Abstract

A surgical stapler, or fastening instrument, may generally comprise a layer, such as a tissue thickness compensator, for example, releasably attached to a fastener cartridge and/or anvil by a flowable attachment portion. The flowable attachment portion may be indefinitely flowable. The flowable attachment portion may be flowable from the time that layer is installed to the fastener cartridge to the time in which the layer is implanted to patient tissue. The flowable attachment portion may comprise a pressure sensitive adhesive. The flowable attachment portion may comprise an adhesive laminate comprising a base layer comprising the tissue thickness compensator and an adhesive layer on at least a portion of a surface of the base layer comprising the pressure sensitive adhesive. Articles of manufacture comprising flowable attachment portion and methods of making and using the flowable attachment portion are also described.

Claims

1. A surgical stapler, comprising:

a staple cartridge, comprising;

a cartridge body comprising a plurality of staple cavities; and

a plurality of staples positioned within said staple cavities;

an anvil configured for a facing relationship with said cartridge body, said anvil comprising at least one staple forming pocket; and

a tissue thickness compensator releasably attached to at least one of said cartridge body and said anvil by a discrete ridge extending from said tissue thickness compensator, wherein said discrete ridge is comprised of a flowable attachment portion, and wherein said flowable attachment portion comprises a pressure sensitive adhesive.

2. The surgical stapler of claim 1, wherein said flowable attachment portion is flowable at body temperature (37°

C.) and/or room temperature (25°

C.).

3. The surgical stapler of claim 1, wherein said flowable attachment portion comprises a flowable polymeric composition that is solid up to 55°

C.

4. The surgical stapler of claim 1, wherein said flowable attachment portion comprises a flowable polymeric composition having an inherent viscosity from 0.6 to 4.0 dL/g in a 0.1 g/dL hexafluoroisopropanol solution at 25°

C.

5. The surgical stapler of claim 1, wherein said flowable attachment portion comprises a first profile and a second profile, wherein the second profile is complementary to a void in at least one of said cartridge body and said anvil.

6. The surgical stapler of claim 1, wherein said flowable attachment portion comprises a first position and a second position, wherein said flowable attachment portion contacts an inner surface of at least one of said cartridge body and said anvil when in the second position.

7. The surgical stapler of claim 1, wherein said flowable attachment portion comprises an adhesive laminate comprising a base layer comprising said tissue thickness compensator and an adhesive layer on at least a portion of a surface of the base layer comprising said flowable attachment portion.

8. The surgical stapler of claim 1, wherein the pressure sensitive adhesive and said tissue thickness compensator differ in at least one characteristic selected from composition, modulus, elongation, inherent viscosity, crystallinity, and bioabsorption.

9. The surgical stapler of claim 1, wherein the pressure sensitive adhesive comprises polylactide (PLA), polyglycolide (PGA), polycaprolactone (PCL), and combinations thereof.

10. The surgical stapler of claim 1, wherein the pressure sensitive adhesive comprises a random copolymer comprising:

about 45 to about 55 mole percent ε

-caprolactone, and

balance glycolide.

11. The surgical stapler of claim 1, wherein a concentration of glycolide in the pressure sensitive adhesive is at least 10 mole percent less than a concentration of glycolide in said tissue thickness compensator.

12. The surgical stapler of claim 1, further comprising a slot configured to slidably receive a cutting member therein, and wherein at least a portion of said flowable attachment portion is within said slot.

13. The surgical stapler of claim 1, wherein at least a portion of said flowable attachment portion is within said staple forming pocket.

14. The surgical stapler of claim 1, wherein said flowable attachment portion releasably adheres said tissue thickness compensator to one of said cartridge body and said anvil when said anvil is in a closed position.

15. The surgical stapler of claim 1, wherein said tissue thickness compensator releases from one of said cartridge body and said anvil when said flowable attachment portion is ruptured by a staple and/or a cutting member.

16. The surgical stapler of claim 1 comprising a retainer assembly comprising a protrusion to apply said tissue thickness compensator to a surgical stapler, wherein said flowable attachment portion is provided on at least a portion of said protrusion.

17. A stapling assembly for use with a stapler, said stapling assembly comprising:

an anvil comprising

an array of forming pockets, and

a slot configured to receive a cutting member; and

a tissue thickness compensator releasably attached to said anvil by at least one ridge extending from said tissue thickness compensator, wherein said ridge is comprised of a strip of a viscous flowable polymeric composition aligned with one of said slot and at least a portion of said array of forming pockets, and wherein said viscous flowable polymeric composition comprises a pressure sensitive adhesive.

The stapling assembly of claim 17, wherein said flowable polymeric composition is spaced from the other of said slot and at least a portion of said array of forming pockets in the absence of a threshold level of pressure applied to the stapling assembly, and said flowable polymeric composition comes into securing engagement with each of said slot and at least a portion of said array of forming pockets when a threshold level of pressure is applied to said stapling assembly.

The stapling assembly of claim 17, wherein said pressure sensitive adhesive is spaced from the other of said slot and at least a portion of said array of forming pockets in the absence of a threshold level of pressure applied to the stapling assembly, and said pressure sensitive adhesive comes into securing engagement with said one of said slot and at least a portion of said array of forming pockets and not the other of said slot and at least a portion of said array of forming pockets when a threshold level of pressure is applied to said stapling assembly.

20. A surgical stapler, comprising:

a staple cartridge, comprising;

a cartridge body comprising a plurality of staple cavities; and

a plurality of staples positioned within said staple cavities;

an anvil configured for a facing relationship with said cartridge body, said anvil comprising at least one staple forming pocket; and

a tissue thickness compensator comprising a discrete ridge extending from said tissue thickness compensator, wherein said discrete ridge is comprised of a flowable attachment portion, wherein said tissue thickness compensator is releasably attached to at least one of said cartridge body and said anvil by said flowable attachment portion, wherein said flowable attachment portion is configured to shift from a first configuration to a second configuration when a threshold level of pressure is applied thereto, wherein said flowable attachment portion remains in said second configuration when the threshold level of pressure is removed therefrom, and wherein said flowable attachment portion conforms to at least one void in at least one of said cartridge body and said anvil when in said second configuration.
