
• 9) monitoring and treating hospital patients and motor vehicle passengers.

Claims

1. An implantable medical device (IMD) treatment system which allows a patient with an IMD prompt access to a medical professional who can remotely monitor and treat a medical condition, said system comprising, in combination:

(a) a central station comprising;

(1) a display device for displaying medical information from a patient for evaluation by said medical professional;

(2) a first input device, responsive to said medical professional, for producing a first patient treatment control signal for controlling the treatment of said patient; and

(3) a first transmitting/receiving (T/R) device, coupled to said display device and to said first input device, for electronic communication with an IMD disposed at each of a plurality of remote sites; and

(b) an IMD disposed at each of a plurality of remote sites and including;

(1) a second transmitting/receiving (T/R) device for electronic communication with said first T/R device of said central station;

(2) a patient treatment device, having a control input coupled to said second T/R device, for providing medical treatment to a patient in response to said first patient treatment control signal received at said treatment device control input;

(3) a logic device for automatic control of the patient treatment device, said logic device having a logic device input and being coupled to the second T/R device and to the control input of said patient treatment device, said logic device automatically producing a second patient treatment control signal for controlling the treatment of said patient, in the absence of proper and timely communication between said first T/R device and said second T/R device, in dependence upon at least one sensor signal received at said logic device input;

(4) a sensor circuit, having at least one sensor circuit input, for producing a sensor signal at a sensor circuit output coupled to said second T/R device and to said logic device input; and

(5) at least one sensor, each coupled to said sensor circuit input and adapted to sense a physiologic state of said patient and produce a sensor signal in response to said physiologic state;

wherein said sensor signal may be transmitted from said at least one sensor via said sensor circuit to said central station for evaluation and subsequent action by said medical professional;

wherein said first patient treatment control signal may be transmitted from said central station to said control input of said treatment device;

wherein in response thereto, said treatment device is operative to treat said patient; and

wherein, in the absence of proper and timely communication between said first T/R device and said second T/R device, said treatment device is responsive to said second treatment control signal received from said logic device to treat said patient.

2. The system defined in claim 1, further comprising a control unit, adapted for use at the remote site of each IMD, said control unit comprising:

(1) a third transmitting/receiving (T/R) device for electronic communication with said first T/R device of said central station; and

(2) a fourth transmitting/receiving (T/R) device, coupled to said third T/R device, for electronic communication with said second T/R device of said IMD;

whereby said IMD communicates with said central station through said control unit.

3. The system defined in claim 2, wherein said central station further comprises telecommunication control means for selecting at least one of a mode and a route of communication between said first T/R device and said third T/R device.

The system defined in claim 2, wherein said control unit includes at least one second input device, responsive to a person at the remote site, coupled to said fourth T/R device, for producing a third patient treatment control signal for controlling the application of treatment to said patient, whereby said IMD may be controlled at the remote site by said person at the remote site.

5. The system defined in claim 2, wherein said control unit further comprises a second input device, designed for and responsive to said patient, and coupled to at least one of said third and fourth T/R device, for initiating at least one of emergency monitoring and treatment of said patient.

6. The system defined in claim 5, wherein said second input device includes a button disposed on said control unit which may be pressed by said patient in case of a medical emergency.

7. The system defined in claim 2, wherein said control unit includes a read/writable storage device for storing information relating to said sensor signals for subsequent readout.

8. The system defined in claim 2, wherein said first T/R device and said third T/R device are each connected to a public telephone network.

9. The system defined in claim 2, wherein said second T/R device and said fourth T/R device include means for wireless duplex communication between them.

10. The system defined in claim 2, wherein said first T/R device and said second T/R device include means for direct duplex communication between them, without transmission through said third T/R device and said fourth T/R device.

11. The system defined in claim 2, wherein said first and said second T/R device include means for communicating in at least one of a plurality of separate modes.

12. The system defined in claim 11, wherein said first and said second T/R device further include means for automatically selecting the modes of communication.

