Abstract

A long term oxygen therapy system having an oxygen supply directly linked with a patient'"'"'s lung or lungs may be utilized to more efficiently treat hypoxia caused by chronic obstructive pulmonary disease such as emphysema and chronic bronchitis. The system includes an oxygen source, one or more valves and fluid carrying conduits. The fluid carrying conduits link the oxygen source to diseased sites within the patient'"'"'s lungs. A collateral ventilation bypass trap system directly linked with a patient'"'"'s lung or lungs may be utilized to increase the expiratory flow from the diseased lung or lungs, thereby treating another aspect of chronic obstructive pulmonary disease. The system includes a trap, a filter/one-way valve and an air carrying conduit. In various embodiments, the system may be intrathoracic, extrathoracic or a combination thereof. A pulmonary decompression device may also be utilized to remove trapped air in the lung or lungs, thereby reducing the volume of diseased lung tissue. A lung reduction device may passively decompress the lung or lungs. In order for the system to be effective, an airtight seal between the parietal and visceral pleurae is required. Chemical pleurodesis is utilized for creating the seal and various devices and/or drugs, agents and/or compounds may be utilized to accelerate wound healing in thoracic anastomosis applications.

Claims

1. A method to create an anastomosis through a chest wall joined to an opening in a visceral membrane of a lung comprising the steps of:

forming an adhesion between a chest wall and a visceral membrane of the lung that is sealed from the pleural space;

forming a channel through the adhesion which communicates with an opening in the visceral membrane of the lung; and

inserting a structural framework in the channel such that said structural framework is in contact with the channel and which structural framework is a lattice that can promote fibrosis.

2. The method of claim 1 wherein said inserting step includes:

inserting the structural framework in order to have a wound healing response.

3. The method of claim 1 wherein said inserting step includes:

inserting the structural framework that is a fabric.

4. The method of claim 1 wherein said inserting step includes:

inserting the structural framework that is porous.

5. The method of claim 1 wherein said inserting step includes:

inserting the structural framework that is includes a polyester material.

6. The method of claim 1 wherein said inserting step includes:

inserting the structural framework that a shape memory material.

7. The method of claim 1 wherein said inserting step includes:

securing the structural framework in the channel with at least one or more of sutures, staples, and adhesives.

8. The method of claim 1 wherein said inserting step includes:

inserting the structural framework that promotes tissue in-growth.

9. The method of claim 1 wherein said inserting step includes:

inserting the structural framework that has been treated with one or more of an antibiotic, and an agent for preventing infection and an agent for eradicating infection.

10. The method of claim 1 wherein said inserting step includes:

inserting the structural framework that can elute a drug.

11. The method of claim 1 wherein said inserting step includes:

inserting the structural framework that has at least one of a cuff or a pocket that contains a drug.

12. The method of claim 1 wherein said inserting step includes:

inserting the structural framework that includes collagen.

13. The method of claim 1 wherein said inserting step includes:

inserting the structural framework that encourages collagen production.

14. The method of claim 1 wherein said inserting step includes:

inserting the structural framework that includes at least one of Platelet-derived Growth Factor and Platelet Factor 4.

15. The method of claim 1 wherein said inserting step includes:

inserting the structural framework that includes a material that promotes platelet aggregation.

16. The method of claim 1 wherein said inserting step includes:

inserting the structural framework that includes at least one of Tissue Factor/VIIa, thrombin, fibrin, plasminogin-activator initiator, adenosine diphosphate and collagen.

17. The method of claim 1 wherein one of:

said adhesion forming step and said channel forming step occur simultaneously with each other, the adhesion forming step occurs before the channel forming step, and the channel forming step occurs before the adhesion forming step.

18. The method of claim 1 including the step of:

delivering a drug to at least one of the structural framework and the channel.

19. A method to create an anastomosis which includes a channel through a chest wall joined to an opening in a visceral membrane of a lung comprising the steps of:

creating a channel through a chest wall into a pleural a pleural space surrounding a lung;

forming an adhesion between the chest wall and a visceral membrane of the lung to seal the channel from the pleural space;

creating an opening in the visceral membrane of the lung which communicates with the channel; and

inserting a structural framework in the channel such that said structural framework is in contact with the channel and which structural framework is a lattice that can promote fibrosis.

20. A method to create an anastomosis through a chest wall joined to an opening in a visceral membrane of a lung comprising the steps of:

forming an adhesion between a chest wall and a visceral membrane of the lung that is sealed from the pleural space with a channel through the adhesion which communicates with an opening in the visceral membrane of the lung; and

inserting a structural framework in the channel such that said structural framework is in contact with the channel and which structural framework is a lattice that can promote fibrosis.
