1. A rod and screw fixation system for stabilizing the spine comprising:
- a first and second bone anchor;
an elongated rod connected to the first and second bone anchor;
a clamp coupled to the elongated rod comprising;
a monolithic yoke having a first end and a second end, the first end comprising a first and second rod engaging portion and a channel between the first and second rod engaging portions for encompassing one of the first or second bone anchor, and the second end being spaced apart from the first end;
a first securing member extending through an upper surface of the first rod engaging portion for securing the elongated rod,a second securing member extending through an upper surface of the second rod engaging portion for securing the elongated rod, andan integrated rod extending from the second end of the yoke and parallel to the elongated rod,wherein the first rod engaging portion is configured and dimensioned to receive the first securing member for securing the elongated rod and the second rod engaging portion is configured and dimensioned to receive the second securing member for securing the rod, andwherein the integrated rod is coupled with a third bone anchor,wherein the integrated rod and the yoke are a monolithic structure,wherein the first rod engaging portion and the second rod engaging portions circumferentially contact a first and second portion of the elongated rod and wherein the first bone anchor is positioned between the first rod engaging portion and the second rod engaging portion,wherein the first and second securing members are a first and second set screws.

Abstract
A spinal implant for connecting to an elongated rod is disclosed. The spinal implant includes a yoke having a first end and a second end, the first end having a first and second rod engaging portion and a channel between the first and second rod engaging portions for encompassing a first bone anchor, and the second end being spaced apart from the first end. An extension rod is also provided which extends from the second end of the yoke parallel to the elongated rod. The first and second rod engaging portions are configured and dimensioned to receive first and second securing member for securing the elongated rod.
