1. A method for crossing through an anatomical structure, the method comprising:
- advancing an elongate member intravascularly to a location proximate an anatomical structure, the elongate member having a proximal end removably coupled to a housing and a drive shaft rotatably disposed within the elongate member extending from a motor positioned in the housing to a distal tip rotatably disposed at a distal end of the elongate member;
rotating the drive shaft within the elongate member with the motor to thereby rotate the distal tip;
forming an opening through the anatomical structure with the distal tip while rotating the drive shaft within the elongate member with the motor;
decoupling the housing from the elongate member while retaining a distal portion of the elongate member proximate the opening; and
advancing an access device over the elongate member to the anatomical structure after decoupling the housing from the elongate member.

advancing a hollow guidewire intravascularly to a location proximate an anatomical structure, the guidewire having a proximal end removably coupled to a housing and a drive shaft extending through the hollow guidewire, a proximal end of the drive shaft being coupled to a motor shaft of a motor positioned in the housing and a distal end of the drive shaft being coupled to a distal tip rotatably disposed at a distal end of the guidewire; rotating the drive shaft within the guidewire with the motor to form an opening through the anatomical structure with the distal tip; decoupling the guidewire and the drive shaft from the housing and the motor shaft, respectively, while retaining the guidewire proximate opening through the anatomical structure; and advancing an access device over the guidewire to the opening of the anatomical structure after decoupling the guidewire and the drive shaft from the housing and the motor shaft. - View Dependent Claims (11, 12, 13, 14, 15, 16, 17)
