Abstract

Pacing parameters are provided to address cross talk and intrinsic ventricular events occurring within a predefined blanking period following an atrial event. The parameters are used in conjunction with protocol for minimizing or reducing ventricular pacing, wherein ignoring intrinsic ventricular events during the blanking period might otherwise affect the performance of the protocol.

Claims

1. An implantable medical system comprising:

means for delivering atrial pacing in a first, atrial pacing mode wherein ventricular pacing is precluded for a complete A-A cardiac cycle following delivery of an atrial pacing pulse;

means for detecting the presence or absence of AV conduction for each said complete A-A cardiac cycle during delivery of the first pacing mode;

means for automatically switching from the first pacing mode to a second, dual chamber pacing mode comprising delivery of ventricular pacing pulses subsequent to a said complete A-A cardiac cycle wherein AV conduction is absent as determined by the means for detecting; and

means for switching from the second pacing mode to the first pacing mode if AV conduction is detected.

2. A system according to claim 1, wherein said second, dual chamber pacing mode comprises a one of:

a DDD/R pacing mode, a DDD pacing mode, a DDI pacing mode, or a DDI/R pacing mode.

3. A system according to claim 1, wherein said means for detecting AV conduction further comprises a sensing circuit comprising at least one sense amplifier electrically coupled to a pair of implantable electrodes.

4. A system according to claim 1, wherein said first pacing mode is selected from the group consisting off:

ADI/R, ADI, AAI, or AAI/R and the second pacing mode is selected from the group consisting of;

DDI/R, DDI, DDD, or DDD/R.

5. A system according to claim 4, wherein said dual chamber pacing mode is selected from the group consisting of:

DDI or DDI/R for a first cycle of operation in the dual chamber pacing mode and the dual chamber pacing mode is selected from the group consisting of;

DDD or DDD/R for subsequent consecutive cycles operated in the dual chamber pacing mode.

6. A system according to claim 1, further comprising:

means for delivering a ventricular pacing stimulus to evoke a paced R-wave at the expiration of an AV interval timer while operating in the second pacing mode; and

means for sustaining continuous operation in the second pacing mode until an intrinsic R-wave is sensed.

A system according to claim 1, wherein said means for switching from the second pacing mode to the first based pacing mode subsequent to the given cycle in which AV conduction is detected, further comprising means for performing a conduction check wherein subsequent to operation in the second pacing mode for at least two consecutive cardiac cycles, the atrial based pacing mode is implemented to permit AV conduction monitoring for a full cardiac cycle.

8. A method of cardiac pacing comprising:

implementing a first, atrial pacing mode, wherein ventricular pacing is precluded for a complete A-A cardiac cycle following delivery of an atrial pacing pulse;

sensing for an AV block condition;

switching to a second, dual-chamber pacing mode automatically if said AV block condition is sensed;

sensing for intrinsic AV conduction while operating in the second pacing mode; and

resuming the first pacing mode if intrinsic AV conduction is sensed.

9. A method according to claim 8, wherein said second pacing mode is selected from the group consisting of DDD, DDI, DDD/R, or DDI/R.

10. A method according to claim 8, wherein the step of sensing for an AV block condition is performed by an assembly comprising a pair of implantable cardiac electrodes electrically coupled to an electronic cardiac depolarization sensing circuit.

11. A method according to claim 8, wherein said pair of electrodes are coupled to a cardiac pacing electronic circuit.

12. A method according to claim 8, wherein said method of cardiac pacing is implemented in a device having a cardioversion/defibrillation capability.

13. The method of claim 8, wherein resuming the first pacing mode if intrinsic AV conduction is sensed further includes performing a conduction check to determine if AV conduction is present, wherein the conduction check includes mode switching from the second pacing mode to the first pacing mode subsequent to operation in the second pacing mode for at least two consecutive cycles.

14. A non-transitory computer readable medium comprising instructions for causing an implantable medical device to perform the following steps:

implementing a first, atrial pacing mode, wherein ventricular pacing is precluded for a complete A-A cardiac cycle following delivery of an atrial pacing pulse;

sensing for an AV block condition;

mode switching to a second, dual-chamber pacing mode that provides ventricular pacing if said AV block condition is sensed;

sensing for intrinsic AV conduction while operating in the second racing mode; and

resuming the atrial based pacing mode if intrinsic AV conduction is sensed.

15. A computer readable medium according to claim 14, wherein said second pacing mode is selected from the group consisting of DDD, DDD/R, DDI, DDI/R.

16. The computer readable medium of claim 14, wherein sensing for intrinsic conduction includes sensing intrinsic R waves prior to a delivery of a ventricular pacing pulse.

17. The computer readable medium of claim 14, wherein sensing for intrinsic conduction includes performing a conduction check by switching to the first pacing mode for at least one cardiac cycle subsequent to operating in the second for at least two consecutive cardiac cycles.

18. The computer readable medium of claim 14, wherein mode switching to the dual chamber pacing mode includes switching to an inhibited dual chamber pacing mode for a first cycle and a dual response dual chamber pacing mode for a second and subsequent consecutive cardiac cycle operated in the dual chamber mode.

19. The computer readable medium of claim 14, wherein sensing for an AV block condition includes monitoring a complete cardiac cycle for intrinsic ventricular activity and determining the presence of the AV block condition only if the complete cardiac cycle passes without ventricular activity.

20. The computer readable medium of claim 19, wherein the second pacing mode is implemented in a cardiac cycle immediately subsequent the complete cardiac cycle that passes without ventricular activity.
