Abstract

A medical device programmer and a method of operation in which a first data value is received and used in the execution of one or more algorithms. One or more suggested pulse generator settings are calculated from the one or more algorithms based on the first data value, and the one or more suggested pulse generator settings are displayed on an interactive display screen of the medical device programmer. In one embodiment, the first data value is a duration interval of a QRS complex. From the duration interval, suggestions are made as to one or more ventricular chambers in which to provide pacing pulses. Additionally, pacing intervals for an AV delay are suggested based on measured P-R intervals, or pacing intervals for an LV offset are suggested based on a measured duration interval of a V-V-interval between a right ventricular event and a left ventricular event.

Claims

1. An external medical device to communicate with an implantable device, the external device comprising:

a user interface configured to receive a prompt to generate one or more therapy settings for the implantable device;

an LV Offset determiner communicatively coupled to the user interface and configured to;

communicate a request to the implantable device for information in response to the prompt;

receive a representation of a cardiac signal sensed in the RV and a representation of a cardiac signal sensed in the LV from the implantable device;

measure a duration interval between a cardiac event in the RV and a cardiac event in the LV using the received representations;

calculate, as a first generated therapy setting, an LV offset interval using the measured duration interval and a previous value of a first indicated pacing interval; and

provide the calculated LV offset interval to at least one of a user or a process executing on the external medical device or a process of a different device.

2. The external device of claim 1, wherein the LV Offset determiner is configured to:

calculate the LV offset interval using a first set of calculation coefficients when the measured duration interval ends with an intrinsic cardiac event; and

calculate the LV offset interval using a second set of calculation coefficients when the measured duration interval ends with a paced cardiac event.

3. The external device of claim 2, wherein the LV Offset determiner is configured to:

4. The external device of claim 2, wherein the LV Offset determiner is configured to:

determine a central tendency of a frequency with which biventricular pacing therapy is delivered by the implantable device;

compare the determined central tendency frequency of pacing to a specified biventricular pacing frequency target value; and

adjust calculation coefficients used to calculate the LV offset interval according to the comparison.

5. The external device of claim 2, including an AV delay determiner configured to:

communicate a request to the implantable device in response to the user prompt;

receive a representation of a cardiac signal sensed in an atrium in response to the communicated request;

measure, using the received representations of the sensed cardiac signals, a first time interval between a cardiac event in an atrium and a cardiac event in at least one ventricle and a second time interval between the cardiac event in the atrium and the cardiac event in the at least one ventricle; and

calculate an AV delay interval as a second generated therapy setting using the first and second measured time intervals.

6. The external device of claim 2, including a pacing site selector configured to:

receive a measure of a QRS complex duration into the external device;

measure a time of occurrence of a peak of depolarization of the cardiac signal sensed in the RV and a time of peak of occurrence of depolarization of the cardiac signal sensed in the LV; and

determine as a second generated therapy setting, a pacing site according to the measure of QRS complex duration and the time relationship of the depolarization peak of the RV and the depolarization peak of the LV.

7. The external device of claim 2, wherein the pacing site selector is configured to:

select a pacing site in the RV when the difference between the time of occurrence of the depolarization peak of the LV and the time of occurrence of the depolarization peak of the RV is less than or equal to zero; and

select a pacing site in the LV when the difference between the time of occurrence of the depolarization peak of the LV and the time of occurrence of the depolarization peak of the RV is less than or equal to zero.

8. The external device of claim 1, including a display screen:

display a first screen image to request the prompt via the user interface;

display a second screen image to present the measured duration interval between a cardiac event in the RV and a cardiac event in the LV; and

display a third screen image to present the calculated LV offset interval.

The external device of claim 8, wherein the external device is configured to display an estimated time to complete calculating the LV offset in response to receiving one or more of the representation of a cardiac signal sensed in the RV and the representation of a cardiac signal sensed in the LV.

10. The external device of claim 8, wherein the external device is configured to:

display, when requesting the user prompt, one or more portions of the screen image unrelated to requesting the user prompt darker than a portion related to requesting the user prompt; and

display, in response to receiving the user prompt, the portion related to the user prompt darker than a portion unrelated to requesting the user prompt.

11. A method of operating an external device, wherein the external device is capable of communicating with an implantable device, the method comprising:

receiving a prompt via a user interface of the external device to generate one or more therapy settings for the implantable device;

communicating a request to the implantable device for information in response to the prompt;

receiving a representation of a cardiac signal sensed in the RV and a representation of a cardiac signal sensed in the LV in response to the communicated request;

measuring, with the external device, a duration interval between a cardiac event in the RV and a cardiac event in the LV using the received representations of the sensed cardiac signals;

calculating, as a first generated therapy setting, an LV offset interval using the measured duration interval and a previous value of a first indicated pacing interval; and

providing the calculated LV offset to at least one of a user or a process executing on the external medical device or a process of a different device.

12. The method of claim 11, wherein calculating an LV offset interval includes:

calculating the LV offset interval using a first set of calculation coefficients when the measured duration interval ends with an intrinsic cardiac event; and

calculating the LV offset interval using a second set of calculation coefficients when the measured duration interval ends with a paced cardiac event.

13. The method of claim 12, wherein calculating the LV offset includes calculating a pacing interval T_n from T_n=a·

14. The method of claim 12, including:

determining, with the external device, a central tendency of a frequency with which biventricular pacing therapy is delivered by the implantable device;

comparing, using the external device, the determined central tendency frequency of pacing to a specified biventricular pacing frequency target value; and

adjusting, by the external device, calculation coefficients used to calculate the LV offset interval according to the comparison.

15. The method of claim 11, including:

communicating a request to the implantable device in response to the user prompt;

receiving, into the external device, a representation of a cardiac signal sensed in an atrium in response to the communicated request;

measuring, with the external device and using the received representations of the sensed cardiac signals, a first time interval between a cardiac event in an atrium and a cardiac event in at least one ventricle and a second time interval between the cardiac event in the atrium and the cardiac event in the at least one ventricle; and

calculating, using the external device, an AV delay interval as a second generated therapy setting using the first and second measured time intervals.

16. The method of claim 11, including:

receiving a measure of a QRS complex duration into the external device;

measuring, with the external device, a time of occurrence of a peak of depolarization of the cardiac signal sensed in the RV and a time of peak of occurrence of depolarization of the cardiac signal sensed in the LV; and

determining with the external device, as a second generated therapy setting, a pacing site according to the measure of QRS complex duration and the time relationship of the depolarization peak of the RV and the depolarization peak of the LV.

17. The method of claim 16, wherein determining a pacing site includes:

selecting a pacing site in the RV when the difference between the time of occurrence of the depolarization peak of the LV and the time of occurrence of the depolarization peak of the RV is less than or equal to zero; and

selecting a pacing site in the LV when the difference between the time of occurrence of the depolarization peak of the LV and the time of occurrence of the depolarization peak of the RV is less than or equal to zero.

18. The method of claim 11, including:

displaying a first screen image to request the prompt via the user interface;

displaying a second screen image to present the measured duration interval between a cardiac event in the RV and a cardiac event in the LV; and

displaying a third screen image to present a suggested setting of the LV offset.

19. The method of claim 18, including displaying an estimated time to complete calculating the LV offset in response to receiving one or more of the representation of a cardiac signal sensed in the RV and the representation of a cardiac signal sensed in the LV.

20. The method of claim 18,

wherein displaying the first screen image includes displaying one or more portions of the screen image unrelated to requesting the prompt darker than a portion related to the user prompt, and

wherein displaying the second screen image includes displaying the portion related to the user prompt darker than a portion unrelated to requesting the prompt.
