1. A system for managing treatment of a particular health condition afflicting a patient, the system comprising:

  • a remote web server system connected to and controlling access to a database, wherein the web server system is accessible by multiple authorized users and stores multiple patient data sets, wherein the multiple patient data sets include patient health condition data received from a respective patient, and the web server system is configured to receive and transmit patient data;

    a patient local processing device configured to connect to the web server system via a network connection, the patient local processing device comprising a display, a communication port, an input device, a processor, a nonvolatile memory, and a health management program stored on the memory and configured to manage treatment of a particular health condition, wherein the processor is configured to access the memory, load into the processor and run the health management program which programs the processor to;

    control the display of the patient local processing device to issue a visual prompt for entry of patient health condition data, including subjective patient symptom data in a predefined drop-down list of subjective symptoms that may be individually selected and entered as data with the input device, and medication delivery data, and to issue a prompt reminding the patient of a planned activity;

    receive patient health condition data related to a particular health condition afflicting a patient, including analyte level data with associated times at which the analyte level data were taken from a patient, and further including subjective patient symptom data with associated times at which the symptoms were experienced from the predefined drop-down list, via the input device;

    store the received patient health condition data, including subjective patient symptom data, in the memory of the patient local processing device along with the times associated with the data, so that analyte data may be associated with subject data at the same or relevant times to determine any possible influences of subjective symptoms on analyte levels; and

    send selected stored patient health condition data to the remote web server system for central storage in the database connected to the web server system, wherein the stored data is accessible by multiple authorized users.
