Abstract

An implantable medical device senses atrial depolarizations having associated refractory periods thereafter and delivers electrical stimulation pulses during the associated refractory periods to stimulate the atrioventricular (AV) node to cause a prolonged conduction time of an atrial depolarization to a ventricular chamber. The device delivers a pacing pulse to a first ventricular chamber during the prolonged conduction time to cause a contraction and associated refractory period of the first ventricular chamber, the first ventricular chamber contraction occurring earlier than a contraction of a second ventricular chamber.

Claims

1. A method for stimulating a patient'"'"'s heart, comprising:

sensing atrial depolarizations having associated refractory periods thereafter;

delivering electrical stimulation pulses during the associated refractory periods to stimulate the atrioventricular (AV) node to cause a prolonged conduction time of the sensed atrial depolarizations to a ventricular chamber; and

delivering a pacing pulse to a first ventricular chamber during the prolonged conduction time to cause a contraction and associated refractory period of the first ventricular chamber, the first ventricular chamber contraction occurring prior to a contraction of a second ventricular chamber.

2. The method of claim 1, further comprising setting a stimulation time window in response to sensing an atrial depolarization and delivering the electrical stimulation pulses during the time window.

3. The method of claim 1 further comprising measuring a time interval corresponding to the conduction of the sensed atrial depolarizations to the ventricular chamber and adjusting a parameter controlling the electrical stimulation pulses to cause an increase in the time interval.

4. The method of claim 3 wherein the parameter comprises one of an electrical pulse amplitude, a pulse frequency, and a number of electrical pulses.

5. The method of claim 1 further comprising:

delivering atrial pacing pulses;

setting a stimulation time window in response to an atrial pacing pulse; and

delivering the electrical stimulation pulses during the stimulation time window.

6. The method of claim 1 wherein the first ventricular chamber pacing pulse is delivered to maximize a time interval between a depolarization of the first ventricular chamber and a depolarization of the second ventricular chamber.

7. The method of claim 1 further comprising delivering a pacing pulse in a second ventricular chamber after delivering the first ventricular chamber pacing pulse.

8. The method of claim 7 further comprising setting a pace interval in response to the pacing pulse delivered to the first ventricular chamber wherein the second ventricular chamber pacing pulse is delivered at the end of the pace interval.

The method of claim 1 further comprising setting a ventricular pace interval wherein the pacing pulse is delivered to the first ventricular chamber at the end of the ventricular pace interval to cause the refractory period of the first ventricular chamber to extend to a time later than the prolonged conduction time.

10. An implantable medical device for stimulating a patient'"'"'s heart, comprising:

at least one pair of electrodes for sensing atrial depolarizations having associated refractory periods thereafter;

sensing circuitry coupled to the sensing electrodes;

a plurality of therapy delivery electrodes for delivering AV node stimulation pulses and for delivering ventricular pacing pulses;

therapy delivery circuitry coupled to the therapy delivery electrodes; and

a control processor coupled to the sensing circuitry and the therapy delivery circuitry configured to control a cardiac stimulation therapy comprising delivering electrical stimulation pulses during the associated refractory periods to stimulate the atrioventricular (AV) node to cause a prolonged conduction time of the sensed atrial depolarizations to a ventricular chamber and delivering a pacing pulse to a first ventricular chamber during the prolonged conduction time to cause a contraction and associated refractory period of the first ventricular chamber, the first ventricular chamber contraction occurring prior to a contraction of a second ventricular chamber.

11. The device of claim 10 wherein the control processor is configured to set a stimulation time window in response to a sensed atrial depolarization, the electrical stimulation pulses delivered during the time window.

The device of claim 10 further wherein the control processor is further configured to measure a time interval corresponding to the conduction of the sensed atrial depolarizations to the ventricular chamber and adjust a parameter controlling the electrical stimulation pulses to cause an increase in the time interval.

13. The device of claim 12 wherein the parameter comprises one of an electrical pulse amplitude, a pulse frequency, and a number of electrical pulses.

14. The device of claim 10 wherein the control processor is further configured to:

control the therapy delivery circuitry to deliver atrial pacing pulses;

set a stimulation time window in response to an atrial pacing pulse; and

deliver the electrical stimulation pulses during the stimulation time window.

The device of claim 10 wherein the control processor is configured to control the therapy delivery circuitry to deliver the first ventricular chamber pacing pulse to maximize a time interval between a depolarization of the first ventricular chamber and a depolarization of the second ventricular chamber.

16. The device of claim 10 wherein the control processor is further configured to control the therapy delivery circuitry to deliver a pacing pulse in a second ventricular chamber after delivering the first ventricular chamber pacing pulse.

17. The device of claim 16 wherein the control processor is further configured to set a pace interval in response to the pacing pulse delivered to the first ventricular chamber wherein the second ventricular chamber pacing pulse is delivered at the end of the pace interval.

The device of claim 10 wherein the control processor is further configured to set a ventricular pace interval to control the therapy delivery circuitry to deliver the pacing pulse to the first ventricular chamber at the end of the ventricular pace interval and cause the refractory period of the first ventricular chamber to extend to a time later than the prolonged conduction time.

19. The device of claim 10, wherein the control processor is configured to:

determine a prolonged PR interval caused by the electrical stimulation pulses delivered to stimulate the AV node;

set a ventricular pacing interval ending prior to the end of the prolonged PR interval; and

deliver the pacing pulse to the first ventricular chamber at the end of the ventricular pacing interval.

20. A non-transitory computer-readable medium storing instructions which cause an implantable medical device to perform a method comprising:

sensing atrial depolarizations having associated refractory periods thereafter;

delivering electrical stimulation pulses during the associated refractory periods to stimulate the atrioventricular (AV) node to cause a prolonged conduction time of the sensed atrial depolarizations to a ventricular chamber; and

delivering a pacing pulse to a first ventricular chamber during the prolonged conduction time to cause a contraction and associated refractory period of the first ventricular chamber, the first ventricular chamber contraction occurring earlier than a contraction of a second ventricular chamber.
