Abstract

An embodiment uses an accelerometer to sense heart sounds, and determines heart rate info nation using the sensed heart sounds. An embodiment uses an accelerometer to sense respiratory activity. An embodiment delivers a programmed neural stimulation therapy with a programmed duty cycle, where the programmed duty cycle includes a stimulation ON portion followed by a stimulation OFF portion. An electrode electrically connected to the implanted neural stimulation device is used to remotely detect cardiac activity. The remotely detected cardiac activity is used to detect heart rate information during the stimulation ON portion and to detect heart rate information during the stimulation OFF portion. The detected heart rate information and/or the detected respiration information are used to control a neural stimulation therapy performed by the neural stimulator device and/or are used to provide diagnostic information for the patient'"'"'s condition.

Claims

1. A system for stimulating a neural target, comprising:

an accelerometer;

a neural stimulator electrically connected to at least one electrode, wherein the neural stimulator is configured to deliver a neural stimulation signal through the at least one electrode to stimulate the neural target;

a heart rate detector operationally connected to the accelerometer to detect heart sounds using the accelerometer, and to determine a heart rate using the detected heart sounds, wherein the detected heart sounds include S1 and S2 heart sounds; and

a controller configured to determine a left ventricular ejection time using the detected S1 and S2 sounds.

2. The system of claim 1, further comprising a controller configured to control, the neural stimulator to deliver a neural stimulation therapy, and to modify the neural stimulation therapy using the heart rate determined using the detected heart sounds.

The system of claim 1, wherein the heart rate detector is configured to determine intervals between a distinguishable type of heart sound, wherein the intervals are intervals between successive S1 sounds, or intervals between successive S2 sounds, or intervals between successive S3 sounds, or intervals between successive S4 sounds.

4. The system of claim 3, wherein the heart rate detector is configured to determine the heart rate using a plurality of intervals.

5. The system of claim 1, wherein the heart rate detector is configured to detect an arrhythmia using the detected heart sounds, and the controller is configured to modify or interrupt the neural stimulation therapy in response to the detected arrhythmia.

6. The system of claim 1, wherein the heart rate detector is configured to determine heart rate variability (HRV) using the detected heart sounds.

7. A method for operating an implanted neural stimulation device, comprising:

using an accelerometer to sense heart sounds including S1 and S2 sounds; and

determining heart rate information using the sensed heart sounds;

using the heart rate information to;

control a neural stimulation therapy performed by the neural stimulation device;

or

perform diagnostics; and

determining a left ventricular ejection time using the detected S1 and S2 sounds.

8. The method of claim 7, wherein using the accelerometer to sense heart sounds includes sensing a distinguishable type of heart sounds where the distinguishable type of heart sounds is S1 or S2 or S3 or S4, and determining heart rate information includes determining an interval between successive occurrences of the distinguishable type of heart sound and determining a heart rate using the interval.

9. The method of claim 8, wherein determining heart rate information includes determining intervals between successive occurrences of the distinguishable type of heart sound, averaging the intervals, and determining the heart rate using the average interval.

10. The method of claim 7, wherein determining heart rate information includes determining heart rate variability using the sensed heart sounds.

11. The method of claim 7, wherein:

determining heart rate information using the sensed heart sounds includes detecting an arrhythmia using the sensed heart sounds, and

using the heart rate information to control the neural stimulation therapy includes interrupting or modulating the neural stimulation therapy in response to a detected arrhythmia.

12. The method of claim 7, further comprising:

using the implanted neural stimulation device to deliver a programmed neural stimulation therapy with a programmed duty cycle, wherein the programmed duty cycle includes a stimulation ON portion followed by a stimulation OFF portion;

wherein determining heart rate information using the sensed heart rate sounds includes determining heart rate information during the stimulation ON portion of the duty cycle and determining heart rate information during the stimulation OFF portion of the duty cycle.

13. The method of claim 12, further comprising trending heart rate information during the stimulation ON portion of the duty cycle and trending heart rate information during the stimulation. OFF portion of the duty cycle.

14. The method of claim 12, further comprising detecting a first heart rate change during the stimulation ON portion of the duty cycle, or detecting a second heart rate change during the stimulation OFF portion of the duty cycle, or both detecting the first heart rate change during the stimulation ON portion and detecting the second heart rate change during the stimulation OFF portion.

