Abstract

Described is a full-thickness resection system which includes a control unit coupled to a proximal end of a flexible endoscope. The control unit remains outside of a body when the stapling head is in an operative position within a body lumen. The control unit includes (i) an anvil actuator coupled to an anvil in the stapling head, actuation of the anvil actuator moves the anvil axially relative to a stapling mechanism in the stapling head to compress a folded full-thickness portion of lumenal tissue between the anvil and the stapling mechanism. In addition, the control unit includes (ii) a stapler actuator coupled to the stapling mechanism in the stapling head, actuation of the stapler actuator causing the stapling mechanism to drive staples through the folded lumenal tissue against the anvil. Also, the control unit includes (iii) a tissue cutter actuator coupled to a tissue cutter in the stapling head, actuation of the tissue cutter actuator causing the tissue cutter to resect portions of the folded lumenal tissue.

Claims

1. A full-thickness resection system, comprising:

a control unit coupled to a proximal end of a flexible endoscope and for controlling a stapling head, the control unit comprising;

an anvil actuator coupled to an anvil in the stapling head;

a stapler actuator coupled to a stapling mechanism in the stapling head, an actuation of the stapler actuator causing a rotatable staple driving cam of the stapling mechanism to drive staples through tissue against the anvil; and

a tissue cutter actuator coupled to a tissue cutter in the stapling head for resecting the tissue.

2. The system according to claim 1, wherein the stapling actuator is rotatable and is coupled to the stapling mechanism by a longitudinally flexible, torsionally stiff drive shaft.

The system according to claim 2, wherein rotation of the stapling actuator causes the rotatable staple driving cam in the stapling head to sequentially strike staple driving members driving each of the staple driving members distally through a respective staple channel, each of the staple driving members forcing a respective staple from the respective staple channel and against the anvil.

The system according to claim 1, wherein the tissue cutter actuator is rotatably coupled to the tissue cutter by a longitudinally flexible torsionally stiff drive shaft, rotation of the tissue cutter actuator causing the tissue cutter to cut tissue along a curve radially within the staple driving members.

5. The system according to claim 1, wherein the anvil actuator is slidably coupled to the anvil.

6. The system according to claim 1, wherein the anvil actuator is rotatably coupled to a translation element which translates rotation of the anvil actuator into axial movement of at least one shaft coupled to the anvil.

7. The system according to claim 1, wherein the anvil is permanently coupled to the stapling head.

8. The system according to claim 1, wherein a distal surface of the anvil forms a distal most end of the stapling head which, on insertion into a body lumen, forms a leading surface thereof and wherein the distal surface of the anvil is tapered.

9. The system according to claim 1, wherein an actuation of the anvil actuator moves the anvil axially relative to the stapling mechanism in the stapling head to compress the tissue between the anvil and the stapling mechanism.

10. The system according to claim 1, wherein the tissue is a folded lumenal tissue.

11. A control unit for a full-thickness resection device slidably coupleable to a flexible endoscope, the device having a stapling head which, when in an operative position, is within a body lumen, the control unit comprising:

an anvil actuator coupled to an anvil in the stapling head;

a stapler actuator coupled to a stapling mechanism in the stapling head, actuation of the stapler actuator causing a rotatable staple driving cam of the stapling mechanism to drive staples through tissue against the anvil; and

a tissue cutter actuator coupled to a tissue cutter in the stapling head for resecting the tissue.

12. The control unit according to claim 11, wherein an actuation of the anvil actuator moves the anvil axially relative to the stapling mechanism in the stapling head to compress a folded full-thickness portion of lumenal tissue between the anvil and the stapling mechanism.
