wherein the diameter of the disk is about 3.0 mm; and
wherein the upper surface has a radius of curvature of about 9.0 mm.

The optical disk is formed of a silicone having at least one weight percent mix of phosphor. Various embodiments of this generally described optical disk are also presented.
