Abstract

A pacing system for providing optimal hemodynamic cardiac function for parameters such as ventricular synchrony or contractility (peak left ventricle pressure change during systole or LV+dp/dt), or stroke volume (aortic pulse pressure) using system for calculating atrio-ventricular delays for optimal timing of a ventricular pacing pulse. The system providing an option for near optimal pacing of multiple hemodynamic parameters. The system deriving the proper timing using electrical or mechanical events having a predictable relationship with an optimal ventricular pacing timing signal.

Claims

1. A cardiac rhythm management system, comprising:

an atrio-ventricular delay (AVD) calculator adapted to receive one or more measured atrio-ventricular time intervals (AVs) each being a time interval between an atrial event and an intrinsic ventricular event and calculate one or more optimal AVDs using the one or more measured AVs and a predetermined equation relating each AV of the one or more measured AVs to each optimal AVD of the one or more optimal AVDs, the each optimal AVD providing for a maximum positive rate of left ventricular pressure change during systole, LV+dp/dt, when ventricular pacing pulses are delivered using the each optimal AVD.

The system of claim 1, further comprising a measurement module adapted to measure a post-sensing AV of the one or more measured AVs, the post-sensing AV being a time interval between an intrinsic atrial event and the intrinsic ventricular event, and wherein the AVD calculator is adapted to calculate a post-sensing AVD of the one or more optimal AVDs using the post-sensing AV and the equation.

The system of claim 2, wherein the measurement module is further adapted to measure a post-pacing AV of the one or more measured AVs, the post-pacing AV being a time interval between a paced atrial event and the intrinsic ventricular event, and wherein the AVD calculator is adapted to calculate a post-pacing AVD of the one or more optimal AVDs using the post-pacing AV and the equation.

4. The system of claim 1, further comprising:

a signal input adapted to receive a signal indicative of an intrinsic left ventricular event; and

a measurement module adapted to measure an AV of the one or more measured AVs being a time interval between the atrial event and the intrinsic left ventricular event.

5. The system of claim 1, further comprising:

a signal input adapted to receive a signal indicative of an intrinsic right ventricular event; and

a measurement module adapted to measure an AV of the one or more measured AVs being a time interval between the atrial event and the intrinsic right ventricular event.

6. The system of claim 1, further comprising a programmer adapted to communicate with an implantable medical device, the programmer including the AVD calculator.

7. The system of claim 6, wherein the programmer is adapted to receive signals indicative of the atrial and ventricular events from the implantable medical device.

8. The system of claim 1, comprising an implantable medical device including the AVD calculator and a pacing output circuit adapted to deliver the ventricular pacing pulses.

9. The system of claim 1, wherein the AVD calculator is adapted to calculate the each optimal AVD of the one or more optimal AVDs using an AV of the one or more measured AVs and an equation:

10. The system of claim 9, further comprising a memory circuit storing values of N1 and N2, the values empirically derived using a patient population.

11. A method for operating a cardiac rhythm management system, the method comprising:

measuring one or more atrio-ventricular time intervals (AVs) each between an atrial event and an intrinsic ventricular event;

calculating one or more optimal atrio-ventricular delays (AVDs) using the one or more AVs and a predetermined equation relating each AV of the one or more AVs to each optimal AVD of the one or more optimal AVDs, the each optimal AVD providing for a maximum positive rate of left ventricular pressure change during systole, LV+dp/dt, when ventricular pacing pulses are delivered using the each optimal AVD; and

delivering ventricular pacing pulses using the calculated one or more optimal AVDs.

The method of claim 11, wherein measuring the one or more AVs comprises measuring a post-sensing AV between an intrinsic atrial event and the intrinsic ventricular event, and calculating the one or more optimal AVDs comprises calculating a post-sensing AVD using the post-sensing AV and the equation.

13. The method of claim 12, wherein measuring the one or more AVs comprises measuring a post-pacing AV between a paced atrial event and the intrinsic ventricular event, and calculating the one or more optimal AVDs comprises calculating a post-pacing AVD using the post-pacing AV and the equation.

14. The method of claim 13, wherein measuring the one or more AVs comprises measuring the one or more AVs each being a time interval between the atrial event and an intrinsic left ventricular event.

15. The method of claim 13, wherein measuring the one or more AVs comprises measuring the one or more AVs each being a time interval between the atrial event and an intrinsic right ventricular event.

16. The method of claim 11, wherein calculating the one or more optimal AVDs comprises calculating the one or more optimal AVDs using a programmer adapted to communicate with an implantable medical device.

17. The method of claim 16, wherein determining each AV of the one or more AVs comprises:

receiving signals indicative of the atrial event and the intrinsic ventricular event from the implantable medical device;

detecting the atrial event and the intrinsic ventricular event using the signals; and

measuring the each AV as a time interval between the detected atrial event and the detected intrinsic ventricular event.

18. The method of claim 11, wherein calculating the one or more optimal AVDs comprises calculating the one or more optimal AVDs using an implantable medical device, and further comprising delivering the ventricular pacing pulses from the implantable medical device.

19. The method of claim 11, wherein calculating the one or more optimal AVDs comprises calculating the each optimal AVD of the one or more optimal AVDs using an AV of the one or more AVs and an equation:

20. The method of claim 19, further comprising deriving values of N1 and N2 empirically using a patient population.
