1. A method for forming a lead, the method comprising:

  • placing a plurality of electrodes into a spaced-apart arrangement, each electrode of the plurality of electrodes comprising an electrode body, at least one anchoring member extending beneath the electrode body, and one or more teeth disposed along the at least one anchoring member;

    forming a lead body around the plurality of electrodes with the electrodes disposed at a distal end of the lead body, the lead body also formed around the at least one anchoring member of each of the plurality of electrodes;

    anchoring the electrode body to the lead body using the one or more teeth disposed along the at least one anchoring member; and

    electrically coupling each of the plurality of electrodes to a plurality of terminals disposed on a proximal end of the lead body.
