Abstract

A cardiac electro-stimulatory device and method for operating same in which stimulation pulses are distributed among a plurality of electrodes fixed at different sites of the myocardium in order to reduce myocardial hypertrophy brought about by repeated pacing at a single site and/or increase myocardial contractility. In order to spatially and temporally distribute the stimulation, the pulses are delivered through a switchable pulse output configuration during a single cardiac cycle, with each configuration comprising one or more electrodes fixed to different sites in the myocardium.

Claims

1. A method for operating a cardiac pacing device, comprising:

outputting pacing pulses to a selected subset of a plurality of pacing electrodes in accordance with a programmed pacing mode, wherein the subset of electrodes to which pulses are output is defined by a pulse output configuration;

sensing a parameter related to cardiac output; and

,

alternately switching between a first pulse output configuration utilized for one or more cardiac cycles and a second pulse output configuration for one or more subsequent cardiac cycles in accordance with a switching algorithm that comprises switching the pulse output configuration in a manner dependent upon the sensed parameter related to cardiac output and switching the pulse output configuration if the parameter related to cardiac output falls below a specified minimum value.

2. The method of claim 1 wherein the parameter related to cardiac output is a transthoracic impedance measurement.

3. The method of claim 1 further comprising performing a testing sequence in which pulse output configurations are alternately switched and then selecting the pulse output configuration in accordance with which one of the pulse output configurations produces the highest cardiac output.

The method of claim 1 wherein the subset of electrodes belonging to the first pulse output configuration includes at least one electrode fixed at a different site of the same or different heart chamber from a site at which the subset of electrodes belonging to the second pulse output configuration are fixed, where an electrode may be fixed to a heart chamber by leaving it to float inside one of the heart chambers near the heart wall, actively fixing the electrode in the myocardium by implantation, or passively fixing the electrode against the endocardium via a tissue encapsulation reaction.

The method of claim 1 wherein the pacing electrodes making up the subsets of electrodes belonging to the first and second pulse output configurations are connected to different pulse generators and further comprising switching between the first and second pulse output configurations by switching between the different pulse generators.

The method of claim 1 wherein the pacing electrodes making up the subsets of electrodes belonging to the first and second pulse output configurations are connected to common pulse generators by switching circuitry and further comprising switching between the first and second pulse output configurations by operating the switching circuitry.

7. The method of claim 1 wherein the stimulation pulses are output to a bipolar electrode having tip and ring electrodes disposed near a heart chamber, and further comprising switching the pulse output configuration by alternating the polarity of the delivered pulses.

8. The method of claim 7 wherein the bipolar electrode is disposed in the coronary sinus to stimulate the left ventricle.

9. The method of claim 1 wherein a pulse output configuration is further defined as a particular temporal sequence of pulses output to a selected subset of electrodes during a cardiac cycle.

10. A cardiac pacemaker, comprising:

pulse generating circuitry for connecting to a plurality of electrodes adapted for disposition near a heart chamber;

wherein the pulse generating circuitry is connectable to the electrodes in a plurality of defined pulse output configurations, wherein each pulse output configuration constitutes a subset of the plurality of the electrodes;

a control unit and pulse generating circuitry for outputting pacing pulses to a selected one of the plurality of pulse output configurations in accordance with a programmed mode;

a sensor for sensing a parameter related to cardiac output; and

,

wherein the control unit is programmed to alternately switch between a first pulse output configuration during one or more cardiac cycles and a second pulse output configuration during one or more cardiac cycles in accordance with a switching algorithm that comprises switching the pulse output configuration in a manner dependent upon the sensed parameter related to cardiac output, wherein the control unit is programmed to switch the pulse output configuration if the parameter related to cardiac output falls below a specified minimum value.

11. The pacemaker of claim 10 wherein the parameter related to cardiac output is a transthoracic impedance measurement.

The pacemaker of claim 10 wherein the control unit is programmed to perform a testing sequence in which pulse output configurations are alternately switched and then selecting the pulse output configuration in accordance with which one of the pulse output configurations produces the highest cardiac output.

The pacemaker of claim 10 wherein the subset of electrodes belonging to the first pulse output configuration includes at least one electrode adapted for fixation at a different site of the same or different heart chamber from site at which the subset of electrodes belonging to the second pulse output configuration is adapted to be fixed, where an electrode may be fixed to a heart chamber by leaving it to float inside one of the heart chambers near the heart wall, actively fixing the electrode in the myocardium by implantation, or passively fixing the electrode against the endocardium via a tissue encapsulation reaction.

The pacemaker of claim 10 wherein the pacing electrodes making up the subsets of electrodes belonging to the first and second pulse output configurations are connected to different pulse generators and wherein the control unit is programmed to switch between the first and second pulse output configurations by switching between the different pulse generators.

The pacemaker of claim 10 wherein the pacing electrodes making up the subsets of electrodes belonging to the first and second pulse output configurations are connected to a common pulse generator by switching circuitry and wherein the control unit is programmed to switch between the first and second pulse output configurations by operating the switching circuitry.

16. The pacemaker of claim 10 wherein the plurality of electrodes include a bipolar electrode having tip and ring electrodes disposed near a heart chamber, and wherein the control unit is programmed to switch the pulse output configuration by alternating the polarity of the delivered pulses.

17. The pacemaker of claim 16 wherein the bipolar electrode is adapted for disposition in the coronary sinus to stimulate the left ventricle.

18. The pacemaker of claim 10 wherein a pulse output configuration is further defined as a particular temporal sequence of pulses output to a selected subset of electrodes during a cardiac cycle.