13. The system defined in claim 11, wherein said central station further includes a communication control device, responsive to said medical professional, for selecting the modes of communication of said first and said second T/R device.

14. The system defined in claim 11, wherein said modes of communication include a wired network, a wireless network and the Internet.

15. The system defined in claim 2, wherein said first and said second T/R device include means for communicating over one of a plurality of separate routes.

16. The system defined in claim 15, wherein said first and said second T/R device further include means for automatically selecting the route of communication.

17. The system defined in claim 15, wherein the central station further includes a communication control device, responsive to said medical professional, for selecting the route of communication between said first and said second T/R device.

18. The system defined in claim 2, wherein said control unit further includes a telephone transmitter and receiver coupled to said third T/R device, thereby enabling voice communication between said medical professional at the central station and said patient at the remote site.

19. The system defined in claim 1, wherein said central station includes a read/writable storage device for storing information relating to said sensor signals for subsequent readout.

20. The system defined in claim 1, wherein said central station further comprises a database storage device for storing medical information, and a microprocessor configured to search said storage device for desired medical information, whereby said information is displayed for said medical professional by said display device.

21. The system defined in claim 20, wherein said storage device stores medical information relating to a plurality of patients, and wherein said microprocessor is responsive to said first input device to search said storage device for medical information relating to the patient.

22. The system defined in claim 20, wherein said storage device stores the location of a plurality of medical emergency teams, and wherein said microprocessor is responsive to said first input device to search said storage device for the location of the medical emergency team nearest to the patient.

23. The system defined in claim 1, wherein at least one of said patient treatment device and said sensor circuit is selected from the group consisting of:

(i) a blood pressure sensing device;

(ii) a blood pressure treating device;

(iii) a blood sugar sensing device;

(iv) a blood sugar treating device;

(v) a nervous system treating device;

(vi) a medication infusion device;

(vii) a cardioverter defibrillator; and

(viii) a pacemaker.

24. An implantable medical device (IMD) treatment system which allows a patient with an IMD prompt access to a medical professional who can remotely monitor and treat a medical condition, said system comprising, in combination:

(a) a central station comprising;

(1) a display device for displaying medical information from a patient for evaluation by said medical professional;

(2) a first input device, responsive to said medical professional, for producing a first patient treatment control signal for controlling the treatment of said patient; and

(3) a first transmitting/receiving (T/R) device, coupled to said display device and to said first input device, for electronic communication with an IMD disposed at each of a plurality of remote sites; and

(b) an IMD disposed at each of a plurality of remote sites and including;

(1) a second transmitting/receiving (T/R) device for electronic communication with said first T/R device of said central station;

(2) a patient treatment device, having a control input coupled to said second T/R device, for providing medical treatment to a patient in response to said first patient treatment control signal received at said treatment device control input;

(3) a logic device for automatic control of the patient treatment device, said logic device having a logic device input and being coupled to the second T/R device and to the control input of said patient treatment device, said logic device automatically producing a second patient treatment control signal for controlling the treatment of said patient in dependence upon at least one sensor signal received at said logic device input;

(4) a sensor circuit, having at least one sensor circuit input, for producing a sensor signal at a sensor circuit output coupled to said second T/R device and to said logic device input; and

(5) at least one sensor, each coupled to said sensor circuit input and adapted to sense a physiologic state of said patient and produce a sensor signal in response to said physiologic state;

wherein said sensor signal may be transmitted from said at least one sensor via said sensor circuit to said logic device for automatic evaluation and may be provided via said sensor circuit to said central station for evaluation by said medical professional;

wherein said patient treatment device is responsive to said second patient treatment control signal received from said logic device to provide treatment to said patient unless said first patient treatment control signal is transmitted from said central station to said control input of said patient treatment device; and

wherein, in response to receipt of said first patient treatment control signal from said central station, said patient treatment device operates under control of the medical professional to possibly provide treatment to said patient, thereby enabling said medical professional at said central station to override the control of said logic device whenever necessary.

