Abstract

A cardiac rhythm management system selects one of multiple electrodes associated with a particular heart chamber based on a relative timing between detection of a depolarization fiducial point at the multiple electrodes, or based on a delay between detection of a depolarization fiducial point at the multiple electrodes and detection of a reference depolarization fiducial point at another electrode associated with the same or a different heart chamber. Subsequent contraction-evoking stimulation therapy is delivered from the selected electrode.

Claims

1. A system, comprising:

a sensing circuit including a sense amplifier to sense, at two or more candidate stimulation sites associated with a first heart chamber, physiologic signals indicative of cardiac activation;

an activation timer circuit coupled to the sensing circuit, the activation timer circuit configured to detect from the sensed physiologic signals activation timings at each of the two or more candidate stimulation sites;

a stimulation site selector circuit coupled to the activation timer circuit, the stimulation site selector circuit configured to identify, from the two or more candidate stimulation sites, a late-responsive stimulation site having the latest activation timing; and

an output circuit configured to generate an indication for delivering electrostimulation, among the two or more candidate stimulation sites, only at the identified late-responsive stimulation site having the latest activation timing.

2. The system of claim 1, wherein the activation timer circuit is configured to detect a QRS complex, and to detect the activation timings corresponding to the two or more candidate stimulation sites each relative to the detected QRS complex.

3. The system of claim 1, wherein the activation timer circuit is configured to detect a P wave timing, and to detect the activation timings corresponding to the two or more candidate stimulation sites each relative to the detected P wave timing.

The system of claim 1, wherein the activation timer circuit is configured to detect a point of minimum slope on the physiologic signal, and to detect the activation timings corresponding to the two or more candidate stimulation sites each relative to the detected point of minimum slope on the physiologic signal.

The system of claim 1, wherein the activation timer circuit is configured to detect a cardiac depolarization at a second heart chamber, and to detect the activation timings corresponding to the two or more candidate stimulation sites each relative to the detected cardiac depolarization at the second heart chamber.

6. The system of claim 1, wherein:

the activation timer circuit is configured to provide a reference time, and to measure time intervals from the reference time to the activation timings corresponding to the two or more candidate stimulation sites; and

the stimulation site selector circuit is configured to identify the late-responsive stimulation site having the latest activation timing as a site having a corresponding measured time interval no shorter than measured time intervals corresponding to other of the two or more candidate stimulation sites.

The system of claim 6, wherein the stimulation site selector circuit is configured to identify the late-responsive stimulation site having the latest activation timing as a site having a corresponding measured time interval exceeding, by a specified margin, the measured time intervals corresponding to other of the two or more candidate stimulation sites.

8. The system of claim 1, wherein the activation timer circuit is configured to detect the activation timings corresponding to the two or more candidate stimulation sites within a same cardiac cycle.

9. The system of claim 1, wherein the two or more candidate stimulation sites are associated with the first heart chamber including one of a left ventricle or a right ventricle.

The system of claim 1, wherein the sensing circuit is configured to sense the physiologic signals including cardiac electrical signals sensed at two or more left-ventricular (LV) candidate sites of the heart, and wherein the activation timer circuit is configured to determine the activation timings including depolarization timings at the two or more LV candidate sites.

11. The system of claim 1, further comprising two or more electrodes positionable at the two or more candidate stimulation sites for sensing the physiologic signals from, or delivering the electrostimulation at, the two or more candidate stimulation sites.

The system of claim 1, wherein the output circuit is configured to generate the indication for delivering the electrostimulation using a subset of the two or more electrodes including an electrode positioned at the identified late-responsive stimulation site having the latest activation timing.

13. The system of claim 1, wherein the stimulation site selector circuit is further configured to identify the late-responsive stimulation site having the latest activation timing as a site spatially closer in distance to a heart apex than other of the two or more candidate stimulation sites.

14. The system of claim 1, further comprising a therapy circuit configured to deliver the electrostimulation at the identified late-responsive stimulation site having the latest activation timing.

15. A method, comprising:

sensing, at two or more candidate stimulation sites associated with a first heart chamber, physiologic signals indicative of cardiac activation;

detecting from the sensed physiologic signals activation timings at each of the two or more candidate stimulation sites;

identifying, from the two or more candidate stimulation sites, a late-responsive stimulation site having the latest activation timing; and

generating an indication for delivering electrostimulation, among the two or more candidate stimulation sites, only at the identified late-responsive stimulation site having the latest activation timing.

16. The method of claim 15, further comprising delivering the electrostimulation using at least one electrode positioned at the identified late-responsive stimulation site having the latest activation timing.

The method of claim 15, further comprising detecting a reference time including one or more of a QRS complex, a P wave timing, a point of minimum slope on the physiologic signal, or timing of a cardiac depolarization at a second heart chamber, and measuring time intervals from the reference time to each activation timing of each candidate stimulation site, wherein the identified late-responsive stimulation site having the latest activation timing has a corresponding measured time interval no shorter than measured time intervals corresponding to other of the two or more candidate stimulation sites.

18. The method of claim 15, wherein the activation timings corresponding to the two or more candidate stimulation sites are detected within a same cardiac cycle.

19. The method of claim 15, wherein:

sensing the physiologic signals includes sensing cardiac electrical signals at two or more left-ventricular (LV) candidate sites of the heart; and

detecting the activation timings including includes detecting depolarization timings at the two or more LV candidate sites.

20. The method of claim 15, further comprising determining spatial distance of the two or more candidate stimulation sites to a heart apex, wherein the identified late-responsive stimulation site having the latest activation timing is spatially closer in distance to the heart apex than other of the two or more candidate stimulation sites.
