System and methods for endovascular aneurysm treatment

1. A method for treating an aneurysm in a patient, the method comprising:
- measuring a blood pressure of the patient at an aneurysm site;
determining a maximum blood pressure value over a period of time;
positioning at least one double-walled filling structure across the aneurysm, the double-walled filling structure having an inner wall and an outer wall that are conformable;
deploying a scaffold positioned within the double-walled filling structure, the scaffold adapted to allow blood flow therethrough;
filling the at least one double-walled filling structure with a hardenable fluid filling medium so that the outer wall conforms to an inside of the aneurysm and the inner wall conforms to the scaffold so as to form a generally tubular lumen to provide for blood flow, wherein the inner wall is conformable to differing diameters along the generally tubular lumen such that the scaffold shapes the generally tubular lumen to conform an upper and lower end of the tubular lumen to aortic and iliac walls, respectively, wherein the blood flow directly contacts a surface of the inner wall, and wherein the at least one double-walled filling structure substantially fills the aneurysm when filled with the fluid filling medium;
supporting the tubular lumen with the scaffold during filling and/or after the filling of the at least one double-walled filling structure;
monitoring a filling pressure during filling of the at least one double-walled filling structure;
controlling filling of the filling medium into the double-walled filling structure in response to the monitored pressure so as to maintain filling pressure at or above the maximum blood pressure value and below a threshold pressure within a range of about 100% to 140% of the patient'"'"'s blood pressure; and
supporting the tubular lumen with the scaffold while the filling medium hardens within the at least one double-walled filling structure.

Abstract
Methods and systems for treating aneurysms using filling structures filled with a curable medium are described herein. Such methods can include positioning a double-walled filling structure across the aneurysm and filling with a filling medium so that an outer wall conforms to the inside of the aneurysm and an inner wall forms a generally tubular lumen to provide for blood flow. The lumen is supported with a balloon or other expandable device during and/or after filling. The pressure within the structure and/or in the space between an external wall of the structure and the aneurysm wall is monitored and a flow of the medium into the structure is controlled responsive to the pressure. The pressure can also be used to determine a filling endpoint.

positioning at least one expandable filling structure within the aneurysm, the filling structure having a conformable inner wall and a conformable outer wall; deploying a support structure positioned within the filling structure, the support structure adapted to allow blood flow therethrough; filling the at least one filling structure with a fluid filling medium so that the outer wall conforms to an inside of the aneurysm and the inner wall conforms to the support structure so as to form a generally tubular lumen to provide for blood flow, wherein the blood flow directly contacts a surface of the inner wall, and wherein the at least one filling structure substantially fills the aneurysm when filled with the fluid filling medium; supporting the tubular lumen with a support structure during filling and/or after the filling of the at least one filling structure; controlling pressurized filling of the filling medium into the filling structure in response to monitored pressure based on a threshold pressure relative a blood pressure of the patient, wherein the threshold pressure is in the range of about 100% to 140% of the patient'"'"'s blood pressure; and supporting the tubular lumen with the support structure while the filling medium hardens within the at least one filling structure. - View Dependent Claims (24, 25)
