1. A disposable apparatus for use in filtering blood comprising:
- a molded plastic substrate having a first flow path and a second flow path formed therein, wherein the first flow path and second flow path are fluidically isolated from each other, wherein the molded plastic substrate comprises a plurality of concave spaces;
a hemofilter cartridge positioned within a first of the plurality of concave spaces and in fluid communication with the first flow path and the second flow path;
at least one sensor positioned within a second of the plurality of concave spaces;
a plurality of ports integrated into the molded plastic substrate; and
embedded data, wherein the embedded data uniquely identifies the disposable apparatus and wherein the disposable apparatus is caused to be unusable based on the embedded data.
