1. A method for generating and using a portable patient file, comprising:
- receiving data relating to a patient;
storing, using a metadata model, said patient data in a medical record for the patient in an adaptive data environment, the medical record including metadata, elements and relationships among the elements, where the metadata model is represented using trees and graphs in a table driven infrastructure, the medical record stored in a plurality of tables having said table driven infrastructure within a relational database;
wherein said computer is configured to respectively accept or generate a file configured for import or export of said patient data,wherein said file is capable of transferring said patient data from said database to another database generally with preservation of semantic meanings associated with said patient data, andwherein said storing step comprises;
generating a first instance of a plurality of data objects during a first encounter;
linking a data object in said first instance to a summarization reference with a pointer;
creating an additional instance of a plurality of data objects during a later encounter, said additional instance of a plurality of data objects comprising data elements further comprising an additional instance identifier and temporal identifier; and
providing continuity for said plurality of data objects of said first instance over time, wherein said providing step comprises tracking a relationship between said data object of said first instance and a data object of said additional instance.

A method for generating and using a portable patient file may include entering data relating to a patient into a computer and storing the patient data in a medical record for the patient in a database using metadata in an adaptive data environment. The computer is configured to instantiate a file configured for import or export of the patient data, and the file is capable of transferring the patient data from the database to another database with preservation of semantic meanings associated with the patient data.

capturing data related to a patient; entering said patient data into a computer; storing, using a metadata model, said patient data in a longitudinal medical record for the patient in an adaptive data environment, the longitudinal medical record including metadata, elements and relationships among the elements, where the metadata model is represented using trees and graphs in a table driven infrastructure, the longitudinal medical record stored in a plurality of tables having said table driven infrastructure within a relational database; instantiating a file configured for import or export of at least some of said patient data; and configuring said patient data in support of a front-end service, wherein said storing step comprises; generating a first instance of a plurality of data objects during a first encounter; linking a data object in said first instance to a summarization reference with a pointer; creating an additional instance of a plurality of data objects during a later encounter, said additional instance of a plurality of data objects comprising data elements further comprising an additional instance identifier and temporal identifier; and providing continuity for said plurality of data objects of said first instance over time, wherein said providing step comprises tracking a relationship between said data object of said first instance and a data object of said additional instance. - View Dependent Claims (12, 13, 14, 15)

storing, using a metadata model, patient data having a polymorphic nature in an adaptive data environment that accommodates said polymorphic nature using metadata, the metadata model including metadata elements and relationships among the metadata elements, where the metadata model is represented using trees and graphs in a table driven infrastructure, the patient data stored in a plurality of tables having said table driven infrastructure within a relational database; updating said patient data; and populating said portable patient file with said updated patient data, wherein said populating is guided by said metadata, wherein said storing step comprises; generating a first instance of a plurality of data objects during a first encounter; linking a data object in said first instance to a summarization reference with a pointer; creating an additional instance of a plurality of data objects during a later encounter, said additional instance of a plurality of data objects comprising data elements further comprising an additional instance identifier and temporal identifier; and providing continuity for said plurality of data objects of said first instance over time, wherein said providing step comprises tracking a relationship between said data object of said first instance and a data object of said additional instance. - View Dependent Claims (17, 18, 19, 20)
