Abstract

The present disclosure provides a provisional tibial prosthesis system for a set of prosthetic knee joints for implantation in a natural knee, the provisional tibial prosthesis system including a bearing component and a bearing support, the spacing of the bearing component from the bearing support is adjustable to allow for representation of a variety of different sized final tibial prostheses. In this system, only one provisional bearing component corresponding to each level of constraint is needed and shims are used to adjust the spacing of the bearing component from the bearing support. The shims are slidably insertable between the bearing component and the bearing support in an anterior/posterior direction to allow for adjustment of the spacing of the bearing component from the bearing support. The number of provisional components needed during knee surgery is reduced and adjustment of the system only requires the knee joint to be distracted by a distance equal to the height of a particular shim.

Claims

1. A tibial prosthesis system for implantation on a resected surface of a proximal tibia, the tibial prosthesis system comprising:

a bearing component having a superior articulating surface and an inferior surface;

a base component having a superior surface and an inferior surface, the base component having a first feature extending from the superior surface and configured to mate with a corresponding second feature on the inferior surface of the bearing component, the first feature and the second feature configured to allow the bearing component to be movable relative to the base component in a proximal/distal direction and the first feature and the second feature configured to prevent significant relative movement between the bearing component and the base component in a medial/lateral direction and an anterior/posterior direction; and

a shim component configured to be received between the inferior surface of the bearing component and the superior surface of the base component, the shim component configured to set a relative proximal/distal position between the inferior surface of the bearing component and the superior surface of the base component.

2. The system of claim 1, further comprising a tibial baseplate having a bone contacting surface adapted to interface with the resected surface of the proximal tibia and an opposing baseplate superior surface, wherein the base component is configured to be coupled to the tibial baseplate.

3. The system of claim 1, wherein the first feature comprises a protrusion of the base component and the second feature comprises a bearing cavity configured to receive the protrusion.

The system of claim 3, wherein the protrusion includes one or more positioning features configured to mate with corresponding second positioning features of the bearing component, and wherein walls of the of bearing cavity are configured to prevent significant relative movement between the base component and the bearing component in a medial/lateral direction and the second positioning features are configured to prevent significant relative movement between the base component and the tibial bearing component in an anterior/posterior direction.

The system of claim 3, wherein an inferior portion of the protrusion has at least one indentation, and wherein a notch of each of the plurality of shim components is configured to engage with the at least one indentation to prevent significant relative movement between the bearing component and the base component in the proximal/distal direction upon insertion of each of the plurality of shim components between the bearing component and the base component.

6. The system of claim 1, wherein the shim component is configured to be insertable and removable from between the inferior surface of the bearing component and the superior surface.

7. The system of claim 6, wherein the shim component is selected from a plurality of shim components including shim components having different proximal/distal heights relative to each other.

The system of claim 7, wherein each of the plurality of shim components has rails with at least one wall tapered from a proximal end portion to a distal end portion, wherein the rails are configured to engage with slots on the inferior surface of the bearing component to prevent significant relative movement between the bearing component and the base component in the proximal/distal direction upon insertion of each of the plurality of shim components between the bearing component and the base component.

The system of claim 7, wherein each of the plurality of shim components is configured to restrain the base component and the bearing component from relative movement in the proximal/distal direction upon insertion of each of the plurality of shim components and at least one of the bearing component and the base component is configured to retain each of the plurality of shim components from movement in the proximal/distal direction and one of a medial/lateral direction and an anterior/posterior direction upon insertion.

10. The system of claim 7, wherein each of the plurality of shim components is configured for insertion between and removal from the bearing component and the base component in substantially only a single direction.

The system of claim 7, wherein a selection of each of the plurality of shim components for use with the bearing component and the base component includes a determination of which of the plurality of shim components creates a proper spacing between the bearing component and the base component when the bearing component, the base component and the tibial baseplate are positioned on the resected surface of the proximal tibia.

12. A method of performing a surgical procedure on a portion of a knee joint, the method comprising:

implanting a base component on a resected tibia;

coupling a first shim component and a bearing component to the base component, the bearing component and base component having mating features configured to prevent significant relative movement between the bearing component and the base component in a medial/lateral direction and an anterior/posterior direction, the first shim component configured to restrain the base component and the bearing component from relative movement in a proximal/distal direction; and

performing range of motion testing to verify proper sizing of a combination of the bearing component, the base component, and the first shim component.

13. The method of claim 12, wherein the step of coupling comprises inserting the first shim component between the bearing component and the base component.

14. The method of claim 13, wherein the step of coupling comprises initially coupling the bearing component to the base component prior to inserting the first shim component between the bearing component and the base component.

15. The method of claim 14, wherein the mating features are configured to allow the bearing component to be movable relative to the base component in a proximal/distal direction to accommodate insertion and removal of the first shim component.

16. The method of claim 12, wherein when the testing indicates that the tibial prosthesis is not properly sized, comprising:

removing the first shim component from between the bearing component and base component; and

inserting a second shim component between the bearing component and the base component, the second shim component having a different proximal/distal size than the first shim component.

17. The method of claim 12, wherein at least one of the bearing component and the base component is configured to retain the first shim component from movement in the proximal/distal direction and one of a medial/lateral direction and an anterior/posterior direction.

18. A tibial prosthesis system for implantation on a resected surface of a proximal tibia, the tibial prosthesis system comprising:

a bearing component having a superior articulating surface and an inferior surface;

a base component having a superior surface and an inferior surface; and

a plurality of shim components including shim components having different proximal/distal heights relative to each other, each shim component is configured to be received between the inferior surface of the bearing component and the superior surface of the base component and couple the tibial prosthesis system together as an assembly, wherein when the tibia prosthesis system is coupled together as the assembly each shim component and the bearing component are restrained from substantial movement in two or more of a medial/lateral direction, an anterior/posterior direction and a proximal/distal direction.

The system of claim 18, wherein the base component has a first feature configured to mate with a corresponding second feature on the inferior surface of the bearing component, the first feature and the second feature are configured to allow the bearing component to be movable relative to the base component in a proximal/distal direction to accommodate each shim component and the first feature and the second feature are configured to prevent significant relative movement between the bearing component and the base component in a medial/lateral direction and an anterior/posterior direction.

20. The system of claim 18, wherein each shim component is configured to set a relative proximal/distal position between the inferior surface of the bearing component and the superior surface of the base component.

21. The system of claim 18, wherein each shim component is configured to be insertable and removable from between the inferior surface of the bearing component and the superior surface.
