Abstract

Improved assemblies, systems, and methods provide a stimulation system for prosthetic or therapeutic stimulation of muscles, nerves, or central nervous system tissue, or any combination. The stimulation system may include both external and internal components, with an implantable pulse generator sized and configured to be implanted subcutaneously in a tissue region. A lead having a proximal end and a distal end is coupled to the implantable pulse generator and is adapted for implantation in subcutaneous tissue. The distal end of the lead is adapted to be remote from the pulse generator and capable of extending up to an anatomical furthest distance from the pulse generator. The circuitry carried within the case is operable for generating electrical stimulation pulses and capable of electrically driving the generated electrical stimulation pulses from the pulse generator through the lead extending up to the anatomical furthest distance.

Claims

1. A pulse generator system adapted to provide electrical stimulation pulses to tissue in a body, the system comprising

a pulse generator adapted for implantation in subcutaneous tissue,

a lead adapted for implantation in subcutaneous tissue, the lead having a proximal end and a distal end, the proximal end adapted to be electrically coupled to the pulse generator, and the distal end adapted to be remote from the pulse generator and capable of extending up to an anatomical furthest distance from the pulse generator, and

circuitry carried within the pulse generator, the circuitry operable for generating electrical stimulation pulses and capable of electrically driving the generated electrical stimulation pulses from the pulse generator through the lead extending up to the anatomical furthest distance.

2. A pulse generator according to claim 1

wherein the pulse generator is adapted to be implanted in subcutaneous tissue at an implant depth of between about five millimeters and about twenty millimeters.

3. A pulse generator according to claim 1

wherein the circuitry includes a power supply adapted to generate up to about 27 volts DC in order to electrically drive the generated stimulation pulses.

4. A pulse generator according to claim 3

wherein the power supply is programmable.

5. A pulse generator according to claim 1

the pulse generator further comprising a housing sized and configured for implantation in subcutaneous tissue, the housing having a thickness of between about 5 mm and 15 mm, a width of between about 30 mm and 60 mm, and a length of between about 45 mm and 60 mm.

6. A pulse generator according to claim 5

wherein the housing includes a connection header coupled to the circuitry, the connection header being sized and configured to accept an IS-1 standard plug-in lead connector.

7. A pulse generator according to claim 5

further including an antenna coupled to the circuitry and carried entirely within the housing to enable communication with a remote device by wireless telemetry.

8. A pulse generator according to claim 5

wherein the housing includes a smaller end and a larger end to facilitate placement within a subcutaneous pocket small end first.

9. A pulse generator according to claim 5

wherein the lead includes one or more monopolar electrodes implanted in subcutaneous tissue remote from the pulse generator, and the pulse generator housing comprises a return path for the electrical stimulation pulses.

10. A pulse generator according to claim 5

wherein the lead includes one or more bipolar electrodes implanted in subcutaneous tissue remote from the pulse generator, and the pulse generator housing being excluded from comprising a return path for the electrical stimulation pulses.

11. A method comprising

providing a pulse generator,

providing a lead having an electrode,

selecting a proximal position for implantation of the pulse generator,

selecting a distal position for implantation of the lead, the distal position capable of being up to an anatomic furthest distance from the proximal position,

implanting the pulse generator at the proximal position in subcutaneous tissue,

implanting the lead at the at the distal position in subcutaneous tissue,

coupling the pulse generator to the lead, and

operating the pulse generator to apply electrical stimulation pulses to the electrode to treat a selected physiologic condition.

12. A method according to claim 11

wherein the physiologic condition is selected from the group consisting of urinary incontinence, fecal incontinence, micturition/retention, defecation/constipation, restoration of sexual function, pelvic floor muscle activity, pelvic pain, obstructive sleep apnea, deep brain stimulation, pain management, heart conditions, gastric function, and restoration of motor control.

13. A method according to claim 11 further including

implanting the pulse generator in subcutaneous tissue at an implant depth of between about five millimeters and about twenty millimeters.

14. A pulse generator system adapted to provide electrical stimulation pulses to tissue in a body, the system comprising

a pulse generator adapted for implantation in subcutaneous tissue,

a first lead length for implantation in a first target site in a body,

a second lead length for implantation in a second target site in a body, the second lead length having a proximal end and a distal end, the proximal end adapted to being electrically coupled to the pulse generator, and the distal end adapted to be remote from the pulse generator and capable of extending up to an anatomical furthest distance from the pulse generator, and

circuitry carried within the pulse generator, the circuitry operable for generating electrical stimulation pulses and capable of electrically driving the generated electrical stimulation pulses through the first lead length and/or the second lead length extending up to the anatomical furthest distance.

15. A pulse generator according to claim 14

wherein the pulse generator adapted to be implanted in subcutaneous tissue at an implant depth of between about five millimeters and about twenty millimeters.