25. The system defined in claim 24, further comprising a control unit, adapted for use at the remote site of each IMD, said control unit comprising:

(1) a third transmitting/receiving (T/R) device for electronic communication with said first T/R device of said central station; and

(2) a fourth transmitting/receiving (T/R) device, coupled to said third T/R device, for electronic communication with said second T/R device of said IMD;

whereby said IMD communicates with said central station through said control unit.

26. The system defined in claim 25, wherein said control unit further comprises telecommunication control means for selecting at least one of a mode and a route of communication between said first T/R device and said third T/R device.

The system defined in claim 25, wherein said control unit includes at least one second input device, responsive to a person at the remote site, coupled to said fourth T/R device, for producing a third patient treatment control signal for controlling the application of treatment to said patient, whereby said IMD may be controlled at the remote site by said person at the remote site.

28. The system defined in claim 25, wherein said control unit includes a read/writable storage device for storing information relating said sensor signals for subsequent readout.

29. The system defined in claim 25, wherein said first T/R device and said third T/R device are each connected to a public telephone network.

30. The system defined in claim 25, wherein said second T/R device and said fourth T/R device include means for wireless duplex transmission between them.

31. The system defined in claim 25, wherein said control unit further comprises a second input device, designed for and responsive to said patient, and coupled to at least one of said third and fourth T/R device, for initiating at least one of emergency monitoring and treatment of said patient.

32. The system defined in claim 31, wherein said second input device includes a button disposed on said control unit which may be pressed by said patient in case of a medical emergency.

33. The system defined in claim 25, wherein said first T/R device and said second T/R device include means for direct duplex communication between them, without transmission through said third T/R device and said fourth T/R device.

34. The system defined in claim 25, wherein said first and said second T/R device include means for communicating in at least one of a plurality of separate modes.

35. The system defined in claim 34, wherein said first and said second T/R device further include means for automatically selecting the modes of communication.

36. The system defined in claim 34, wherein said central station further includes a communication control device, responsive to said medical professional, for selecting the modes of communication of said first and said second T/R device.

37. The system defined in claim 25, wherein said modes of communication include a wired network, a wireless network and the Internet.

38. The system defined in claim 24, wherein said first and said second T/R device include means for communicating over one of a plurality of separate routes.

39. The system defined in claim 38, wherein said first and said second T/R device further include means for automatically selecting the route of communication.

40. The system defined in claim 38, wherein the central station further includes a communication control device, responsive to said medical professional, for selecting the route of communication between said first and said second T/R device.

41. The system defined in claim 25, wherein said control unit further includes a telephone transmitter and receiver coupled to said third T/R device, thereby enabling voice communication between said medical professional at the central station and said patient at the remote site.

42. The system defined in claim 24, wherein said central station includes a read/writable storage device for storing information relating to said sensor signals for subsequent readout.

43. The system defined in claim 24, wherein said central station further comprises a database storage device for storing medical information, and a microprocessor configured to search said storage device for desired medical information, whereby said information is displayed for said medical professional by said display device.

44. The system defined in claim 43, wherein said storage device stores medical information relating to a plurality of patients, and wherein said microprocessor is responsive to said first input device to search said storage device for medical information relating to the patient.

45. The system defined in claim 43, wherein said storage device stores the location of a plurality of medical emergency teams, and wherein said microprocessor is responsive to said first input device to search said storage device for the location of the medical emergency team nearest to the patient.

46. The system defined in claim 24, wherein at least one of said patient treatment device and said sensor circuit is selected from the group consisting of:

(i) a blood pressure sensing device;

(ii) a blood pressure treating device;

(iii) a blood sugar sensing device;

(iv) a blood sugar treating device;

(v) a nervous system treating device;

(vi) a medication infusion device;

(vii) a cardioverter defibrillator; and

(viii) a pacemaker.
