Abstract

An energy delivering probe is used for thermally coagulating and/or constricting hollow anatomical structures (HAS) including, but not limited to, blood vessels such as perforator veins. The probe includes a shaft and an energy source.

Claims

1. A method of treating a perforator vein comprising:

achieving direct percutaneous access into tissue with a rigid stylet having a resistive heating coil, at least one electrode forming a portion of an exterior surface of the stylet, and a sharp tip and without the use of an introducer sheath, the resistive heating coil being covered by an electrically insulative sleeve;

advancing the stylet to the perforator vein; and

occluding the perforator vein by applying energy from the resistive heating coil to the perforator vein.

2. The method of claim 1, wherein occluding further comprises applying energy to an exterior surface of the perforator vein.

3. The method of claim 1, wherein occluding further comprises applying energy to an interior vessel wall of the perforator vein.

4. The method of claim 1, wherein the sharp tip is fixedly received within the stylet.

5. The method of claim 1, wherein the resistive heating coil is at a distal section of the stylet.

6. The method of claim 1, wherein the resistive heating coil is tapered.

7. The method of claim 1, wherein the steps of advancing and occluding are performed under ultrasound guidance.

8. The method of claim 1, wherein the step of advancing the stylet to the perforator vein further comprises advancing the stylet to the perforator vein below a fascial layer, the fascial layer comprising deep fascia through which the perforator vein passes to connect a superficial vein with a deep vein.

9. The method of claim 8, wherein occluding the perforator vein comprises occluding the perforator vein below the fascial layer.

10. The method of claim 1, wherein occluding the perforator vein comprises occluding a portion of the perforator vein.

11. The method of claim 1, wherein the at least one electrode comprises two electrodes.

12. A method comprising:

directly accessing a target site with a rigid probe through the skin, the target site being within or near a perforator vein, the rigid probe comprising;

a tapered resistive heating coil on an outside surface of and near a distal end of the probe;

at least one electrode on the outside surface of the probe adjacent the resistive heating coil;

a nonconductive sleeve on an outside surface of the resistive heating coil; and

a sharp tip at a distal end of the probe and configured to penetrate tissue;

applying heat energy from the resistive heating coil to the perforator vein with the probe; and

constricting a region of the perforator vein.

13. The method of claim 12, wherein applying heat energy comprises applying heat energy to an exterior surface of the perforator vein.

14. The method of claim 12, wherein applying heat energy comprises applying heat energy to an interior vessel wall of the perforator vein.

15. The method of claim 12, further comprising constricting additional regions of the perforator vein.

16. The method of claim 12, further comprising retracting the sharp tip within the probe.

17. A method comprising:

achieving direct access with a rigid probe through the skin to a target site within or near a perforator vein, the rigid probe comprising;

a sharp tip at a distal end of the probe and configured to penetrate tissue;

a resistive heating coil proximal of the sharp tip, the resistive heating coil being covered by an electrically insulative sleeve; and

at least one electrode adjacent the resistive heating coil, the at least one electrode being configured to contact the perforator vein;

retracting the sharp tip within the probe;

applying energy to the perforator vein with the probe resistive heating coil; and

constricting a region of the perforator vein.

18. The method of claim 17, wherein the method further comprises positioning the resistive heating coil adjacent an exterior of the vein.

19. The method of claim 18, wherein applying energy further comprises energizing the resistive heating coil to heat tissue sufficiently to constrict the wall of the vein, without any penetration of the vein.

20. The method of claim 17, further comprising the steps of penetrating the perforator vein and advancing the probe within the perforator vein.

21. The method of claim 17, further comprising fully penetrating the perforator vein so that the sharp tip passes all the way through the vein to a far side of the vein.

22. The method of claim 21, wherein applying energy further comprises applying energy as the probe is drawn back from the far side of the vein and through the vein.

23. The method of claim 22, wherein constricting further comprises constricting as the probe is withdrawn from the vein.

24. The method of claim 21, wherein the at least one electrode is spherical or toroidal in shape and wherein penetrating comprises penetrating until the at least one electrode engages and collapses a portion of the vein.
