Abstract

An atrial based pacing protocol promotes intrinsic conduction. An entire cardiac cycle is monitored for ventricular activity and permitted to lapse with ventricular activity. Ventricular pacing is available in a cardiac cycle immediately subsequent to such a skipped beat. When monitoring for intrinsic ventricular events, an event is expected within a given window. If no such event is detected, the cardiac cycle is truncated, leading to a shorter cycle that is devoid of ventricular activity. The subsequent cycle has a high likelihood of a ventricular sensed event and a greater than normal AV interval is provided prior to pacing.

Claims

1. A method of collecting data for intrinsic ventricular conduction times as a function of atrial rate for an implantable medical device, the method comprising:

pacing at an atrial test rate;

sensing to determine if ventricular conduction occurs;

measuring the AV delay if conduction occurred;

recording the AV delay as corresponding to the test rate in a memory of the implantable medical device; and

increasing the test rate by a predetermined increment and repeating the steps of sensing measuring and recording.

2. The method of claim 1, further comprising:

pacing at an atrial lower rate interval (LRI) that is below the test rate for a predetermined number of cardiac cycle prior to increasing the test rate.

3. A method of collecting data for intrinsic ventricular conduction times as a function of atrial rate for an implantable medical device, the method comprising:

pacing at an atrial test rate;

sensing to determine if ventricular conduction occurs;

measuring the AV delay if conduction occurred;

recording the AV delay as corresponding to the test rate in a memory of the implantable medical device;

increasing the test rate by a predetermined increment and repeating the steps of sensing measuring and recording;

operating the implantable medical device according to a ventricular pacing protocol (VPP) wherein ventricular pacing is precluded in a given cardiac cycle where a ventricular event occurred in a cardiac cycle immediately prior to the given cardiac cycle;

establishing an A-A interval;

initiating a first AV interval timer at the initiation of the given cardiac cycle;

sensing for ventricular events;

scheduling an accelerated atrial pacing (AP(E)) pulse prior to the termination of the A-A interval if no ventricular event is sensed within the duration of the AV interval timer, wherein an A-AP(E) interval is compared to the data recorded in the memory of the implantable medical device and only utilized if the data indicates that conduction has successfully occurred at this A-AP(E) interval.

4. The method of claim 3, further comprising:

canceling the atrial pacing pulse and reverting to the A-A interval if a ventricular event is sensed prior to the atrial pacing pulse.

5. The method of claim 3, further comprising:

initiating a second AV interval timer upon delivery of the atrial pace;

delivering a ventricular pace at the expiration of the second AV interval timer unless inhibited by a sensed ventricular event.

6. The method of claim 3, further comprising selecting an alternative A-AP(E) interval if the data recorded in the memory does not indicate successful conduction at the A-AP(E) interval.

7. An implantable medical device comprising:

means for conducting an AV interval test to determine AV delays for intrinsic ventricular conduction at a plurality of atrial rates;

means for storing results of the AV interval test in a memory of the IMD;

means for operating an implantable medical device (IMD) according to a ventricular pacing protocol (VPP) wherein ventricular pacing is precluded in a given cardiac cycle wherein a ventricular event occurred in a cardiac cycle immediately subsequent to the given cardiac cycle;

means for establishing an A-A interval;

means for initiating a first AV interval timer at the initiation of the given cardiac cycle;

means for sensing for ventricular events;

means for scheduling an atrial pacing pulse at an accelerated interval prior to the termination of the A-A interval if no ventricular event is sensed within the duration of the AV interval timer and the results stored in memory indicate successful AV conduction at the accelerated interval.

8. The IMD of claim 7, further comprising:

means for canceling the atrial pacing pulse and reverting to the A-A interval if a ventricular event is sensed prior to the atrial pacing pulse.

9. The IMD of claim 8, further comprising:

means for initiating a second AV interval timer upon delivery of the atrial pace;

means for delivering a ventricular pace at the expiration of the second AV interval timer unless inhibited by a sensed ventricular event.

10. A method comprising:

pacing at an atrial test rate;

sensing to determine if ventricular conduction occurs;

measuring the AV delay if conduction occurred;

recording the AV delay as corresponding to the test rate in a memory of an implantable medical device (IMD);

increasing the test rate by a predetermined increment and repeating the steps of sensing measuring and recording;

operating the IMD according to a ventricular pacing protocol (VPP);

sensing a premature ventricular contraction (PVC) in a first cardiac cycle;

measuring a duration of a first ventricular interval from a ventricular event immediately preceding the PVC to the PVC;

calculating a duration of a second ventricular interval to occur from the PVC to a target ventricular sensed event; and

modifying atrial timing in the first cardiac cycle to effect the second ventricular interval, wherein the modification to atrial timing is only made if data recorded in the memory indicates successful AV conduction at the modified atrial rate.

11. The method of claim 10, wherein the second ventricular interval occurs without ventricular pacing.

12. The method of claim 10, modifying atrial timing in subsequent consecutive cycles until a resultant V-V interval is substantially equal to a V-V interval prior to the first ventricular interval.

13. The method of claim 12, wherein the modification of atrial timing in subsequent consecutive cycles results in a smoothing of the resultant V-V intervals.

14. The method of claim 10, wherein calculating the duration of the second ventricular interval includes adding a constant to the duration of the first ventricular interval.

15. The method of claim 14, wherein the constant is selected from a table and the table includes constant values for corresponding ventricular intervals.

16. The method of claim 10, wherein calculating the duration of the second ventricular interval includes increasing the first ventricular interval by a predetermined percentage.

17. The method of claim 10, further comprising:

sensing for atrial refractory events; and

adding a predetermined value to the modified atrial timing if an atrial refractory event is sensed.

18. The method of claim 10, further comprising:

setting a first AV interval for delivering a ventricular backup pacing pulse following an A-A interval devoid of a sensed ventricular event according to the VPP;

setting a second AV interval for delivering a ventricular pacing pulse following a truncated A-A interval as modified to effect the second ventricular interval wherein the second AV interval is longer than the first AV interval.

19. The method of claim 10, wherein modifying the atrial timing includes determining an AV interval that would result in the target sensed ventricular event and delivering an atrial pacing pulse at so that the atrial pacing pulse initiated the AV interval.
