Abstract

A glenoid guide has an upper surface and a lower surface, wherein the lower surface is a patient-specific surface configured as a negative surface of a glenoid face based on a three-dimensional image of a shoulder joint of a patient reconstructed preoperatively from image scans of the shoulder joint of the patient. The glenoid guide includes an anatomic tubular drill guide extending from the upper surface along an anatomic alignment axis configured preoperatively with a patient-specific orientation and insertion location for guiding a glenoid implant into the glenoid face for anatomic shoulder arthroplasty. The glenoid guide includes a reverse tubular drill guide extending from the upper surface along a reverse alignment axis configured preoperatively with a patient-specific orientation and insertion location for guiding a glenoid baseplate into the glenoid face for reverse shoulder arthroplasty.

Claims

1. An orthopedic device for a shoulder joint of a patient comprising:

a glenoid guide having an upper surface and a lower surface, wherein the lower surface is a patient-specific surface configured as a negative surface of a glenoid face based on a three-dimensional image of a shoulder joint of a patient reconstructed preoperatively from image scans of the shoulder joint of the patient, the glenoid guide including an anatomic tubular drill guide extending from the upper surface along an anatomic alignment axis configured preoperatively with a patient-specific orientation for guiding a glenoid implant into the glenoid face for anatomic shoulder arthroplasty, the glenoid guide including a reverse tubular drill guide extending from the upper surface along a reverse alignment axis configured preoperatively with a patient-specific orientation for guiding a glenoid baseplate into the glenoid face for reverse shoulder arthroplasty, wherein the reverse alignment axis is tilted inferiorly relative to the anatomic alignment axis.

2. The orthopedic device of claim 1, wherein the glenoid guide includes a patient-specific peripheral surface forming a lip configured as a negative of a glenoid rim of the patient.

3. The orthopedic device of claim 2, wherein the lip includes a through slot configured for viewing and marking the glenoid face.

4. The orthopedic device of claim 1, wherein the glenoid guide includes a patient-specific peripheral surface portion configured as a negative of a coracoid surface of the patient.

5. The orthopedic device of claim 1, further comprising a drill guide configured to engage a baseplate implant for reverse arthroplasty, the drill guide including a central tubular drilling post having a patient-specific drilling axis oriented along the reverse alignment axis.

6. The orthopedic device of claim 5, wherein the central tubular drilling post is configured to have a patient-specific height for controlling drilling depth therethrough.

7. The orthopedic device of claim 6, wherein the drill guide includes a plurality of peripheral tubular drilling posts having patient-specific orientation and height.

8. The orthopedic device of claim 6, further comprising a drill having a stop configured to engage the central drilling post for controlling drilling depth there through.

9. The orthopedic device of claim 1, wherein the glenoid guide includes an alignment notch for marking the glenoid face.

10. The orthopedic device of claim 9, further comprising a drill guide configured to engage a baseplate implant for reverse arthroplasty, the drill guide including a central tubular drilling post having a patient-specific drilling axis oriented along the reverse alignment axis, the drill guide having a projection configured to be aligned with a marking made using the alignment notch.

11. An orthopedic device for a shoulder joint of a patient comprising:

a glenoid guide having an upper surface and a lower surface, wherein the lower surface is a patient-specific surface configured as a negative surface of a glenoid face based on a three-dimensional image of a shoulder joint of a patient reconstructed preoperatively from image scans of the shoulder joint of the patient, the glenoid guide including a first elongated tubular drill guide extending from the upper surface along a first alignment axis configured preoperatively with a patient-specific orientation and insertion location for guiding a first glenoid implant into the glenoid face, and the glenoid guide including a second elongated tubular drill guide extending from the upper surface along a second alignment axis configured preoperatively with a patient-specific orientation and location for guiding a second glenoid implant into the glenoid face,

wherein the second alignment axis is tilted inferiorly relative to the first alignment axis; and

wherein each of first and second elongated tubular drill guides includes corresponding elongated viewing holes, and wherein the first elongated tubular guide and first alignment axis are configured for anatomic shoulder arthroplasty, and wherein the second elongated tubular drill guide and the second alignment axis are configured for reverse shoulder arthroplasty.

12. The orthopedic device of claim 11, wherein the first and second elongated tubular drill guides are connected with a web.

13. The orthopedic device of claim 11, wherein the glenoid guide includes a patient-specific peripheral surface forming a lip configured as a negative of a glenoid rim of the patient.

14. The orthopedic device of claim 13, wherein the lip includes a through slot configured for viewing and marking the glenoid face.

15. The orthopedic device of claim 11, wherein the glenoid guide includes a patient-specific peripheral surface portion configured as a negative of a coracoid surface of the patient.

16. The orthopedic device of claim 11, wherein the glenoid guide includes an alignment notch for marking the glenoid face.

17. The orthopedic device of claim 11, further comprising a drill guide configured to engage a baseplate implant for reverse arthroplasty, the drill guide including a central tubular drilling post having a patient-specific drilling axis oriented along the second alignment axis.

18. The orthopedic device of claim 17, wherein the central tubular drilling post is configured to have a patient-specific height for controlling drilling depth therethrough.

19. The orthopedic device of claim 18, wherein the drill guide includes a plurality of peripheral tubular drilling posts having patient-specific orientation and height.

20. The orthopedic device of claim 18, further comprising a drill having a stop configured to engage the central drilling post for controlling drilling depth there through.

21. The orthopedic device of claim 16, further comprising a drill guide configured to engage a baseplate implant for reverse arthroplasty, the drill guide including a central tubular drilling post having a patient-specific drilling axis oriented along the second alignment axis, the drill guide having a projection configured to be aligned with a marking made using the alignment notch.

22. The orthopedic device of claim 11, wherein the second alignment axis has a different insertion location relative to the first alignment axis.

23. An orthopedic system comprising:

a glenoid guide having an upper surface and a lower surface, wherein the lower surface is a patient-specific surface configured as a negative surface of a glenoid face based on a three-dimensional image of a shoulder joint of a patient reconstructed preoperatively from image scans of the shoulder joint of the patient, the glenoid guide including an anatomic tubular drill guide extending from the upper surface along an anatomic alignment axis configured preoperatively with a patient-specific orientation for guiding a glenoid implant into the glenoid face for anatomic shoulder arthroplasty, the glenoid guide including a reverse tubular drill guide extending from the upper surface along a reverse alignment axis configured preoperatively with a patient-specific orientation for guiding a glenoid baseplate into the glenoid face for reverse shoulder arthroplasty, wherein the reverse alignment axis is tilted inferiorly relative to the anatomic alignment axis;

a reverse shoulder arthroplasty implant including a baseplate;

an anatomic shoulder arthroplasty implant; and

a secondary drill guide configured to engage the baseplate implant for reverse arthroplasty, the secondary drill guide including a central tubular drilling post having a patient-specific drilling axis oriented along the reverse alignment axis.

24. The orthopedic system of claim 23, wherein the central tubular drilling post of the secondary drill guide is configured to have a patient-specific height for controlling drilling depth there through.

25. The orthopedic system of claim 24, further comprising a drill having a stop configured to engage the central drilling post for controlling drilling depth there through.

26. The orthopedic system of claim 23, wherein the glenoid guide includes a peripheral notch for marking the glenoid and the secondary drill guide includes a projection configured to be aligned with a marking made through the peripheral notch on the glenoid.
