Abstract

Methods, systems and devices for monitoring patient orientation, direction, altitude and location to determine the need for turns as well as to detect impending or actual bed exits or falls. A patient-worn sensor communicates with at least one host server through a mesh network of relay antennae to provide data representative of various patient characteristics. The sensor comprises one or more of an accelerometer, a magnetometer and an altimeter, along with optional other sensors. Various user interfaces are provided for managing patient care using the information developed by algorithms processed at the server based on data received from the patient sensors.

Claims

1. A system for patient monitoring comprising:

a plurality of relay antennae for receiving signals from one or more patient-worn sensors and from other relay antennae, each patient-worn sensor comprising an accelerometer and configured to collect sensor data, and

a processor in communication with the plurality of relay antennae and configured to;

receive sensor data collected by a particular patient-worn sensor,

calculate an angular orientation of the patient relative to the bed based on a physical orientation of the bed and the sensor data collected by the patient-worn sensor, and

identify a bed exit condition based at least on the determined angular orientation of the patient relative to the bed, wherein identifying the bed exit condition comprises identifying an occurrence or anticipated occurrence of a bed exit by the patient.

2. The system of claim 1, wherein the particular patient-worn sensor comprises the accelerometer and at least one of a magnetometer or an altimeter.

3. The system of claim 1, wherein the particular patient-worn sensor comprises at least two of an accelerometer, a magnetometer, and an altimeter.

4. The system of claim 1, wherein the processor is configured to:

determine a compass orientation of the bed, and

calculate the angular orientation of the patient relative to the bed based on the determined compass orientation of the bed and the sensor data collected by the particular patient-worn sensor.

5. The system of claim 1, wherein identifying the bed exit condition comprises determining an anticipated imminent bed exit by the patient.

6. The system of claim 1, wherein identifying the bed exit condition comprises determining a probability of a bed exit by the patient.

7. The system of claim 1, wherein the particular patient-worn sensor is affixed directly to the patient or secured to or embedded in an article worn by the patient.

8. A method for detecting or anticipating a bed exit of a patient, the method comprising:

determining or accessing a physical orientation of a bed,

collecting sensor data regarding the patient by a patient-worn sensor device secured to the patient, the sensor data comprising at least acceleration data collected by an accelerometer of the patient-worn sensor device,

receiving, at a processor, the sensor data collected by the patient-worn sensor,

calculating, by the processor, an angular orientation of the patient relative to the bed based on the determined physical orientation of the bed and the sensor data collected by the patient-worn sensor, and

identifying a bed exit condition based at least on the determined angular orientation of the patient relative to the bed, wherein identifying the bed exit condition comprises identifying an occurrence or anticipated occurrence of a bed exit by the patient.

9. The method of claim 8, wherein the processor is provided in the patient-worn sensor device.

10. The method of claim 8, wherein the processor is distinct from the patient-worn sensor device, and wherein receiving, at the processor, sensor data collected by the patient-worn sensor device comprises receiving the sensor data at the processor via a wireless transmission from the patient-worn sensor device.

11. The method of claim 8, wherein determining or accessing a physical orientation of a bed comprises determining an angular orientation of a bed using an accelerometer, magnetometer, or other sensor secured to the bed.

12. The method of claim 8, wherein determining or accessing a physical orientation of a bed comprises monitoring an orientation of the patient over time based on sensor data collected by the patient-worn sensor device, and determining a physical orientation of the bed based on the monitored orientation of the patient over time.

13. The method of claim 8, wherein:

determining a physical orientation of a bed comprises determining a compass orientation of the bed, and

the angular orientation of the patient relative to the bed is calculated based on the determined compass orientation of the bed and the sensor data collected by the patient-worn sensor device.

14. The method of claim 8, wherein identifying the bed exit condition comprises determining an anticipated imminent bed exit by the patient.

15. The method of claim 8, wherein identifying the bed exit condition comprises determining a probability of a bed exit by the patient.

