Abstract

A power supply for an implantable cardioverter-defibrillator for subcutaneous positioning between the third rib and the twelfth rib and using a lead system that does not directly contact a patient'"'"'s heart or reside in the intrathoracic blood vessels and for providing anti-tachycardia pacing energy to the heart, comprising a capacitor subsystem for storing the anti-tachycardia pacing energy for delivery to the patient'"'"'s heart; and a battery subsystem electrically coupled to the capacitor subsystem for providing the anti-tachycardia pacing energy to the capacitor subsystem.

Claims

1. A method of treating a patient including implanting a subcutaneous-only defibrillator, the subcutaneous-only defibrillator including an active housing containing operational circuitry and coupled to a subcutaneous lead having a proximal end for attachment to the active housing and a distal end including a coil electrode and a small electrode, in which implanting the subcutaneous-only defibrillator comprises:

implanting the housing lateral to the tip of the inferior portion of the scapula; and

implanting the subcutaneous lead in a path around the thorax with the distal end in the anterior precordial region;

wherein the coil electrode of the subcutaneous lead has a length in the range of 6-10 centimeters; and

wherein the step of implanting the subcutaneous lead comprises placing the small electrode close to the patient'"'"'s sternum in the tissues overlying the patient'"'"'s right ventricle on the anterior thorax of the patient.

2. The method of claim 1 further comprising the operational circuitry delivering pacing therapy using the small electrode.

3. The method of claim 1 wherein the housing comprises a fractal surface.

4. The method of claim 1 wherein the housing comprises a wrinkled surface.

5. The method of claim 1 wherein the step of implanting the housing comprises placing the housing at approximately the left posterior axillary line.

6. A method of implanting a defibrillator in a child, the defibrillator comprising an active housing and a subcutaneous lead having a distal end and a proximal end adapted for coupling to the housing, the method comprising:

implanting the housing at the left posterior axillary line and lateral to the tip of the inferior portion of the scapula;

implanting the subcutaneous lead in a path around the thorax with the distal end in the anterior precordial region; and

coupling the housing to the subcutaneous lead;

wherein the subcutaneous lead comprises a coil electrode having a length in the range of 6-10 centimeters;

wherein the subcutaneous lead comprises at least one sensing electrode in addition to the coil electrode; and

wherein implanting the subcutaneous lead comprises placing at least a portion thereof close to the patient'"'"'s sternum in the tissues overlying the patient'"'"'s right ventricle on the anterior thorax of the patient.

7. The method of claim 6 wherein the housing comprises a fractal surface.

8. The method of claim 6 wherein the housing comprises a wrinkled surface.

9. The method of claim 6 wherein the housing has a volume of less than 60 cubic centimeters and a weight of less than 100 grams.

10. The method of claim 9 wherein the housing contains operational circuitry including defibrillation therapy delivery circuitry configured to deliver therapeutic energy of greater than 40 Joules using the housing active surface and the coil electrode.

11. The method of claim 9 wherein the step of implanting the subcutaneous lead includes placing the subcutaneous lead such that it has numerous waves or bends.
