Abstract

Methods and systems are provided for performing ventricular arrhythmia monitoring using at least two sensing channels that are each associated with different sensing vectors, for example by different pairs of extracardiac remote sensing electrodes. Myopotential associated with each of the sensing channels in monitored, and a ventricular arrhythmia monitoring mode is selected based thereon (e.g., based on determined myopotential levels). Ventricular arrhythmia monitoring is then performed using the selected monitoring mode.

Claims

1. A method for monitoring myopotential, said method comprising:

(a) sensing a cardiac signal using a pair of extracardiac remote sensing electrodes corresponding to a sensing vector;

(b) sampling the cardiac signal to produce a plurality of samples that are representative of a window of the cardiac signal;

(c) processing the samples to obtain a result and comparing the result to a low threshold and a high threshold; and

(d) categorizing the result as indicative of low, medium or high myopotential based on the comparison.

2. The method of claim 1 wherein processing the samples comprises:

producing a histogram based on the plurality of samples, wherein the histogram has a plurality of bins corresponding to different amplitude ranges and including a highest bin and a lowest bin; and

calculating the ratio of the number of samples in the highest bin to the number of samples in the lowest bin to obtain the result.

3. The method of claim 2 further comprising:

categorizing the result as indicative of low myopotential if the result is less than the low threshold

categorizing the result as indicative of medium myopotential if the result is between the low threshold and the high threshold; and

categorizing the result as indicative of high myopotential if the result is greater than the high threshold.

4. The method of claim 2 wherein the lowest bin has a bin edge and the highest bin has a bin edge and further comprising updating the locations of the bin edges to thereby change the number of samples in the lowest bin and the number of samples in the highest bin.

5. The method of claim 1 wherein processing the samples comprises:

producing a histogram based on the plurality of samples, wherein the histogram has a plurality of bins corresponding to different amplitude ranges;

fitting a line to the histogram distribution;

determining a slope of the line fit to the histogram distribution to obtain the result.

6. The method of claim 5 further comprising:

categorizing the result as indicative of low myopotential if the result is less than the low threshold

categorizing the result as indicative of medium myopotential if the result is between the low threshold and the high threshold; and

categorizing the result as indicative of high myopotential if the result is greater than the high threshold.

7. The method of claim 1 wherein the cardiac signal comprises R waves and further comprising updating the low threshold and the high threshold as a function of changes in a R-wave detection threshold used to sense R waves.

8. The method of claim 7 wherein:

the low threshold and the high threshold are increased if the R-wave detection threshold has increased; and

the low threshold and the high threshold are decreased if the R-wave detection threshold has decreased.

9. The method of claim 7 further comprising:

increasing the present R-wave detection threshold to define an updated R-wave detection threshold when a detected R wave exceeds a second detection threshold that is greater than the present R-wave detection threshold;

decreasing the present R-wave detection threshold to define an updated R-wave detection threshold when a detected R wave is between the second detection threshold and the present R-wave detection threshold.

10. The method of claim 9 further comprising maintaining the present R-wave detection threshold when a detected R wave is between the second detection threshold and a third detection threshold that is between the present R-wave detection threshold and the second detection threshold.

11. The method of claim 1 further comprising updating the low threshold and the high threshold as a function of the ratio of the number of samples in the lowest bin of a histogram to the number of samples in the highest bin of a histogram.

12. The method of claim 1 further comprising filtering the cardiac signal sensed at step (a) and the samples produced at step (b) in order to filter out at least some of the frequencies that are not of interest, prior to step (c).

13. The method of claim 1 further comprising processing the cardiac signal sensed at step (a) and the samples produced at step (b) such that all the samples upon which monitoring is based at step (c) have a common polarity.

14. A system for detecting myopotential, said system comprising:

a sensing channel to sense a cardiac signal using a pair of extracardiac remote sensing electrodes corresponding to a sensing vector;

a sampler to sample the cardiac signal to produce a plurality of samples that are representative of a window of the cardiac signal; and

a myopotential detector to process the samples to obtain a result, compare the result to a low threshold and a high threshold, and categorize the result as indicative of low, medium or high myopotential based on the comparison.

15. The system of claim 14 wherein the myopotential detector is configured to:

produce a histogram based on the plurality of samples, wherein the histogram has a plurality of bins corresponding to different amplitude ranges and including a highest bin and a lowest bin; and

calculate the ratio of the number of samples in the highest bin to the number of samples in the lowest bin to obtain the result.

16. The system of claim 15 wherein the myopotential detector is configured to:

categorize the result as indicative of low myopotential if the result is less than the low threshold

categorize the result as indicative of medium myopotential if the result is between the low threshold and the high threshold; and

categorize the result as indicative of high myopotential if the result is greater than the high threshold.

17. The system of claim 14 wherein the myopotential detector is configured to:

produce a histogram based on the plurality of samples, wherein the histogram has a plurality of bins corresponding to different amplitude ranges;

fit a line to the histogram distribution; and

determine a slope of the line fit to the histogram distribution to obtain the result.

18. The system of claim 15 wherein the myopotential detector is configured to:

categorize the result as indicative of low myopotential if the result is less than the low threshold;

categorize the result as indicative of medium myopotential if the result is between the low threshold and the high threshold; and

categorize the result as indicative of high myopotential if the result is greater than the high threshold.

19. A method for updating at least one parameter that is used for monitoring myopotential associated with a sensing vector corresponding to a pair of subcutaneous extracardiac electrodes implanted in a patient, said method comprising:

monitoring for changes in a cardiac polarization detection threshold;

monitoring the heart rate of the patient; and

updating the at least one parameter when the detection threshold has changed by at least a specific amount since the at least one parameter was last updated and the heart rate of the patient is within an acceptable range.
