an engagement bore,wherein the extension portion is disposed beneath the upper portion,wherein the head and extension portion are integrally formed,wherein the upper portion comprises a recessed region sized to receive the medallion, and wherein the engagement bore protrudes from the upper portion, and the engagement bore protrudes through the medallion when the medallion is received in the upper portion.
