Abstract

Apparatus and methods for manipulating and securing tissue are described herein. In creating tissue folds within the body of a patient, a tissue manipulation assembly may generally have an elongate tubular member, an engagement member slidably disposed through the tubular member and a distal end adapted to engage tissue via a helical member, tissue stabilizing members positioned at the tubular member distal end which are adapted to stabilize tissue therebetween, and a delivery tube pivotable about the tissue stabilizer. The stabilizing members can be adapted to become angled relative to a longitudinal axis of the elongate tubular member. Moreover, one or all the articulation controls and functions can be integrated into a singular handle assembly connectable to the tissue manipulation assembly via a rigid or flexible tubular body.

Claims

1. A surgical method, comprising:

positioning an end of a tube body adjacent to a surgical site;

advancing an engager through the tube body to the surgical site;

engaging tissue at the surgical site with the engager;

moving the engager to position a section of the engaged tissue between first and second bails;

pivoting an end of a delivery tube relative to the tube body about a hinge joint pivotally attaching the end of the delivery tube to the first bail; and

securing the engaged tissue.

2. The method of claim 1 wherein end of the delivery tubular member is pivoted to an orientation substantially perpendicular to the tube body.

3. The method of claim 1 further comprising advancing a needle through the delivery tube;

passing the needle through the engaged tissue;

deploying at least one anchor from the needle; and

securing the engaged tissue by drawing the at least one anchor against the tissue.

4. The method of claim 1 further comprising pivoting the end of the delivery tube by moving the delivery tube longitudinally relative to the first bail.

5. A method for forming a tissue fold, comprising:

positioning an elongate tubular member adjacent to a region of tissue to be plicated;

advancing an engagement member slidably disposed through the tubular member and having a distal end adapted to engage the region of tissue thereto;

engaging the region of tissue with the engagement member distal end; and

proximally withdrawing engaged tissue between a first stabilizing member and a second stabilizing member;

pivoting a distal portion of a delivery tubular member relative to the elongate tubular member about a hinge joint pivotally attaching the distal portion of the delivery tubular member to the first stabilizing member; and

securing the engaged tissue.

6. The method of claim 5 wherein positioning an elongate tubular member comprises advancing a rigid tubular member to the region of tissue.

7. The method of claim 5 wherein advancing an engagement member comprises pushing a proximal end of the engagement member to advance the engagement member through the tubular member.

8. The method of claim 5 further comprising angling the first and second stabilizing members relative to the longitudinal axis of the tubular member prior to securing the engaged tissue.

9. The method of claim 5 wherein engaging the region of tissue comprises rotating a helically-wound member about its longitudinal axis such that the tissue is engaged to the helically-wound member.

10. The method of claim 9 further comprising advancing a sheath over the helically-wound member.

11. The method of claim 9 further comprising advancing an atraumatic insert within the helically-wound member.

12. The method of claim 5 further comprising pivoting the first and/or second stabilizing members relative to one another.

13. The method of claim 12 further comprising proximally advancing a linear bearing over the first stabilizing member such that the second stabilizing member is pivoted into a closed configuration relative to the first stabilizing member.

14. The method of claim 12 further comprising actuating a linkage assembly such that the first and/or second stabilizing members pivot relative to one another.

15. The method of claim 5 further comprising biasing the first and/or second stabilizing members towards one another prior to securing the engaged tissue.

16. The method of claim 5 wherein the distal portion of the delivery tubular member is pivoted perpendicularly relative to the elongate tubular member.

17. The method of claim 5 further comprising advancing a needle body through the delivery tubular member such that the needle body passes through the engaged tissue.

18. The method of claim 17 further comprising deploying at least one anchor from the needle body for securement against the tissue.

19. The method of claim 18 wherein securing the engaged tissue comprises securely drawing the at least one anchor against the tissue.

20. The method of claim 17 wherein advancing a needle body through the delivery tubular member comprises vibrating the needle body while piercing into or through the tissue.

21. The method of claim 17 wherein advancing a needle body through the delivery tubular member comprises directing the needle body into a curved or angled path relative to the delivery tubular member.

22. The method of claim 5 further comprising electrically charging the first and/or second stabilizing member prior to securing the engaged tissue.

23. The method of claim 5 further comprising electrically charging the first and/or second stabilizing member after securing the engaged tissue.
