Abstract

A dermatological skin treatment device is provided. The device comprises a handpiece and a cutting tool, wherein the tool is inserted through the conduit and percutaneously inserted into a tissue disposed within a recessed area of the handpiece. The device and method cut the fibrous structures under the skin that cause cellulite at an angle substantially parallel to the surface of the skin and replace these structures with a non-cellulite forming structure by deploying a highly fibrous mesh through a single needle hole to create a highly fibrous layer directly or through wound healing processes.

Claims

1. A method of treating tissue, comprising:

engaging a first portion of tissue with a handpiece having a recessed chamber;

applying a vacuum force to move a target portion of tissue into the recessed chamber which defines a treatment region of the handpiece so that the target portion is elevated relative to surrounding tissue;

inserting a tool into the treatment region through a conduit opening of the recessed chamber so that a distal end of the tool penetrates the target portion of tissue and enters a subcutaneous region of the target portion of the tissue;

coupling the tool to a guidance track connected to the handpiece; and

moving the tool longitudinally and laterally along the guidance track so that a portion of the tool pivots about the conduit opening and so that the distal end of the tool is moved within a region of predetermined shape within the target portion of tissue defined by the longitudinal and lateral movement of the tool;

wherein the tool treats the target portion of tissue by severing septae at a first depth below the surface of the dermis.

2. The method of claim 1, wherein the distal end of the tool creates a lesion in the target portion at the first depth.

3. The method of claim 1, wherein the distal end of the tool cuts tissue in the target portion of tissue at the first depth.

4. The method of claim 3, wherein the tool cuts the septae with a blade.

5. The method of claim 4, further comprising reciprocating the blade to cut the septae.

6. The method of claim 1, further comprising adjusting the handpiece such that the distal end of the tool severs septae at a second depth below the surface of the dermis.

7. The method of claim 1, wherein the distal end of the tool moves along a first surface in the target region generally parallel to the surface of the dermis.

8. The method of claim 1, wherein the distal end of the tool emits energy to treat the target portion of tissue.

9. The method of claim 8, wherein the distal end of the tool emits RF energy or laser energy.

10. The method of claim 1, further comprising connecting the tool to a second guidance track connected to the handpiece, and moving the tool along the second guidance track to move the distal end within a second region of a second predetermined shape in the target portion of tissue.

The method of claim 1, further comprising connecting a second guidance track to the handpiece and coupling an injection device to the second guidance track, wherein the injection device is constrained to move within a predetermined area such that a needle on the injection device moves in a predetermined trajectory in the target portion of tissue.

12. A method of treating dermal tissue, comprising:

cutting septae in a predetermined shape in subcutaneous tissue by;

capturing a target portion of tissue with a handpiece having a recessed chamber;

applying a vacuum to the handpiece and elevating the target portion of tissue relative to surrounding tissue and into the recessed chamber;

coupling a guidance track to the handpiece; and

manipulating an interventional tool longitudinally and laterally along the guidance track such that a portion of the interventional tool pivots about an opening of a conduit in the recessed chamber, such that a distal end of the interventional tool cuts the septae,

wherein the interventional tool is coupled to the guidance track which constrains the interventional tool to move the distal end in response to the longitudinal and lateral motion of the interventional tool along the guidance track.

13. The method of claim 12, wherein the distal end of the tool cuts the septae in the target portion at a first depth below the surface of the dermis.

14. The method of claim 12, wherein the septae are cut with a blade.

15. The method of claim 14, further comprising reciprocating the blade to cut the septae.

16. The method of claim 13, further comprising adjusting the handpiece to cut septae in the target portion of tissue at a second depth below the surface of the dermis.

17. The method of claim 12, wherein the movement of the distal end of the tool in the subcutaneous tissue defines a surface which is substantially parallel to the surface of the dermis.

18. The method of claim 12, wherein the septae are cut by applying RF energy or laser energy.

19. A method of treating cellulite, comprising:

engaging a first portion of tissue comprising epidermis, dermis and subcutaneous tissue with a handpiece having a chamber;

applying a vacuum to draw a target portion of tissue into the chamber of the handpiece so that the target portion of tissue is elevated relative to surrounding tissue;

inserting a tool through an opening of a conduit in the chamber into the chamber such that a distal end of the tool penetrates the target portion of tissue and extends within the subcutaneous tissue;

coupling the tool to a guidance track connected to the handpiece; and

moving the tool longitudinally and laterally along the guidance track such that a portion of the tool pivots about the opening and the distal end of the tool is moved within a region of predetermined shape within the target portion of tissue that is defined by the movement of the tool along the guidance track;

wherein the tool severs septae in the target portion of tissue at a first depth below the surface of the dermis.

20. The method of claim 19, wherein the septae are severed along a first surface in the target portion of tissue, the first surface being generally parallel to the surface of the dermis.

21. The method of claim 19, further comprising adjusting the chamber so that the tool severs septae in the target portion of tissue at a second depth below the surface of the dermis.

22. The method of claim 21, wherein adjusting the chamber comprises altering the elevation of the target portion of tissue relative to the surrounding tissue.

23. The method of claim 22, wherein altering the elevation comprises reversing a reversible lid of the chamber.

24. The method of claim 19, wherein the chamber has an apposition surface and the target portion of tissue is drawn into engagement with the apposition surface.

25. The method of claim 24, wherein the apposition surface is at a predetermined height relative to the guidance track such that the distal end of the tool is positioned at the first depth.

26. The method of claim 19, wherein a guide pin is attached to the tool and engages the guidance track so that as the guide pin moves along the guidance track the tool is moved within the target portion of tissue in the predetermined shape.

27. The method of claim 19, wherein the guidance track comprises one or more channels defining a pattern such that as the tool moves through the pattern the distal end of the tool moves in a pattern within the target portion of tissue defined by the movement of the tool along the guidance track.
