One or more computer storage devices storing computer-useable instructions that, when used by one or more computing devices, cause the one or more computing devices to perform a method providing outcome information associated with a patient plan of care in conjunction with documentation of assessment items for a patient, the method comprising:
- receiving a selection of one or more assessment items for a patient for documentation by a clinician;
presenting a documentation area, the documentation area having one or more areas configured to receive patient assessment information for the selected one or more assessment items;
presenting an outcome icon in conjunction with at least one of the one or more assessment items, the outcome icon relating to at least one outcome corresponding with at least one of the one or more selected assessment items and being based on a patient plan of care, wherein presenting an outcome icon comprises identifying at least one outcome from the patient plan of care corresponding with at least one of the selected one or more assessment items;
receiving a selection of the outcome icon;
presenting an outcome window in response to receiving the selection of the outcome icon, wherein presenting the outcome window comprises presenting information associated with at least one of the one or more outcomes in conjunction with the documentation area for entering patient assessment information, wherein the information presented in the outcome window corresponds to the assessment item associated with the selected outcome icon;
receiving patient assessment information from the clinician for at least one of the one or more assessment items corresponding with an outcome from the patient plan of care;
comparing the received patient assessment information against the corresponding outcome;
based at least in part on comparing the received patient assessment information against the corresponding outcome, determining whether the corresponding outcome has been met;
and presenting information associated with the outcome from the patient plan of care that corresponds to the received patient assessment information, wherein the information associated with the outcome is presented in context of the documentation area, the information including an indication regarding whether the outcome has been met,wherein presenting information associated with the outcome in context of the documentation area comprises simultaneously presenting
1) at least a portion of the documentation area for documenting one or more assessment items for the patient and
2) a summary window comprising met outcomes for one or more patient assessment items, and unmet outcomes for one or more patient assessment items.

Abstract
Outcomes from a patient plan of care are provided in conjunction with documentation of patient assessment items to assist clinicians in the healthcare process. A documentation area for documenting assessment items for a patient and having areas for entering patient assessment information is presented. Additionally, an outcome icon is presented in conjunction with at least one assessment item. The outcome icon relates to at least one outcome corresponding with the assessment item and is based on the patient plan of care. In response to receiving a user selection of the outcome icon, information associated with the outcome is presented in context of the documentation area.

receiving a selection of a patient assessment item by a clinician documenting one or more items of patient assessment information corresponding to the selected patient assessment item; presenting a documentation user interface, the documentation user interface having one or more areas configured to receive the one or more items of patient assessment information for the selected patient assessment item; receiving the one or more items of patient assessment information for the selected patient assessment item, wherein receiving the one or more items of patient assessment information comprises receiving one or more items of patient assessment information from the clinician via the documentation user interface, wherein the documentation user interface further includes one or more items of historical patient assessment information for the patient assessment item, wherein the one or more items of patient assessment information are received from the clinician in the context of the one or more items of historical patient assessment information based on the documentation user interface simultaneously presenting; (1) the area configured to receive the clinician-entered one or more items of patient assessment information for a corresponding patient assessment item and (2) the one or more items of historical patient assessment information for the patient assessment item; comparing the received one or more items of patient assessment information against one or more outcomes from a patient plan of care; based on comparing the received one or more items of patient assessment information against the one or more outcomes from the patient plan of care, determining at least one outcome of the one or more outcomes from the patient plan of care that corresponds to the received patient assessment information; presenting, on the documentation user interface, information associated with the at least one outcome of the one or more outcomes in conjunction with the area configured to receive the clinician entered one or more items of patient assessment information, wherein the documentation user interface simultaneously presents; (1) the area configured to receive the clinician-entered at least one item of patient assessment information corresponding to the selected patient assessment item, (2) the one or more items of historical patient assessment information for the selected patient assessment item, and (3) the information associated with the at least one outcome of the one or more outcomes, wherein the clinician may review outcomes from the patient plan of care while the clinician enters the patient assessment information; determining whether each of the one or more outcomes compared against the patient assessment information has been met; and presenting an indication whether each of the one or more outcomes has been met, wherein presenting an indication of whether each of the one or more outcomes has been met comprises simultaneously presenting; (1) met outcomes for one or more patient assessment items corresponding to the selected patient assessment item; (2) unmet outcomes for one or more patient assessment items corresponding to the selected patient assessment item; and (3) at least one of the one or more items of historical patient assessment information, wherein the documentation user interface comprises the one or more areas configured to receive the clinician-entered one or more items of patient assessment information for the corresponding patient assessment item in the context of a summary of met and unmet outcomes. - View Dependent Claims (8, 9)