15. A method for operating an implanted neural stimulation device, comprising:

using an accelerometer to sense respiratory activity; and

using the respiratory activity to;

control a neural stimulation therapy performed by the neural stimulation device;

or

perform diagnostics,

wherein using the accelerometer to sense respiratory activity includes using the accelerometer to determine respiratory flow, and using the respiratory activity to control the neural stimulation therapy includes using the respiratory flow to control the neural stimulation therapy.

16. The method of claim 15, wherein:

using the accelerometer to sense respiratory activity includes using the accelerometer to determine a respiratory rate; and

using the respiratory activity to control the neural stimulation therapy includes using the respiratory rate to control the neural stimulation therapy.

17. The method of claim 15, wherein:

using the accelerometer to sense respiratory activity includes using the accelerometer to detect apnea; and

using the respiratory activity to control the neural stimulation therapy includes using the detected apnea to control the neural stimulation therapy.

18. The method of claim 15, further comprising bandpass filtering a signal from the accelerometer to pass a frequency range from approximately 75 Hz to 1500 Hz.

19. The method of claim 15, further comprising bandpass filtering a signal from the accelerometer to pass a frequency range from approximately 75 Hz to 600 Hz.

20. The method of claim 15, further comprising removing heart sounds from a signal from the accelerometer.

21. A method for operating an implanted neural stimulation device, comprising:

using the implanted neural stimulation device to deliver a programmed neural stimulation therapy with a programmed duty cycle, wherein the programmed duty cycle includes a stimulation ON portion followed by a stimulation OFF portion;

using an electrode electrically connected to the implanted neural stimulation device to remotely detect cardiac activity, including using the remotely detected cardiac activity to detect heart rate information during the stimulation ON portion and to detect heart rate information during the stimulation OFF portion; and

using the detected heart rate information during the stimulation ON portion and the detected heart rate information during the stimulation OFF portion to;

control a neural stimulation therapy performed by the neural stimulation device;

or

perform diagnostics.

22. The method of claim 21, further comprising detecting:

a first change in heart rate or AV duration during the ON portion of the duty cycle;

a second change in heart rate or AV duration during the OFF portion of the duty cycle;

or

both the first change and the second change; and

modifying the neural stimulation therapy using;

the first change;

the second change;

or

the first change and the second change.

23. The method of claim 21, further comprising:

trending the detected heart rate during the stimulation ON portion or during the stimulation OFF portion or during both the stimulation ON portion and the stimulation OFF portion; and

controlling the neural stimulation therapy using the trended heart rate for the ON portion or the trended heart rate for the OFF portion or both the trended heart for the ON portion and the OFF portion.

24. The method of claim 21, further comprising detecting an arrhythmia using the electrode electrically connected to the implanted medical device, and interrupting or modulating the neural stimulation therapy in response to a detected arrhythmia.

25. An implanted neural stimulator, comprising:

an implantable housing with a header for connecting leads;

a neural stimulator lead configured to connect to the header;

an antenna lead configured to connect to the header, the antenna lead including at least one electrode for use to remotely detect cardiac activity;

a neural stimulator configured to deliver a neural stimulation therapy through the neural stimulator lead; and

a controller configured to detect cardiac activity using the antenna lead, and control the neural stimulator using the detected cardiac activity.

26. An implanted neural stimulator for stimulating a target nerve, comprising:

an implantable housing with a header for connecting a lead;

a lead configured to connect to the header, wherein the lead includes a strain relief cuff configured to be positioned around the target nerve, at least one cardiac ECG sensing electrode configured for use to remotely detect cardiac activity, a neural electrode cuff with at least one neural stimulation electrode configured for use to stimulate the target nerve, and at least one neural sensing electrode configured for use to detect action potentials in the target nerve;

a neural stimulator configured to deliver a neural stimulation therapy through a neural stimulation electrode; and

a controller configured to detect cardiac activity using a cardiac ECG sensing electrode and to detect neural activity using the neural sensing electrode, and control the neural stimulator using the detected cardiac activity and the detected neural activity.

27. A method for operating an implanted neural stimulation device, comprising:

using an accelerometer to sense heart sounds;

using an electrode electrically connected to the implanted neural stimulation device to remotely detect cardiac activity; and

using both the heart sounds and the remotely detected cardiac activity to;

control a neural stimulation therapy performed by the neural stimulation device;

or

perform diagnostics.
