Abstract

A health care management system and method for chronic disease management such as diabetes management, hypertension management and like chronic diseases. The system is utilized by both patients and clinicians to manage the patients'"'"' chronic diseases and for clinicians to better manage a population of patients.

Claims

1. A medical system comprising:

a database of patient information accessible by both a patient and a clinician to monitor the management of the patient'"'"'s disease, the database including information relating to the patient'"'"'s health, education, diet and exercise.

2. A medical system comprising:

a database of information relating to the management of a patient'"'"'s disease including medical, education, exercise and diet information;

a patient component accessible by a patient and in communication with the database to allow the patient access to their medical, education, diet and exercise information; and

a clinician component accessible by a clinician and in communication with the database to allow the clinician access to the patient'"'"'s medical, education, diet and exercise information.

3. A medical system comprising:

a database containing information relating to a patient'"'"'s disease including medical, education, exercise and diet information;

a patient component into which biological data from medical devices is uploadable, the patient component being accessible by a patient and in communication with the database to provide the patient their medical, education, exercise and diet information; and

a clinician component accessible by a clinician and in communication with the database to provide the clinician with the patient'"'"'s medical, education, exercise and diet information.

4. A medical system comprising:

a plurality of patient components, each accessible by an individual patient to provide each patient access to their individual medical data; and

a clinician component accessible by a clinician and in communication with the plurality of patient components, the clinician component enabling the clinician to provide disease management to the patients as individuals or as part of clinician defined groups.

5. A web-based medical system comprising:

a web server;

a database server;

a plurality of patient residence systems in communication with the web server, each accessible by an individual patient to manage a disease; and

at least one clinician management system in communication with the web server, the clinician management system enabling the clinician to manage the disease of patients individually or as a group.

6. A medical system comprising:

a plurality of patient residence systems, each accessible by an individual patient to manage a disease including aspects of health, education, diet and exercise; and

at least one clinician management system in communication with the plurality of patient residence systems, the clinician management system enabling the clinician to manage the disease of each patient individually or as a group.

7. A diabetes management system comprising:

a database containing information relating to a patient'"'"'s diabetes including medical, education, exercise and diet information;

a patient component into which biological data from a glucose meter is uploadable, the patient component being accessible by a patient and in communication with the database to provide the patient their medical, education, exercise and diet information; and

a clinician component accessible by a clinician and in communication with the database to provide the clinician with the patient'"'"'s medical, education, exercise and diet information enabling the clinician to manage the diabetes of the patient.

8. A diabetes management system comprising:

a plurality of patient residence systems, each accessible by an individual patient to manage their diabetes including aspects of health, education, diet and exercise; and

at least one clinician management system in communication with the plurality of patient residence systems, the clinician management system enabling the clinician to manage the diabetes of patients individually or as a group.

9. A diabetes management system comprising:

a database of patient information accessible by both a patient and a clinician to monitor the management of the patient'"'"'s diabetes, the database including information relating to the patient'"'"'s health, education, diet and exercise.

10. A diabetes management system comprising:

a web server;

a plurality of patient residence systems in communication with the web server, each accessible by an individual patient;

at least one clinician management system in communication with the web server, the clinician management system enabling a clinician to manage the diabetes of patients individually or as a group; and

a database server in communication with the web server, the database server containing information relating to health, education, diet and exercise for each patient such information available to the individual patients and the clinician.

11. A medical system comprising:

a database of patient information accessible by both a patient and a clinician to monitor the management of the patient'"'"'s disease, the database including information relating to the patient'"'"'s health and education to assist the clinician in monitoring the patient'"'"'s health and education relative to their disease.

12. A medical system comprising:

a database of patient information accessible by both a patient and a clinician to monitor the management of the patient'"'"'s disease, the database including information relating to the patient'"'"'s health and diet.

13. A medical system comprising:

a database of patient information accessible by both a patient and a clinician to monitor the management of the patient'"'"'s disease, the database including information relating to the patient'"'"'s health and exercise.

14. A medical system comprising:

a database of patient information accessible by both a patient and a clinician to monitor the management of the patient'"'"'s disease, the database including information relating to the patient'"'"'s health, education, diet and exercise, and the database accessible by other databases in a healthcare environment.

15. A diabetes management system comprising:

a plurality of patient residence systems, each accessible by an individual patient to manage their diabetes including aspects of health, education, diet and exercise; and

at least one clinician management system in communication with the plurality of patient residence systems, the clinician management system enabling the clinician to acquire group data relating to the health of the plurality of patients.