16. The method of claim 8, wherein the patient-worn sensor device is affixed directly to the patient or secured to or embedded in an article worn by the patient.

17. The method of claim 8, wherein the acceleration data collected by the accelerometer of the patient-worn sensor device comprises at least one of static acceleration data or dynamic acceleration data.

18. A method for identifying a fall condition of a patient, the method comprising:

collecting patient altitude data by an altimeter of a patient-worn sensor device secured to the patient,

determining an altitude of one or more relay antennae,

determining a reference altitude based on the determined altitude of the one or more relay antennae,

receiving, at a processor, the patient altitude data collected by the altimeter of the patient-worn sensor device, the patient altitude data indicating changes in altitude of the patient,

determining, by the processor, a relative altitude of the patient based on (a) the received patient altitude data and (b) the reference altitude determined based on the determined altitude of the one or more relay antennae, and

identifying, by the processor, a patient fall condition based on the determined relative altitude of the patient.

19. The method of claim 18, wherein identifying a patient fall condition based on results of the comparison comprises determining a probability of a patient fall.

20. The method of claim 18, wherein the patient-worn sensor device is affixed directly to the patient or secured to or embedded in an article worn by the patient.

21. The method of claim 18, wherein determining an altitude of each relay antenna comprises determining an altitude of the respective relay antenna relative to a floor.

22. The method of claim 18, wherein the processor is provided in the patient-worn sensor device.

23. The method of claim 18, wherein:

the patient-worn sensor device is configured to wirelessly transmit patient altitude data regarding the patient; and

the processor is distinct from the patient-worn sensor device, and

receiving patient altitude data at the processor comprises receiving the patient altitude data from the patient-worn sensor device wirelessly via at least one of the relay antennae.

24. The method of claim 18, wherein the steps of determining a relative altitude of the patient and identifying a patient fall condition based on the determined relative altitude of the patient comprise:

comparing, by the processor, the altitude of the patient, as indicated by the patient altitude data, to the reference altitude determined based on the determined altitude of the one or more relay antennae, to determine the relative altitude of the patient, and

identifying a patient fall condition based on the determined relative altitude of the patient.

25. The method of claim 18, wherein:

the patient-worn sensor device further comprises at least one accelerometer configured to collect patient acceleration data indicating accelerations of the patient; and

the method further comprises;

receiving, at the processor, patient acceleration data collected by the at least one accelerometer,

identifying the patient fall condition based at least on (a) the determined relative altitude of the patient, as determined based on the received patient altitude data and the reference altitude based on the altitude of the one or more relay antennae, and (b) the received patient acceleration data.

26. The method of claim 25, wherein identifying the patient fall condition comprises:

comparing, by the processor, the relative altitude of the patient with at least one predefined altitude threshold;

comparing, by the processor, the patient acceleration data with at least one predefined acceleration threshold; and

identifying the patient fall condition based at least on (a) the comparison of the relative altitude of the patient with the at least one predefined altitude threshold and (b) the comparison of the patient acceleration data with the at least one predefined acceleration threshold.

27. The method of claim 18, wherein determining the relative altitude of the patient comprises determining an altitude of the patient relative to a floor.

28. A method of identifying a fall condition of a patient using a patient-worn sensor device secured to a patient, the method comprising:

collecting patient altitude data by an altimeter of the patient-worn sensor device;

collecting patient acceleration data by an accelerometer of the patient-worn sensor device;

determining, by a processor, a relative altitude of the patient based on (a) the collected patient altitude data and (b) a reference altitude; and

identifying, by the processor, the fall condition based at least on (a) the determined relative altitude of the patient, as determined based on the collected patient altitude data and the reference altitude, and (b) the collected patient acceleration data.

29. The method of claim 28, comprising:

receiving, at the processor, reference altitude data from at least one reference altimeter not secured to the patient;

determining, by the processor, the reference altitude based on the reference altitude data received from the at least one reference altimeter not secured to the patient.

30. The method of claim 29, wherein each reference altimeter not secured to the patient is provided in a respective relay antennae configured to wirelessly communicate with the patient-worn sensor device.

