1. A computer-implemented method for producing electronic health records from graphical inputs by computer users, the method comprising:

  • receiving, on a graphical user interface of a computer system, a user identification of a diagnosis for a patient, the user identification produced by user selection of a first selectable item from a plurality of selectable items that are each indicative of diagnoses and that are displayed in a first zone on the graphical user interface;

    identifying, with the computer system and in response to receiving the user selection of the diagnosis, a plurality of parameters that each correspond to and characterize the diagnosis;

    displaying in a second zone that is spatially separated from the first zone on the graphical user interface and in response to identifying the plurality of parameters, a plurality of selectable values for each of multiple ones of the plurality of parameters, wherein the first and second zones have edges that are adjacent to each other that do not overlap each other;

    receiving, by the computer system and from the user, sequential user selections that represent multiple particular ones of the values from the plurality of parameters that each correspond to and characterize the diagnosis; and

    generating an electronic health record representation that represents a particularized version of the identified diagnosis with text for values selected by the user from the parameters that correspond to and characterize the diagnosis, wherein the representation is generated by applying a syntax to the selected diagnosis and values for the selected diagnosis along with text not selected by the user, to generate a natural language description from multiple sub-sentence clauses after the user identification and the sequential user selections are received,wherein, once the user identification of the diagnosis for the patient and the sequential user selections of the values for the one or more parameters that correspond to and characterize the diagnosis have been selected, the first selectable item that is representative of the diagnosis is displayed along with the selected values of parameters that correspond to and characterize the diagnosis as a visually-identified group, for review by the user, andwherein the user can move from any of the plurality of selectable items that are representative of a diagnosis in the first zone to any of the plurality of selectable values of parameters that correspond to and characterize the diagnosis in the second zone without moving over any other item in the first zone or any other value in the second zone.
