Abstract

A system detects events related to cardiac activity. The system comprises a primary cardiac signal sensing circuit, at least one secondary cardiac signal sensing circuit having a higher sensitivity than the primary sensing circuit, and a controller circuit coupled to the primary and secondary cardiac signal sensing circuits. The controller circuit determines a rate of depolarization using the primary sensing circuit and detects tachyarrhythmia using the rate. The controller circuit also detects tachyarrhythmia using the secondary sensing circuit and also deems the tachyarrhythmia valid if the controller circuit detects the tachyarrhythmia using both the primary and secondary sensing circuit.

Claims

1. A system comprising:

a first cardiac signal sensing circuit operable to sense at least a first intrinsic cardiac signal, wherein the first intrinsic cardiac signal is associated with a ventricular depolarization of a heart, and wherein the first sensing circuit is configured to detect bradyarrhythmia;

a second cardiac signal sensing circuit operable to sense at least a second intrinsic cardiac signal, the second intrinsic cardiac signal also associated with the ventricular depolarization of the heart, wherein the second sensing circuit has a higher signal sensing sensitivity than the first sensing circuit and is configured to detect tachyarrhythmia;

a pacing therapy circuit; and

a controller circuit coupled to the first and second cardiac signal sensing circuits and the pacing therapy circuit, the controller circuit operable to adjust a sensing threshold of the first cardiac signal sensing circuit based on a comparison of outputs of the first and second cardiac signal sensing circuits.

The system of claim 1, wherein the controller circuit includes an escape interval timer and wherein the controller circuit restarts the escape interval timer at the occurrence of a sensed depolarization event if the depolarization event is sensed by both the first and second cardiac signal sensing circuits.

3. The system of claim 1, wherein the first cardiac signal sensing circuit and the second cardiac signal sensing circuit both include a same sensing vector.

4. The system of claim 1, wherein the first cardiac signal sensing circuit includes a first sensing vector and the second cardiac signal sensing circuit includes a second sensing vector.

5. The system of claim 4, wherein system further includes a high-energy shock therapy circuit coupled to the controller and the second cardiac signal sensing circuit includes at least one electrode operable to deliver high-energy shock therapy.

6. The system of claim 4, wherein the first cardiac signal sensing circuit includes a tip electrode and a ring electrode of a cardiac lead and the second cardiac signal sensing circuit includes a can electrode and a shock therapy coil electrode.

7. The system of claim 4, wherein the first cardiac signal sensing circuit includes a tip electrode and a ring electrode of a cardiac lead and the second cardiac signal sensing circuit includes a can electrode and one of a ring electrode or a tip electrode of another cardiac lead.

8. The system of claim 4, wherein the second cardiac signal sensing circuit includes a wireless ECG system.

The system of claim 1, wherein the second cardiac signal sensing circuit has a lower signal sensing threshold than the first cardiac signal sensing circuit, or the second cardiac signal sensing circuit provides greater amplification to an input cardiac signal than the first cardiac signal sensing circuit, or the second cardiac signal sensing circuit both has a lower signal sensing threshold and provides greater amplification to an input cardiac signal than the first cardiac signal sensing circuit.

10. The system of claim 9, wherein the controller circuit is further operable to determine time intervals between sensed intrinsic R-waves using signals sensed by the second cardiac signal sensing circuit and detect tachyarrhythmia using the time intervals.

11. The system of claim 1, wherein the controller circuit includes an escape interval timer and wherein the controller circuit is further operable to restart the escape interval timer and inhibit pacing therapy if a depolarization event is sensed only by the first cardiac signal sensing circuit.

The system of claim 1, wherein the controller circuit is further operable to deem a depolarization event sensed by the second cardiac signal sensing circuit to be a noise if event when all three of a first depolarization event is sensed only by the second cardiac signal sensing circuit, a subsequent depolarization event is sensed by both the first and second cardiac signal sensing circuits, and the subsequent depolarization event is deemed a normal depolarization event.

13. The system of claim 12, wherein the controller circuit deems a depolarization event to be a normal depolarization event when time intervals determined between sensed intrinsic R-waves substantially matches an expected time interval.

14. The system of claim 12, wherein the controller circuit deems a depolarization event to be a normal depolarization event when a peak value of an R-wave matches a specified normal R-wave peak value.

15. The system of claim 12, wherein the controller circuit deems a depolarization event to be a normal depolarization event when an R-wave morphology matches a specified normal R-wave morphology.

16. The system of claim 1, wherein the controller circuit is further operable to adjust a sensing threshold of the first cardiac signal sensing circuit if at least one depolarization event is sensed only by the second cardiac signal sensing circuit.

17. The system of claim 16, wherein the controller circuit is configured to limit a maximum of the adjusted sensing threshold of the first cardiac signal sensing circuit when the depolarization event is sensed only by the second cardiac signal sensing circuit.

18. The system of claim 1, wherein the controller circuit is further operable to deem a depolarization event as a noise event or a tachyarrhythmia event if an R-wave is sensed only by the second cardiac signal sensing circuit.

19. The system of claim 1, wherein the first and second cardiac signal sensing circuits include an automatic gain control circuit, and the automatically determined gain of the second cardiac signal sensing circuit is greater than the gain of the first cardiac signal sensing circuit.

20. The system of claim 1, wherein the first and second sensing cardiac signal circuits include an automatic sensing control circuit configured to automatically change a sensing threshold, and wherein the automatically determined threshold of the second cardiac signal sensing circuit changes faster than the threshold of the first cardiac signal sensing circuit.
