1. A trans-luminal, guidewire-advanced, rapid-exchange surgical delivery device having a proximal end, a primary shaft and a distal zone to be advanced over a guidewire along a bodily lumen to a site of surgery, the distal zone comprising:

  • a sleeve including a tapered proximal portion having a guidewire lumen exit port, a proximal end through which the primary shaft extends, and a distal end tapered to a leading annulus that defines approximately an outer diameter of the guidewire;

    a swaging ring compressing an annulus of the sleeve adjacent a distal end of the primary shaft;

    a shaped member positioned within the sleeve, including a first guidewire lumen and a second lumen, the shaped member having a tapered distal end; and

    a push wire extending through the primary shaft, through the second lumen of the shaped member and connecting to a pusher ring.
