Abstract

A steerable rotate-to-advance device for accessing a bodily passageway, said steerable rotate-to-advance device comprising:

• • an inner flexible tube having a distal end and a proximal end, a longitudinal axis extending between said distal end and said proximal end, and a lumen extending from said distal end to said proximal end;
  • an outer flexible tube having a distal end and a proximal end, a longitudinal axis extending between said distal end and said proximal end, and a lumen extending from said distal end to said proximal end;
  • the outer flexible tube being disposed coaxially over the inner flexible tube and being rotatable with respect thereto;
  • an external thread disposed over said distal end of said outer flexible tube, said external thread having a sufficient structural integrity, and a sufficient surface profile, such that when said outer flexible tube is disposed in a bodily passageway, rotation of said outer flexible tube about said longitudinal axis will result in longitudinal motion of said outer flexible tube along said bodily passageway; and
  • at least one articulating cable secured to the distal end of said inner flexible tube at an attachment point, and extending proximally along said inner flexible tube, such that pulling the at least one articulating cable proximally causes the inner flexible tube to be bent toward the radial position of said attachment point, whereby to cause the steerable rotate-to-advance device to be steered in the direction of the attachment point.

Claims

1. A steerable overtube for accessing a bodily passageway, said steerable overtube comprising:

an inner flexible tube having a distal end and a proximal end, a longitudinal axis extending between said distal end and said proximal end, and a lumen extending from said distal end to said proximal end;

an outer flexible tube having a distal end and a proximal end, a longitudinal axis extending between said distal end and said proximal end, and a lumen extending from said distal end to said proximal end;

said outer flexible tube being disposed coaxially over said inner flexible tube and being rotatable with respect thereto;

an external thread disposed over said distal end of said outer flexible tube, said external thread having a sufficient structural integrity, and a sufficient surface profile, such that when said outer flexible tube is disposed in a bodily passageway, rotation of said outer flexible tube about said longitudinal axis will result in longitudinal motion of said outer flexible tube along said bodily passageway; and

at least one articulating cable disposed between said outer flexible tube and said inner flexible tube, and secured to the distal end of said inner flexible tube at an attachment point, and extending proximally along said inner flexible tube, such that pulling the at least one articulating cable proximally causes the inner flexible tube to be bent toward the radial position of said attachment point, whereby to cause the steerable overtube to be steered in the direction of the attachment point.

2. A steerable overtube according to claim 1 comprising a pair of articulating cables, and further wherein the pair of articulating cables have diametrically-opposed attachment points.

3. A steerable overtube according to claim 1 comprising a plurality of articulating cables, and further wherein the plurality of articulating cables are equally-circumferentially-spaced about the circumference of the flexible inner tube.

4. A steerable overtube according to claim 1 wherein said steerable overtube further comprises visualization apparatus for visualizing structures disposed adjacent to said distal end of said tube.

9. A steerable overtube according to claim 2 wherein the inner flexible tube comprises a pair of parallel channels formed in an outer surface of the inner flexible tube, and further wherein the channels are formed to receive the pair of articulating cables.

10. A steerable overtube according to claim 9 wherein the outer flexible tube is free to rotate relative to the inner flexible tube and the pair of articulating cables.

11. A steerable overtube according to claim 10 wherein the outer flexible tube comprises a distal smooth tube and a proximal smooth tube, and further wherein the distal smooth tube is thinner, and has lower durometer, than the proximal smooth tube.

12. A steerable overtube according to claim 11 wherein the external thread is disposed on at least a portion of the outer surface of the distal smooth tube.

13. A steerable overtube according to claim 10 wherein a collet is disposed at said proximal end of said inner flexible tube, and wherein said collet can selectively secure a device extending through said inner flexible tube.

A steerable overtube according to claim 13 wherein the proximal ends of the pair of articulating cables are attached to a spool rotatably mounted to said collet so that rotation of said spool in one direction pulls one of the articulating cables, and rotation of said spool in an opposite direction pulls the other of the articulating cables.

5. A method for accessing a bodily passageway, the method comprising:

providing a steerable overtube, said steerable overtube comprising;

an inner flexible tube having a distal end and a proximal end, a longitudinal axis extending between said distal end and said proximal end, and a lumen extending from said distal end to said proximal end;

an outer flexible tube having a distal end and a proximal end, a longitudinal axis extending between said distal end and said proximal end, and a lumen extending from said distal end to said proximal end;

the outer flexible tube being disposed coaxially over said inner flexible tube and being rotatable with respect thereto;

an external thread disposed over said distal end of said outer flexible tube, said external thread having a sufficient structural integrity, and a sufficient surface profile, such that when said outer flexible tube is disposed in a bodily passageway, rotation of said outer flexible tube about said longitudinal axis will result in longitudinal motion of said outer flexible tube along said bodily passageway; and

at least one articulating cable disposed between said outer flexible tube and said inner flexible tube, and secured to the distal end of said inner flexible tube at an attachment point, and extending proximally along said inner flexible tube, such that pulling the at least one articulating cable proximally causes the inner flexible tube to be bent toward the radial position of said attachment point, whereby to cause the steerable overtube to be steered in the direction of the attachment point;

positioning the steerable overtube in said bodily passageway; and

pulling the at least one articulating cable proximally.

6. A method according to claim 5 wherein the steerable overtube comprises a pair of articulating cables, and further wherein the pair of articulating cables have diametrically-opposed attachment points.

7. A method according to claim 5 wherein the steerable overtube comprises a plurality of articulating cables, and further wherein the plurality of articulating cables are equally-circumferentially-spaced about the circumference of the flexible inner tube.

8. A method according to claim 5 wherein the steerable overtube further comprises visualization apparatus for visualizing structures disposed adjacent to said distal end of said tube.
