Abstract

Methods and systems for detecting capture using pacing artifact cancellation are described. One or more pacing artifact templates are provided and a cardiac signal is sensed in a cardiac verification window. Each of the pacing artifact templates may characterize the pacing artifact associated with a particular pacing energy level, for example. A particular pacing artifact template is canceled from the cardiac signal. Capture is determined using the pacing artifact canceled cardiac signal. Detection of fusion/pseudofusion beats may be accomplished by comparing a cardiac signal to a captured response template.

Claims

1. A method for operating a medical device, comprising:

delivering non-capturing electrical stimulation pulses to a heart at a plurality of pulse amplitudes;

sensing pacing artifact waveforms associated with each pulse amplitude of the plurality of pulse amplitudes;

forming pacing artifact templates associated with the plurality of pulse amplitudes, each pacing artifact template associated respectively with a particular pulse amplitude or pulse amplitude range;

delivering a subsequent electrical stimulation pulse to the heart;

sensing a cardiac signal following the subsequent electrical stimulation pulse;

canceling a pacing artifact template of the plurality of pacing artifact templates from the sensed cardiac signal to form a pacing artifact template cancelled signal; and

detecting capture of the heart using the pacing artifact template cancelled signal.

2. The method of claim 1, wherein the pacing artifact template that is cancelled from the cardiac signal is associated with a pulse amplitude.

3. The method of claim 1, wherein the pacing artifact template that is cancelled from the cardiac signal is associated with range of pulse amplitudes.

4. The method of claim 1, wherein forming the pacing artifact templates comprises combining at least some of the pacing artifact waveforms, the at least some of the pacing artifact waveforms being associated with a range of pulse amplitudes.

The method of claim 1, wherein forming the pacing artifact templates comprises forming a set of pacing artifact templates, the set of pacing artifact templates corresponding to a pulse amplitude range to be evaluated, each pacing artifact template of the set associated with a smaller range of pulse amplitudes within the pulse amplitude range to be evaluated.

6. The method of claim 1, wherein delivering the non-capturing electrical stimulation pulses comprises delivering the non-capturing stimulation pulses while the myocardium is refractory.

7. The method of claim 1, wherein detecting capture of the heart using the pacing artifact template cancelled signal comprises comparing the pacing artifact cancelled cardiac signal to an amplitude reference characterizing an evoked response.

8. The method of claim 1, wherein detecting capture of the heart using the pacing artifact template cancelled signal comprises comparing the pacing artifact cancelled cardiac signal to a morphology template characterizing an evoked response.

9. The method of claim 1, wherein forming the pacing artifact templates comprises:

averaging the pacing artifact waveforms to form an average pacing artifact waveform; and

estimating a time constant from the average pacing artifact waveform.

10. The method of claim 1, further comprising periodically updating the pacing artifact templates.

11. The method of claim 1, wherein each of the pacing artifact templates is formed as an exponential function or a combination of exponential functions.

12. A medical device, comprising:

electrodes configured to electrically couple to a heart;

pulse generator circuitry coupled to the electrodes and configured to provide stimulation pulses to the heart at a plurality of pulse amplitudes;

sensing circuitry configured to sense cardiac electrical signals following the stimulation pulses;

template circuitry configured to form pacing artifact templates associated with the plurality of pulse amplitudes, each pacing artifact template associated respectively with a particular pulse amplitude or pulse amplitude range; and

a capture detector configured to cancel a pacing artifact template of the plurality of pacing artifact templates from a cardiac signal sensed following a stimulation pulse provided subsequent to formation of the pacing artifact templates and to detect capture of the heart using the pacing artifact cancelled cardiac signal.

13. The medical device of claim 12, wherein the pacing artifact template cancelled from the cardiac signal is associated with a range of pulse amplitudes.

14. The medical device of claim 13, wherein the pacing artifact template is associated with a pulse amplitude near a center of the range of pulse amplitudes.

15. The medical device of claim 12, wherein forming the pacing artifact templates comprises combining at least some of the cardiac electrical signals, the at least some of the cardiac electrical signals being associated with a range of pulse amplitudes.

The medical device of claim 12, wherein forming the pacing artifact templates comprises forming a set of pacing artifact templates, the set of pacing artifact templates corresponding to a pulse amplitude range to be evaluated, each pacing artifact template of the set associated with a smaller range of pulse amplitudes within the pulse amplitude range to be evaluated.

17. The medical device of claim 12, wherein the capture detector is configured to detect capture by comparing the pacing artifact cancelled cardiac signal to an amplitude reference characterizing an evoked response.

18. The medical device of claim 12, wherein the capture detector is configured to detect capture by comparing the pacing artifact cancelled cardiac signal to a morphology template characterizing an evoked response.

19. The medical device of claim 12, wherein the template circuitry is configured to average pacing artifact waveforms to form an average pacing artifact waveform, estimate a time constant from the average pacing artifact waveform, and form a pacing artifact template associated with a pacing pulse amplitude using the time constant.

20. A medical device, comprising:

electrodes configured to electrically couple to a heart;

pulse generator circuitry coupled to the electrodes and configured to provide stimulation pulses at a plurality of pulse amplitudes to the heart via the electrodes;

sensing circuitry configured to sense cardiac electrical signals following the stimulation pulses;

means for forming pacing artifact templates associated with the plurality of pulse amplitudes, each pacing artifact template associated respectively with a particular pulse amplitude or pulse amplitude range; and

means for canceling a pacing artifact template of the plurality of pacing artifact templates from a cardiac signal sensed following a stimulation pulse provided subsequent to formation of the pacing artifact templates and detecting capture of the heart using the pacing artifact cancelled cardiac signal.
