Abstract

Methods and devices are disclosed for employing mechanical measurements to synchronize contractions of ventricular wall locations. Accelerometers that may be placed within electrode leads are positioned at ventricular wall locations, such as the left ventricle free wall, right ventricle free wall, and the anterior wall/septum wall. The accelerometers produce signals in response to the motion of the ventricular wall locations. A processor may then compare the signals to determine a difference in the synchronization of the ventricular wall location contractions. The difference in synchronization can be determined in various ways such as computing a phase difference and/or amplitude difference between the accelerometer signals. One or more stimulation pulses may be provided per cardiac cycle to resynchronize the contractions as measured by the accelerometers to thereby constantly and automatically optimize the cardiac resynchronization therapy.

Claims

1. A method comprising:

sensing motion at a first heart wall location to produce a first signal;

sensing motion at a second heart wall location to produce a second signal;

using information from the first and second signals to determine at least one control parameter for a stimulation pulse to resynchronize a heart contraction; and

applying the stimulation pulse or changing at least one control parameter of the stimulation pulse to resynchronize the heart contraction.

The method of claim 1, comprising detecting a change in at least one of a contraction of the first heart wall location, a contraction of the second heart wall location, or a synchronization between the contractions of the first and second heart wall locations, the change resulting from applying or changing at least one control parameter of the stimulation pulse.

The method of claim 2, comprising applying a stimulation pulse or changing at least one control parameter of the stimulation pulse in response to the detected change in at least one of a contraction of the first heart wall location, a contraction of the second heart wall location, or a synchronization between the contractions of the first and second heart wall locations.

4. The method of claim 1, wherein the using information includes obtaining information including detecting a difference between a time of an onset of contraction indicated by the respective first and second signals.

5. The method of claim 1, wherein the using information includes obtaining information including detecting a difference between a time of peak contraction indicated by the respective first and second signals.

6. The method of claim 1, wherein the using information includes obtaining information including detecting an amplitude difference.

7. The method of claim 1, wherein the using information includes obtaining information including detecting an amplitude difference and a phase difference.

8. The method of claim 1, comprising:

sensing motion at a third heart wall location to produce a third signal; and

using information from the first, second, and third signals to determine at least one control parameter for a stimulation pulse to resynchronize a heart contraction.

9. The method of claim 1, wherein the first heart wall location and the second heart wall location are on a same wall, and wherein applying the stimulation pulse or changing at least one control parameter of the stimulation pulse resynchronizes the heart contraction of the same wall.

10. An apparatus comprising:

means for sensing motion at a first heart wall location to produce a first signal;

means for sensing motion at a second heart wall location to produce a second signal;

means for using information from the first and second signals to determine at least one control parameter for a stimulation pulse to resynchronize a heart contraction; and

means for applying the stimulation pulse or changing at least one control parameter of the stimulation pulse to resynchronize the heart contraction.

The apparatus of claim 10, comprising means for detecting a change in at least one of a contraction of the first heart wall location, a contraction of the second heart wall location, or a synchronization between the contractions of the first and second heart wall locations, the change resulting from applying or changing at least one control parameter of the stimulation pulse.

The apparatus of claim 11, comprising means for applying a stimulation pulse or changing at least one control parameter of the stimulation pulse in response to the detected change in at least one of a contraction of the first heart wall location, a contraction of the second heart wall location, or a synchronization between the contractions of the first and second heart wall locations.

13. An apparatus comprising:

a first motion sensor, configured to be located at a first heart wall location and configured to produce a first signal;

a second motion sensor, configured to be located at a second heart wall location and configured to produce a second signal;

a processor, coupled to the first and second motion sensors, the processor configured to use information from the first and second signals to determine at least one control parameter for a stimulation pulse to resynchronize a heart contraction; and

an output circuit, configured to apply or change at least one control parameter to deliver a stimulation pulse to resynchronize the heart contraction.

The apparatus of claim 13, wherein the processor is configured to detect a change in at least one of a contraction of the first heart wall location, a contraction of the second heart wall location, or a synchronization between the contractions of the first and second heart wall locations, the change resulting from applying or changing at least one control parameter of the stimulation pulse.

The apparatus of claim 14, wherein the processor is configured to apply a stimulation pulse or to change at least one control parameter of the stimulation pulse in response to the detected change in at least one of a contraction of the first heart wall location, a contraction of the second heart wall location, or a synchronization between the contractions of the first and second heart wall locations.

16. The apparatus of claim 13, wherein at least one of the first and second motion sensors includes an accelerometer.

17. The apparatus of claim 13, comprising at least one electrode coupled to the output circuit.

18. The apparatus of claim 17, wherein the at least one electrode is configured to be disposed within a coronary sinus vein.

19. The apparatus of claim 17, comprising:

a first electrode configured to be disposed at a free wall of a left ventricle; and

a second electrode, configured to be disposed at an anterior wall of the left ventricle.

20. The apparatus of claim 17, comprising at least one electrode configured to be disposed at a septum between left and right ventricles.

21. The apparatus of claim 17, comprising at least one electrode configured to be disposed at an anterior wall of a right ventricle.

22. The apparatus of claim 17, comprising at least one electrode configured to be disposed at a free wall of a right ventricle.

23. The apparatus of claim 13, wherein the first heart wall location and the second heart wall location are on a same wall, and wherein the output circuit is further configured to resynchronize the heart contraction of the same wall.
