Abstract

A cardiac rhythm management system recognizes patterns of interval durations, distinguishing between events in different heart chambers even though signals associated with those different heart chambers are processed using a commonly shared sensing circuit. A therapy delivery algorithm ignores intervals between cardiac events occurring in different heart chambers when determining a cardiac rate upon which the delivery of therapy is based. This reduces the risk of inappropriate delivery of therapy to the patient. Delayed conduction left ventricular beats are not erroneously recognized as a subsequent right ventricular beat, preventing such short intervals from inappropriately triggering a defibrillation countershock.

Claims

1. A method of operating a pacemaker, comprising:

sensing right and left ventricular depolarizations through a single channel;

implementing a programmable timing interval to determine that two successive ventricular events occur in different ventricular chambers; and

delivering or withholding pacing therapy based on the determination that the two successive ventricular events occur in different ventricular chambers.

2. The method of claim 1, implementing the programmable timing interval comprises comparing durations between cardiac events to the programmable timing interval.

3. The method of claim 2, wherein comparing the durations to the programmable timing interval comprises comparing the durations of previous and current interval to the programmable timing interval.

4. The method of claim 2, wherein a duration between the cardiac events that is greater than the timing interval is classified as long and a duration between the cardiac events that is less than the timing interval are classified as short.

5. The method of claim 1, further comprising initiating a refractory period following a ventricular depolarization.

6. An implantable cardiac device, comprising:

a sensing circuit configured to sense cardiac events through a single channel, the cardiac events including right and left ventricular depolarizations;

a timer configured to implement a programmable timing interval between right and left ventricular depolarizations;

a pattern recognition module configured to use the timing interval to recognize a pattern of the durations of intervals between the cardiac events; and

control circuitry configured to control delivery of cardiac therapy contingent on the recognized pattern;

a therapy circuit configured to deliver cardiac therapy contingent on whether or not two successive ventricular events occur in different ventricular chambers during a cardiac cycle.

7. The device of claim 6, wherein the therapy circuit is configured to control delivery of pacing therapy to the right and left ventricles.

8. The device of claim 6, wherein the controller is configured to initiate a post-sense refractory period after detection of a ventricular depolarization.

9. The device of claim 6, wherein the sensing circuit comprises a bifurcated lead coupled to both a right ventricular electrode and a left ventricular electrode.

10. The device of claim 6, wherein the sensing circuit comprises:

a right ventricular lead comprising a right ventricular conductor that is attached to a first receptacle in a header of the medical device; and

a left ventricular lead comprising a left ventricular conductor that is attached to a second receptacle in the header of the medical device.

11. The device of claim 10, wherein the right ventricular conductor is electrically coupled to the left ventricular conductor within the header.

12. The device of claim 6, wherein the sensing circuit is configured to sense right ventricular depolarizations via a right ventricular electrode and to sense left ventricular depolarizations via a left ventricular electrode and receives no other independent information in which the sensed signals originate.
