Abstract

A spinal implant which is configured to be deployed between adjacent vertebral bodies. The implant has at least one extendable support element with a retracted configuration to facilitate deployment of the implant and an extended configuration so as to expand the implant and effectively distract the disc space, stabilize the motion segments and eliminate pathologic spine motion. Angular deformities can also be corrected, and natural curvatures restored. Preferably, the implant has a minimal dimension in its unexpanded state that is smaller than the dimensions of the neuroforamen through which it typically passes to be deployed within the intervertebral space. The implant is provided with a locking system preferably having a plurality of locking elements to lock the implant in an extended configuration. Bone engaging anchors also may be provided to ensure secure positioning.

Claims

1. A spinal implant with hydraulically actuatable, locking bone engaging surfaces, comprising:

a housing defining at least two laterally spaced circular openings;

a hydraulic piston disposed in each said opening, each said piston having an outer end movable from a contracted position within said housing to an extended position out of said housing;

a bone engaging surface disposed at each piston outer end;

a locking mechanism cooperating with said pistons to lock the pistons in the extended position; and

bone engaging anchors extendable from more than one side of the implant.

2. The spinal implant of claim 1, wherein said bone engaging surface comprises the bone engaging anchors, the bone engaging anchors having a sharp leading edge configured to penetrate bony tissue under the force of the piston.

3. The spinal implant of claim 1, wherein the bone engaging anchors comprise at least one of a conical member, a spike, a blade or a pyramid.

4. The spinal implant of claim 3, wherein the bone engaging anchors further comprise at least one barb.

5. The spinal implant of claim 1, wherein said at least two laterally spaced circular openings are formed around non-parallel central axes.

6. The spinal implant of claim 5, wherein the at least two circular openings are formed with central axes at substantially right angles to each other.

7. The spinal implant of claim 1, wherein:

the housing defines a cylinder wall within each circular opening, the cylinder wall spaced from the housing to provide an annular space there between; and

the locking mechanism is slidably disposed in said annular space.

8. The spinal implant of claim 7, wherein the locking mechanism comprises:

a first upper arcuate member with stepped engagement surfaces bearing against the piston;

a second lower arcuate member with stepped engagement surfaces opposing and engaging the stepped engagement surfaces of the first upper arcuate member; and

a biasing element biasing said first and second arcuate member engagement surfaces in a contact direction.

9. The spinal implant of claim 8, wherein the first upper arcuate member is fixed to the piston by a linking member extending over the cylinder wall.

10. The spinal implant of claim 9, wherein:

the bone engaging anchor comprises a screw member with bone cutting threads therearound;

the first upper arcuate member has screw threads disposed on an outer surface thereof; and

the circular opening in the housing is defined by an inner wall having at least in part screw threads disposed thereon and mating with the screw threads on the first upper arcuate member such that outward movement of the piston in response to hydraulic pressure in the cylinder causes rotation of the bone engaging anchor as it is extended outward.

11. The spinal implant of claim 1, wherein said hydraulic piston is a first piston and the implant further comprises an intermediate piston disposed between the first piston and the housing, the intermediate piston also defining an intermediate cylinder receiving the first piston therein.

12. The spinal implant of claim 1, wherein:

the housing defines a cylinder wall within each circular opening, the cylinder wall spaced from the housing to provide an annular space therebetween;

the intermediate pistons are received within base cylinders defined by said cylinder walls; and

at least one locking member is slidably disposed in said annular space.

13. The spinal implant of claim 12, wherein the locking mechanism comprises:

an upper arcuate member with stepped engagement surfaces secured to the first piston and configured and dimensioned to be received in said annular space;

a lower arcuate member with stepped engagement surfaces slideably disposed in said annular space;

an intermediate arcuate member with upper and lower facing stepped engagement surfaces secured to the intermediate piston and configured and dimensioned to be received in said annular space with the upper engagement surfaces opposing and engaging the upper arcuate member engagement surfaces and the lower engagement surfaces opposing and engaging the lower arcuate member engagement surfaces; and

a biasing element acting on the lower arcuate member to cause engagement of said stepped engagement surfaces.

14. The spinal implant of claim 13, wherein said bone engaging surface comprises a bone engaging anchor having a sharp leading edge configured to penetrate bony tissue under the force of the piston.