31. The method of claim 28, wherein identifying the fall condition comprises:

comparing, by the processor, the relative altitude of the patient with at least one predefined altitude threshold;

comparing, by the processor, the patient acceleration data with at least one predefined acceleration threshold; and

identifying the fall condition based at least on (a) the comparison of the relative altitude of the patient with the at least one predefined altitude threshold and (b) the comparison of the patient acceleration data with the at least one predefined acceleration threshold.

32. A method for detecting or anticipating a bed exit of a patient, the method comprising:

receiving, by a processor, signals from a magnetometer of the patient-worn sensor device;

determining, by the processor, a magnetic orientation of the patient based on the collected signals from the magnetometer by a process including;

determining or accessing, by the processor, an orientation of a bed; and

calculating, by the processor, a magnetic orientation of the patient relative to the bed based on the collected signals from the magnetometer;

receiving, by the processor, patient acceleration data from an accelerometer of the patient-worn sensor device; and

identifying, by the processor, a bed exit condition based at least on the determined magnetic orientation of the patient and the received patient acceleration data, wherein identifying the bed exit condition comprises identifying an occurrence or anticipated occurrence of a bed exit by the patient.

33. The method of claim 32, wherein identifying the bed exit condition comprises:

comparing, by the processor, the determined magnetic orientation of the patient with at least one predefined patient orientation threshold;

comparing, by the processor, the patient acceleration data with at least one predefined acceleration threshold; and

identifying the bed exit condition based at least on (a) the comparison of the magnetic orientation of the patient with the at least one predefined patient orientation threshold and (b) the comparison of the patient acceleration data with the at least one predefined acceleration threshold.

34. The method of claim 32, wherein identifying the bed exit condition comprises determining an anticipated imminent bed exit by the patient.

35. The method of claim 32, wherein identifying the bed exit condition comprises determining a probability of a bed exit by the patient.

36. The method of claim 32, wherein determining or accessing an orientation of a bed comprises determining an orientation of a bed using an accelerometer, magnetometer, or other sensor secured to the bed.

37. The method of claim 32, wherein determining or accessing an orientation of a bed comprises monitoring an orientation of the patient over time based on sensor data collected by the patient-worn sensor device, and determining an orientation of the bed based on the monitored orientation of the patient over time.

38. The method of claim 32, wherein the sensor data collected by the patient-worn sensor device comprises at least one of (a) signals from the magnetometer of the patient-worn sensor device or (b) patient acceleration data from an accelerometer of the patient-worn sensor device.

39. The method of claim 32, wherein:

determining an orientation of a bed comprises determining a compass orientation of the bed, and

the magnetic orientation of the patient relative to the bed is calculated based on the determined compass orientation of the bed and sensor data collected by the patient-worn sensor device.

40. A system for detecting or anticipating a bed exit of a patient, the system comprising:

a processor; and

computer instructions stored in non-transitory computer readable media and executable by the processor to;

determine or access data indicating a physical orientation of a bed,

receive sensor data regarding the patient from one or more sensors provided in a patient-worn sensor device secured to the patient, the sensor data comprising at least acceleration data collected by an accelerometer of the patient-worn sensor device,

calculate an angular orientation of the patient relative to the bed based on the determined physical orientation of the bed and the sensor data collected by the patient-worn sensor, and

identify a bed exit condition based at least on the determined angular orientation of the patient relative to the bed, wherein identifying the bed exit condition comprises identifying an occurrence or anticipated occurrence of a bed exit by the patient.

41. The system of claim 40, wherein:

the system includes the patient-worn sensor device, and

the processor and non-transitory computer readable media are provided in the patient-worn sensor device.

42. The system of claim 40, wherein:

the system includes the patient-worn sensor device, and a separate computing device, and

the processor and non-transitory computer readable media are provided in the separate computing device.

43. The system of claim 40, wherein the processor and non-transitory computer readable media are provided in a computing device distinct from the patient-worn sensor device.
